Mycophenolate Mofetil Treatment Reduces the Risk of Treatment Escalation Due to Vascular Complications in Limited Cutaneous Systemic Sclerosis: Emulation of a Target Trial From the Italian Rheumatology Society SPRING Registry.

OBJECTIVE: Mycophenolate mofetil (MMF) use in limited cutaneous systemic sclerosis (lcSSc) is relatively uncommon because of the lower fibrotic burden and the predominance of vascular complications. In vitro observations and clinical data from transplanted patients suggest a protective effect of MMF on endothelial function. Our aim was to evaluate the reasons for prescribing MMF treatment in patients with lcSSc and its impact on the need for escalation of vascular complication-related treatments during follow-up. METHODS: Patients with lcSSc enrolled in the Italian Systemic Sclerosis Progress

Immune Checkpoint Inhibitor-Associated Myocarditis With Myositis/Myasthenia Gravis Overlap Syndrome: Two Case Reports Highlighting Early Onset, Diagnostic Uncertainty, and Management Considerations.

Immune checkpoint-inhibitors (ICIs) have revolutionized and significantly improved cancer care through remarkable efficacy, but significant adverse effects have been observed. Among these, ICI-induced myocarditis with myositis/myasthenia gravis overlap syndrome represents a life threatening, severe immune-related adverse event that clinicians need to be aware of and be able to diagnose rapidly given its aggressive course. We present two patients who were treated with ICIs and subsequently developed said syndrome. Both patients experienced significant cardiac and neuromuscular manifestations an

Rapid bedside diagnosis of occult pyomyositis using point-of-care ultrasound in a patient with sepsis of unknown origin.

Pyomyositis is a rare bacterial infection of skeletal muscle characterised by abscess formation, having significant morbidity. Early diagnosis is often challenging, especially in patients with atypical presentations. We report a case of pyomyositis in an immunocompetent adult male, promptly diagnosed using point-of-care ultrasound (POCUS) in the emergency department. The patient presented with fever, hypotension and confusion, and no source of infection was initially identified. Careful re-examination revealed subtle thigh asymmetry and tenderness. POCUS revealed an intramuscular collection in

CAR-T cells in the treatment of neurological autoimmune diseases.

Chimeric antigen receptor T cells (CAR-T cells) have revolutionised cancer treatment by offering personalised therapy of unprecedented efficacy to patients with relapsed B-cell malignancies and myeloma. CAR-T cells are designed selectively to target CD19 or other B-cell antigens with high affinity, leading to a potent immune response and effective killing of malignant B cells. More recently, CAR-T treatment has been shown to be safe and effective in a very limited number of patients with severe, refractory autoimmune conditions such as systemic lupus erythematosus, systemic sclerosis and myosi

Multi-kingdom gut microbiota characterization in Chinese patients with idiopathic inflammatory myopathies.

Idiopathic inflammatory myopathies (IIMs) are systemic autoimmune disorders with unknown etiology. Despite the established link between gut microbes and immunity, the roles of gut bacteriome, mycobiome, and virome in IIM are unexplored. We performed shotgun metagenomic sequencing on fecal samples from 34 IIM patients and 37 healthy controls to profile gut microbiota. Taxonomic, functional, network, and machine-learning analyses revealed microbial dysbiosis and its potential for discriminating IIM. All three microbial kingdoms were significantly altered in IIM. Several inflammation-associated b

CD19 CAR-T cells for treatment-refractory autoimmune diseases: the phase 1/2 CASTLE basket trial.

Chimeric antigen receptor (CAR)-T cells are considered a powerful therapeutic tool to reset the immune system in patients with autoimmune diseases. Innovative trial designs are needed to allow feasible testing of the safety and efficacy of CAR-T cells in clinical studies. CASTLE (CAR-T cells in systemic B cell mediated autoimmune disease) is a phase 1/2a two-stage optimal design basket study that investigated the safety and efficacy of zorpocabtagene autoleucel (Zorpo-cel, also known as MB-CART19.1), an autologous CD19 CAR-T cell product, in patients with treatment-resistant systemic lupus ery

The distribution and clinical significance of anti-nuclear antibodies in patients with idiopathic inflammatory myopathy in Southwest China.

