Development of siRNAs targeting interferon beta as a novel therapeutic approach for adult and juvenile dermatomyositis.

BACKGROUND: A key feature of the autoimmune conditions adult and juvenile dermatomyositis (DM/JDM) is the correlation of interferon (IFN) beta induction with disease initiation and activity. Our aim was to establish a short interfering RNA (siRNA) approach to knockdown IFNB1 expression as a therapeutic strategy for DM/JDM. METHODS: A library of 74 siRNAs targeting IFNB1 was designed and screened for IFNB1 knockdown by qRT-PCR. Lead siRNAs were analysed for secreted IFN beta protein reduction and acute cytotoxicity. IFNB1 expression was induced for efficacy screening in vitro, including in imm

Of Pompe and Circumstance: A Case Review of Late Onset Pompe Disease in a 52-Year-Old Male.

Late-onset Pompe disease (LOPD) is a rare glycogen storage disorder caused by deficiency of the enzyme lysosomal acid alpha-glucosidase (GAA). It can mimic inflammatory myopathies, leading to diagnostic delays. We present a case of LOPD in a 52-year-old man who came to us with a diagnosis of dermatomyositis for which he had been undergoing treatment for over a decade. Although he was on seemingly adequate immunosuppression, the patient continued to have progressive proximal muscular weakness during the first 4 years of our care. It was not until his sister was diagnosed with LOPD that we pursu

Persistent HyperCKemia Beyond Post-Viral Myositis: A Case of Anti-HMGCR Immune-Mediated Necrotizing Myopathy.

Anti-3-hydroxy-3-methylglutaryl-coenzyme A reductase (anti-HMGCR) immune-mediated necrotizing myopathy (IMNM) is a rare autoimmune myopathy that should be suspected in patients with progressive proximal muscle weakness and marked creatine kinase (CK) elevation persisting despite statin discontinuation, although distinguishing it from more common conditions such as post-viral myositis and toxic statin-associated myopathy remains a major diagnostic challenge. We report the case of a 64-year-old man receiving long-term atorvastatin therapy who presented with progressive proximal muscle weakness,

Trends in Hospitalizations of Patients with Idiopathic Inflammatory Myopathies: Long-Term Observational Study, 2010-2024.

Background/Objectives: Idiopathic inflammatory myopathies (IIMs) are very rare, chronic, immune-mediated diseases that clinically manifest with myositis symptoms and extramuscular manifestations. The rarity of the disease makes it difficult to conduct double-blind studies, and large population-based studies are needed to improve our knowledge of them. This study aims to analyze the hospitalization rates and trends of IIMs between 2010 and 2024. Additionally, a characterization of the IIM patient population in Poland was conducted. Methods: A retrospective study using data from the Nationwide G

Extracardiovascular immune-related adverse events in patients with suspected cardiac toxicity associated with immune checkpoint inhibitors.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer therapy but may cause immune-related adverse events (iRAEs). Cardiovascular iRAEs, especially myocarditis, carry high morbidity and mortality. Extracardiovascular iRAEs are included as diagnostic criteria for myocarditis, yet their prevalence, spectrum, and clinical impact in patients with suspected cardiovascular iRAEs are poorly defined. METHODS: We conducted a prospective, single-center observational study between March 2022 and December 2023. Consecutive patients referred to cardio-oncology for suspected ICI-related c

Necrotizing Myopathies: A Myopathology Experience from a Single Tertiary Care Center.

BACKGROUND AND OBJECTIVES: Necrotizing myopathy (NM) is a rare inflammatory myopathy characterized by prominent necrosis and minimal to absent inflammation, arising from heterogeneous immune- and non-immune-mediated causes. Muscle biopsy aids diagnosis, especially in seronegative cases and those with overlapping presentations, helping to distinguish NM from other idiopathic inflammatory myopathies (IIMs). This tertiary center study examines the myopathologic spectrum in NM, correlating histopathologic findings with clinical features and autoantibody profiles. METHODS: We studied the records a

Exploring the landscape of flagellate erythema as a rare cutaneous symptom of idiopathic inflammatory myopathy: a case series-based review.

Flagellate erythema is an exceptionally rare cutaneous manifestation of idiopathic inflammatory myopathies (IIM) with poorly understood clinical significance. We report two patients who developed zebra-like flagellate erythema: one with immune-mediated necrotising myopathy (IMNM) and one with anti-Mi-2-positive cancer-associated myositis (CAM). To place these observations in broader context, we performed a thorough literature search across PubMed, Scopus and Embase and identified 34 previously published cases of IIM-associated flagellate erythema. The most frequent underlying diagnosis was der

Suspected immune-mediated inflammatory myositis with rhabdomyolysis following yellow fever vaccination: a case report.

BACKGROUND: The yellow fever vaccine is a live-attenuated vaccine with an excellent safety profile; however, rare adverse events have been reported. Musculoskeletal complications, particularly inflammatory myositis with rhabdomyolysis, are exceedingly uncommon and not well characterized in the literature. CASE PRESENTATION: We report a case of a 49-year-old man who developed acute febrile illness, severe myalgia, and progressive proximal muscle weakness 1 week after receiving his first yellow fever vaccination. An extensive diagnostic evaluation was performed. The patient presented with high-

A rare case of ultrastructurally proven toxoplasmic myositis masquerading as necrotizing myopathy with emphasis on pathobiology.

