OBJECTIVES: Anti-MDA5 autoantibodies are strongly associated with interstitial lung disease (ILD) and rapidly progressive ILD (RP-ILD) in Asian patients with dermatomyositis (DM) or amyopathic DM (ADM). However, this association has not yet been established in Brazilian patients with anti-MDA5(+) DM/ADM. This study aimed to investigate the phenotypic differences between Brazilian and Japanese patients with anti-MDA5(+) DM/ADM, with a particular focus on ILD.
METHODS: This was an international, tricentric, retrospective cohort study conducted in one Brazilian and two Japanese tertiary centres.

OBJECTIVES: This study aimed to establish a risk prediction model for the relapse of anti-synthetase syndrome-associated interstitial lung disease (ASyS-ILD).
METHODS: Patients diagnosed with ASyS-ILD and treated with prednisolone and calcineurin inhibitors as remission induction therapy were enrolled in the Japanese multicentre MYKO cohort. Antisynthetase syndrome accounts for one-third of cases of idiopathic inflammatory myopathy. It is characterised by the presence of serum antibodies against aminoacyl-tRNA synthetase and most often manifests as interstitial lung disease, myositis and/or arthritis. We report a 67-year-old woman with gait disturbance and muscle weakness, which worsened over 7 years, especially during intermittent illnesses with fever. Extensive neurophysiological and immunological examination led to a diagnosis of antisynthetase syndrome. She improved with corticosteroids and methotrexate.

METHODS: Patients with DM were evaluated in a prospective study at 2 timepoints for skin rash assessment using the Cutaneous Dermatomyositis Disease Area and Severity Index (CDASI) and 3D images. A 3D image disease activity score (3DAS) was calculated based on the percentage of the rashes relative to the total body surface area, multiplied by the degree of rash redness.