BACKGROUND: Idiopathic inflammatory myopathies (IIMs) are a group of autoimmune diseases characterized by skeletal muscle involvement. The association of anti-nuclear antibody (ANA) testing with clinical features in IIM patients has not been fully elucidated. METHODS: We retrospectively included patients who were diagnosed with IIM between January 2012 and February 2023, then systematically tested ANA titers and analyzed the ANA patterns of IIM patients by indirect immunofluorescence assay (IFA). Demographic, clinical and relevant laboratory data were recorded and further analyzed in 673 ANA-

Prolonged remission of dermatomyositis and autoantibody negativization following SARS-CoV-2 infection in an elderly patient.

This case report presents the clinical course of an 80-year-old patient diagnosed with dermatomyositis in 2014, initially managed successfully with methotrexate (MTX). In March 2022, the patient experienced a severe SARS-CoV-2 infection with significant pulmonary involvement, necessitating discontinuation of MTX. Notably, the patient maintained complete remission of dermatomyositis post-infection without relapse. Sustained negativization of autoantibodies, confirmed through multiple evaluations, was observed, while persistent capillaroscopic abnormalities suggested ongoing subclinical disease

Immune-Driven Expression in Inclusion Body Myositis With T-Cell Large Granular Lymphocytic Leukemia.

OBJECTIVES: T-cell large granular lymphocytic leukemia (T-LGLL), reported in up to 58% of inclusion body myositis (IBM) patients, is a rare leukemia of cytotoxic or less commonly helper T cells. The range of myopathies in T-LGLL and the impact of coexisting T-LGLL in IBM are not well understood. Our objectives are to investigate the spectrum of myopathies in patients with T-LGLL and compare the clinical, serological, pathological, and transcriptomic features of IBM patients with and without T-LGLL. METHODS: We reviewed two Mayo Clinic cohorts: (1) T-LGLL patients evaluated for myopathies (200

Progressive ossification in an adolescent with fibrodysplasia ossificans progressiva.

Fibrodysplasia ossificans progressiva (FOP), also known as Munchmeyer disease or 'stone man syndrome', is a rare, disabling genetic disorder caused by an ACVR1/ALK2 mutation, resulting in progressive heterotopic ossification of muscles, tendons and ligaments. Even minor trauma can trigger flare-ups, causing joint fusion and severe disability. An early adolescent male presented with progressive joint stiffness, painful swellings and malnutrition. Examination revealed hallmark signs of FOP, including malformed great toes and widespread soft tissue ossification. Differential diagnoses such as prog

Paraneoplastic neurologic disorders and neurologic complications of immune checkpoint inhibitors.

Paraneoplastic neurologic syndromes (PNS) are disorders resulting from an aberrant immune response against the nervous system, triggered by a tumor. Immune checkpoint inhibitor (ICI) cancer immunotherapies, enhance antitumor immunity by targeting immune checkpoint molecules on different cells, including immune and tumor cells. While ICIs improve cancer outcomes, these therapies can lead to immune-related adverse events (irAEs) in several systems including the nervous system. Neurologic irAEs may affect any level of the neuraxis, and, a subset of patients may present with syndromes resembling c

Expanding the Differential Diagnosis of Ultrasonographic Flexor Digitorum Profundus-Flexor Carpi Ulnaris Dissociation of Echogenicity: Muscular Dystrophies.

INTRODUCTION/AIMS: Dissociation of echogenicity of the flexor digitorum profundus (FDP) and flexor carpi ulnaris (FCU) on neuromuscular ultrasound has been reported to be a useful sign to differentiate inclusion body myositis (IBM) from more common disease mimics, but it is not clear that this finding is pathognomonic of IBM. Our study aimed to evaluate whether this sign may be present in certain muscular dystrophies. METHODS: This case series included 6 genetically confirmed muscular dystrophy patients (2 Becker muscular dystrophy [BMD], 2 myotonic dystrophy type 2 [DM2], 1 myotonic dystroph