Toxoplasmosis is a zoonotic infection caused by the obligate protozoan Toxoplasma gondii. In an immunocompetent human host, the parasite forms bradyzoites (tissue cysts), mainly in skeletal muscle, eye, brain, or cardiac muscle, and remains latent. Once cysts are established, type 1 cytokine-based immunity controls the conversion to tachyzoites from bradyzoites. We propose that short-term immunosuppression can only cause a transient defect in this cycle of conversion control. The temporary lapse in conversion control releases few tachyzoites, and the host immunity in response gives rise to cyt

Diagnostic Utility of USP6 RNA Chromogenic In Situ Hybridization in the Differential Diagnosis of Nodular Fasciitis and Aneurysmal Bone Cyst.

Recurrent USP6 gene rearrangements underlie a subset of mesenchymal neoplasms, including nodular fasciitis and aneurysmal bone cyst. These rearrangements place USP6 under the regulatory control of another gene's constitutively active promoter, resulting in USP6 overexpression. Recently, it has been demonstrated in a limited number of cases that detection of USP6 overexpression by RNA chromogenic in situ hybridization (CISH) can serve as a surrogate marker for USP6-rearranged tumors. To further assess the diagnostic utility of USP6 RNA CISH, we evaluated its application in 1210 bone and soft ti

Nontraumatic Myositis Ossificans Adjacent to the Femoral Nerve: A Ubiquitin-Specific Peptidase 6 (USP6)-Rearranged Lesion Mimicking Malignancy.

Myositis ossificans (MO) is a benign, self-limiting lesion characterized by heterotopic ossification that can mimic malignancy both radiographically and histologically. Non-traumatic MO is particularly challenging to diagnose, especially when arising adjacent to major neurovascular structures. Recent studies have identified ubiquitin-specific peptidase 6 (USP6) gene rearrangements in a subset of MO, linking it to a family of transient fibroblastic/myofibroblastic neoplasms. We present a 21-year-old female who presented with progressive right lower extremity pain and weakness without a history

Interlaboratory comparison of immunoprecipitation-based testing for myositis-specific autoantibodies.

INTRODUCTION: Myositis-specific autoantibodies (MSAs) are clinically important biomarkers that help classify idiopathic inflammatory myopathies and guide clinical management, but reliable detection remains challenging. Immunoprecipitation (IP) is widely regarded as a reference method for MSA detection in specialist laboratories. However, IP comprises a range of related techniques rather than a single standardised assay, and variability between expert laboratories using different IP-based workflows is not well characterised. METHODS: Serum samples previously submitted to the Bath Diagnostic Se

Responsiveness and clinical correlates of continuous turns-amplitude analysis measures in myositis: A prospective longitudinal study.

OBJECTIVE: Turns-amplitude analysis (TAA) is a rapid quantitative electromyographic technique used in neuromuscular disorders, but its categorical interpretation (normal, myopathic, neurogenic) limits the granularity needed for longitudinal monitoring. We investigated whether continuous TAA-derived metrics could reflect longitudinal clinical change in idiopathic inflammatory myopathies (IIM). METHODS: In this prospective single-center study, 30 adults with IIM underwent TAA of biceps brachii, quadriceps, and iliopsoas at baseline, 6, and 12 months. From each cloud, we extracted descriptors of

Patients know best: DESTINIES Patient Panel appraisal of a novel, risk-stratified approach to COVID-19 disease surveillance in the adult immunosuppressed.

The immunosuppressed are poorly represented in disease surveillance and pharmacovigilance dataflows, proving especially consequential during the COVID-19 pandemic. This mixed methods study recruited a panel of immunosuppressed patients (DESTINIES Patient Panel) to quality assure a new digital phenotype for COVID-19 risk in the adult immunosuppressed (DESTINIES phenotype) prior to its implementation in real-world surveillance. Applying their lived experiences, the DESTINIES Patient Panel met to critique the strategy and structure of the provisional phenotype before voting on its adoption in UK

Myositis Ossificans Affecting the Rotator Cuff Muscles and Proximal Humerus: A Rare Clinical Entity.

INTRODUCTION: Myositis Ossificans (MO) is a benign condition characterized by the formation of non-neoplastic heterotropic ossification in extra-skeletal soft tissues. It is generally associated with preceding trauma; however, cases of atraumatic MO have also been reported in the literature. While the elbow is the most common site, MO can occur anywhere in the body and is particularly common in young male athletes in the second to third decade of life. MO involving the rotator cuff muscles is an extremely rare entity with its pathology poorly understood till date. In such cases where it origin

A Case of Eosinophilic Granulomatosis With Polyangiitis Presenting With Conjunctival Lesions During Treatment With IL-5 Inhibitors.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic rheumatic disease characterized by necrotizing vasculitis in the small vessels with eosinophilic inflammation. EGPA involves multiple organs, such as the heart, lungs, kidneys, gastrointestinal tract, nerves, and ophthalmic organs. Ophthalmic involvement includes conjunctival nodules, orbital myositis, orbital inflammatory syndrome, dacryoadenitis, and cranial nerve palsy. Notably, conjunctival nodules in patients with EGPA are very rare lesions but may present with atypical eyelid swelling. Herein, we present a case of EGPA wi
