Paraneoplastic Autoimmune Multiorgan Syndrome: Diagnostic Lessons for Pediatricians.

PURPOSE: Persistence of skin and wound infections is nowadays accepted being linked to bacterial biofilms, which are highly recalcitrant to treatments and contribute to maintain a constant inflammation state and prevent a correct healing. Topical antimicrobials are the most common first-line self-medications; however, treatment failure is not uncommon and emerging resistance to antibiotics is alarming. Chlorquinaldol is an antimicrobial with a wide spectrum of activity and desirable characteristics for topical application. Aim of this study was to evaluate the efficacy of chlorquinaldol to pre

Potential mechanisms linking polyethylene terephthalate microplastic-associated exposure factors to abnormal biomineralization.

Microplastic exposure has raised health concerns. However, whether degradation products and migrants associated with polyethylene terephthalate microplastics (PET-MPs) contribute to diseases associated with abnormal biomineralization remains unclear. Here, terephthalic acid (TPA), ethylene glycol (EG), mono(2-hydroxyethyl) terephthalate (MHET), bis(2-hydroxyethyl) terephthalate (BHET), PET cyclic trimer, acetaldehyde, and antimony trioxide (Sb₂O₃) were defined as PET-MP-associated exposure factors (PAEFs). This study establishes a cross-disease biomineralization framework separates PET-MP part

Radioulnar Synostosis After Isolated Undisplaced Distal Ulna Fracture: A Rare Case Report and Review of the Literature.

CASE: A 50-year-old man developed radioulnar synostosis and presented with restricted forearm rotation, 15 months after sustaining an undisplaced fracture of the ulna. Radiographs and computed tomography revealed a mature bony bridge between the forearm bones. Surgical excision of the heterotopic bone was performed, and bone wax was applied to mitigate hemorrhage and prevent recurrence. Postoperatively, the patient achieved near-complete pronation and supination and no recurrence was found at the end of 2 years of follow-up. CONCLUSION: Radioulnar synostosis is an infrequent complication asso

Metastatic melanoma presenting with paraneoplastic dermatomyositis mimicking polymyalgia rheumatica.

A man in his late 70s presented with 6-week bilateral shoulder and hip girdle pain, morning stiffness and significant weight loss. Although polymyalgia rheumatica was initially suspected, red flag weight loss prompted referral through the 2 week-wait cancer pathway with parallel laboratory and imaging assessment.Markedly elevated creatine kinase (>5000 U/L), proximal weakness and subtle cutaneous features raised suspicion of dermatomyositis. A CT scan then demonstrated extensive intra-abdominal and right inguinal lymphadenopathy, initially suggesting lymphoma and prompting haematology referral

Paraneoplastic dermatomyositis preceding detection of metastatic oesophageal squamous cell carcinoma after prolonged remission following pembrolizumab therapy.

Dermatomyositis is a recognised paraneoplastic inflammatory myopathy although its association with oesophageal malignancy is uncommon. Most reported cases either precede or accompany the initial diagnosis of malignancy. We report a man in his late fifties with previously treated oesophageal squamous cell carcinoma who developed florid dermatomyositis after approximately 6 years of sustained remission following chemotherapy, radiotherapy and pembrolizumab therapy. He presented with progressive proximal muscle weakness, dysphagia and characteristic cutaneous manifestations accompanied by markedl

Efficacy and safety of garetosmab, an activin A-blocking antibody, in fibrodysplasia ossificans progressiva (OPTIMA): a randomised, double-blind, placebo-controlled, phase 3 trial.

BACKGROUND: Fibrodysplasia ossificans progressiva (FOP) is an ultrarare genetic disorder resulting in progressive heterotopic ossification of ligaments, tendons, and muscles, leading to disability and early mortality. FOP is caused by heterozygous gain-of-function mutations in the activin receptor type 1A, allowing this receptor to be aberrantly activated by activin A (ActA), which normally inhibits this receptor, thus driving heterotopic bone formation. In preclinical models and an earlier phase 2 trial (LUMINA-1) in FOP, an antibody blocking ActA (garetosmab) prevented heterotopic ossificati

Back to the Basics: When Radiographs Solve the Case.

Myositis ossificans circumscripta is a benign, self-limiting heterotopic ossification usually occurring after a traumatic event. Our case describes a rare etiology given the occurrence after vaccination in a child. Indeed, to date, only two cases of myositis ossificans circumscripta after COVID-19 vaccination have been reported, both in adults. In this first pediatric case, radiographs performed at appropriate time allowed for a conclusive tissue characterization and hence diagnosis. CT and MR scans performed early after clinical manifestation showed only progressive, severe edema of the soft

Tissue calcinosis in dermatomyositis - potential role of combination therapy with JAK inhibitors. Case report and literature review.

Calcinosis is a disabling and difficult-to-treat complication of adult dermatomyositis (DM), juvenile dermatomyositis (JDM), and systemic sclerosis (SSc), for which no standardized therapy is currently available. Growing evidence implicating JAK/STAT signaling in dermatomyositis has prompted interest in Janus kinase inhibitors (JAKi) as a therapeutic option. We describe a 52-year-old woman with long-standing juvenile-onset dermatomyositis and extensive, treatment-refractory calcinosis who received off-label baricitinib after multiple previous therapies. After one month, the patient reported im

Myositis autoantibodies: from new specificities to new paradigms.

PURPOSE OF REVIEW: Myositis-specific autoantibodies have long been used to classify patients into clinically meaningful subsets; however, recent studies show that the field is moving beyond the simple question of which autoantibody is present. This review examines how emerging specificities, epitope architecture, autoantibody coexistence, pathogenic mechanisms, and detection platforms are reshaping the interpretation of myositis serology. RECENT FINDINGS: Recent work has expanded the autoantibody repertoire through the identification of additional autoantibody specificities, multiprotein anti

Troponin T versus troponin I in immune checkpoint inhibitor-associated myocarditis: Complementary roles in screening and confirmation.

Immune checkpoint inhibitor-associated myocarditis is a rare but highly fatal complication. Cardiac troponin T (cTnT) and cardiac troponin I (cTnI) are central to diagnosis and prognostic assessment, yet their clinical differences remain poorly defined. This review compares the biological characteristics, analytical factors, and clinical interpretation of cTnT and cTnI in ICI-associated myocarditis. Available evidence suggests that cTnT is more frequently elevated and has a stronger evidence base for surveillance, whereas cTnI provides complementary information for myocardial source attributio

Pulmonary hypertension in anti-synthetase syndrome: prevalence, comparative risk across autoimmune diseases, and impact on survival.

OBJECTIVES: Anti-synthetase syndrome (ASyS) is a distinct entity within the myositis spectrum characterized by interstitial lung disease (ILD) which raises the concern of increased pulmonary vascular pressures. However, the prevalence of pulmonary hypertension (PH) in real-world ASyS populations remains unclear. We investigated the prevalence and impact of PH in ASyS. METHODS: Data were obtained from a world-wide, physician-reported multicenter registry based on physician-diagnosed ASyS and/or a positive anti-synthetase antibody. Comparative analyses and multivariable logistic regression asse

Intravascular Large B-Cell Lymphoma Presenting with Rhabdomyolysis and Inflammatory Myositis: A Diagnostic Challenge.

INTRODUCTION: Intravascular large B-cell lymphoma (IVLBCL) is a rare and aggressive extranodal lymphoma characterised by the proliferation of lymphoma cells within small blood vessels. Distinctively, IVBCL typically lacks the lymphadenopathy, discrete tumour masses, or circulating lymphoma cells characteristic of other lymphomas. This results in substantial diagnostic delays and historically high rates of postmortem diagnosis. Common clinical features of IVLBCL include fever of unknown origin, cytopenias, elevated lactate dehydrogenase, and neurological dysfunction. This case highlights the di

One more opportunity in early-stage cervical cancer: the role of photodynamic therapy in managing positive surgical margins.

OBJECTIVE: To evaluate recurrence of squamous intraepithelial lesions (SILs) and human papillomavirus (HPV) infection after photodynamic therapy (PDT) in patients with early-stage cervical cancer (CC) and positive surgical margins. METHODS: We retrospectively analyzed 46 patients aged 25 to 52 years with stage IA-IB invasive CC treated with PDT. Chlorin e6-based photosensitizers were used to irradiate both the cervical canal and surface with standardized doses. Patients were stratified by residual tumor status (R1-2 vs. R0/SIL+). Outcomes were assessed by HPV testing, cytology and pelvic magn

The ambrosial mycobiota of Treptoplatypus oxyurus (Coleoptera, Platypodidae): a unique island of fungal diversity revealing Wilhelmdebeerea oxyuri gen. et sp. nov. (Ophiostomatales), and two new yeast species Blastobotrys sasensis sp. nov., and Sugiyamaella casensis sp. nov. (Dipodascales).

Ambrosia beetles (Coleoptera, Curculionidae) form obligate nutritional symbioses with ambrosia fungi cultivated within their galleries. Among them, the pinhole borers (Platypodinae) are predominantly tropical, with only two representatives native to Europe. One of them, the rare and understudied Treptoplatypus oxyurus, primarily colonises Abies alba. We investigated its fungal symbionts using a cultivation-dependent approach. We identified three numerically dominant associates in the prothorax containing mycangia: Candida schatavii, Magnusiomyces fungicola, and a novel member of Ophiostomatale

A retrospective metatranscriptomic study of respiratory pathogens causing adult community-acquired pneumonia in Wuxi, China, before the pandemic.

The COVID-19 pandemic has substantially altered respiratory pathogen circulation, underscoring the critical need for pre-pandemic baseline data to interpret current epidemiological trends. To establish this baseline, we employed metatranscriptomic sequencing to characterize the etiology of community-acquired pneumonia (CAP) in 20 adult patients hospitalized in Wuxi, China, during 2018-2019. Following ribosomal RNA depletion, sequencing data were analyzed using a stringent dual-filter strategy (RPM ≥ 100 and Z-score ≥ 2) to identify high-confidence pathogens. Our analysis revealed a complex, po

The Function of Poly (U) Binding Splicing Factor 60 (PUF60) in Disease Regulation.

The alternative splicing (AS) of pre-mRNA is an important process in controlling the expression of human genes, which can enrich the diversity of the proteome and regulate gene function. On the contrary, aberrant splicing contributes significantly to numerous human diseases progression, including tumors, neurological diseases, metabolic diseases, infections, and immune diseases. The PUF60, a protein related to RNA splicing, plays critical functions in RNA splicing and gene transcription regulation. In addition, it can achieve synergistic binding with U2AF65 on RNA through interactions in the p

Secondary Limb-Salvage Fifty Years After Vascular Injury: A Case Report.

A 73-year-old veteran presented to vascular surgery with a nonhealing left lower leg wound. Fifty years before the presentation, he sustained a gunshot wound to the left leg during combat. Emergent superficial femoral artery to popliteal artery bypass using vein graft and complex bone and soft-tissue repair resulted in successful primary limb salvage. He was entered into the Vietnam Vascular Registry at the time of his initial injury. On current presentation, he was diagnosed with infected myositis ossificans and bypass occlusion. He was originally offered wound care alone with possible amputa
