There is a relationship between the acromial index, the critical shoulder angle, and rotator cuff tears.

Understanding anatomical parameters is essential not only for the treatment but also for the prevention of new rotator cuff tears. There are still controversies and conflicting results regarding the association or causal inference between the acromion index, the critical shoulder angle, and this disease. The study aimed to (1) investigate the relationship between the acromial index, the critical shoulder angle, and rotator cuff tears, as well as (2) analyze the intra- and inter-rater reliability of the measurements performed. This was a cross-sectional study conducted at a single diagnostic ce

Impact of Intracompartmental Septation on Long-Term Treatment Result for de Quernain's Disease.

BACKGROUND: This study aimed to assess the impact of intracompartmental septation in the first dorsal extensor compartment on nonsurgical treatment of de Quervain's disease. METHODS: Two hundred thirty-four wrists of 213 patients followed for more than 1 year were retrospectively reviewed on their final treatment outcomes. We compared patients' data at their initial visit including septal condition in the first dorsal extensor compartment assessed using ultrasound between 2 groups divided according to the outcomes. These groups were made based on whether they had surgery or not, and if patien

Effectiveness of 1064-nm high-power laser therapy combined with thumb spica splinting in De Quervain's tenosynovitis: a randomized double-blind sham-controlled trial.

BACKGROUND: De Quervain’s tenosynovitis (DQT) is an inflammatory condition of the first dorsal compartment of the wrist that causes pain and impaired thumb function. High-power laser therapy (HPLT) has been proposed as a non-invasive modality for musculoskeletal pain management; however, evidence supporting its effectiveness in DQT remains limited. This study aimed to evaluate the effectiveness of 1064-nm HPLT combined with thumb spica splinting in reducing pain and improving hand function in patients with DQT. DESIGN: A randomized, double-blind, sham-controlled clinical trial. METHODS: Twenty

De Quervain's tenosynovitis after trapeziometacarpal arthroplasty: Is it related to thumb lengthening?

Trapeziometacarpal (TMC) arthroplasty is a widely accepted treatment for osteoarthritis of the TMC joint. However, one potential complication is De Quervain's tenosynovitis and its cause post-arthroplasty remains unclear. The aim of this study was to evaluate whether thumb lengthening, as measured by the Scaphometacarpal Index (SMI), is associated with the incidence of De Quervain's tenosynovitis following TMC arthroplasty. A retrospective analysis was conducted of 114 TMC arthroplasties performed between 2015 and 2022. Thumb lengthening was quantified using pre- and postoperative radiographs.

[Extracorporeal shockwave therapy (ESWT) in hand surgery].

Extracorporeal shockwave therapy (ESWT) acts as an acoustic wave via momentum transfer and mechanotransduction in a plethora of regenerative ways. Scaphoid non-unions benefit from high-energetic focused ESWT. In lunate necrosis aka Morbus Kienböck focused high-energetic ESWT can reduce pain and improve MRI. In CMC 1 osteoarthritis focused ESWT appears to be clinically equal to intraarticular ultrasound-guided hyaluronic acid injection with more pronounced effects 6 months after therapy in the ESWT group. Tendinopathies of the hand can be treated with both, radial and focused ESWT, especially i

Exercise Therapy for Chronic ECU Tenosynovitis: A Case Report.

Background and Clinical Significance: This case highlights the management of chronic extensor carpi ulnaris (ECU) tenosynovitis in a patient exposed to non-traditional wrist-loading activities. Exercise therapy rehabilitation is well established in shoulder and knee tendinopathies, although it remains less well described for wrist tendinopathies beyond De Quervain's disease. Moreover, the patient's active engagement in non-traditional, wrist-intensive sports such as handstands, slacklining, and yoga may have contributed to the development and persistence of chronic extensor carpi ulnaris (ECU)

An Ultrasonographic Study of the Superficial Radial Nerve in Healthy Subjects: Suggesting a Safe Zone for Wrist Extensor Compartment Injections.

Background/Objectives: The superficial radial nerve (SRN) is highly susceptible to iatrogenic injury during wrist injection procedures. This study aimed to identify the anatomical trajectory of the SRN using high-resolution ultrasonography and to establish a reliable "safe zone" for wrist extensor compartment injections. Methods: Fifty-eight forearms from 29 healthy volunteers (15 males, 14 females) were evaluated. Four anatomical levels were defined: the proximal and distal ends of the extensor compartment I-II intersection area (Levels A and B), and the proximal and distal points of SRN cros

Management of De-Quervain's tenosynovitis by Agnikarma with Suvarana Shalaka: A case report.

De-Quervain's tenosynovitis is a painful condition characterized by wrist pain and swelling at the radial styloid process due to inflammation and thickening of the tendon sheath of the abductor pollicis longus and extensor pollicis brevis. Conventional treatment includes rest, analgesics, corticosteroid injections, and surgical release. In Ayurveda, this condition is correlated with Snayugata Vata, where Agnikarma (therapeutic heat) is recommended. A 55-year-old female presented with severe left wrist pain, swelling and difficulty in performing household work for 20 days. Finkelstein's test re

Inflammatory Arthritis Uncovered Through Imaging of Osteonecrosis: A Case Series.

Osteonecrosis (ON) is typically viewed as an isolated condition stemming from vascular compromise. We present a three-case series demonstrating that MRI, performed to confirm the diagnosis of ON, incidentally revealed features consistent with underlying, previously undiagnosed inflammatory arthritis (IA). Even in the absence of typical clinical features, the presence of imaging findings such as synovitis, tenosynovitis, and erosions should prompt a thorough clinical and serological evaluation for coexisting IA. An interesting point to note is that none of the three patients reported a history

Brachioradialis muscle pain: a common source of underdiagnosed or misdiagnosed forearm pain.

The brachioradialis (BR) muscle is a long, large, fusiform muscle on the lateral side of the forearm. It originates from the lateral distal humerus and inserts to the base of the styloid process of the radius. The function of the BR muscle is to flex the elbow, especially with the hand in a neutral position, to pronate and supinate the forearm, and to support wrist extension, especially gripping or picking up something. BR muscle pain arises from repetitive overuse, sudden overloading, and direct blow. Common painful daily activities include putting a cup back down after drinking, opening door

Flexor Tendon Rupture after One Corticosteroid Injection in the Treatment of Trigger Finger: A Case Report and Literature Review.

Corticosteroid injection is a widely accepted first-line treatment for trigger finger. However, it carries a potential risk of flexor tendon rupture. This complication is rare, particularly after a single injection. We present a 59-year-old woman who experienced spontaneous ruptures of the flexor digitorum profundus and superficialis tendons of the middle finger 2 months after a single corticosteroid injection for trigger finger. We review the literature and discuss underlying mechanisms for flexor tendon rupture. To minimise the risk of this complication, we recommend administering extra-tend

GLP-1 Receptor Agonist Use and A1 Pulley Release After Corticosteroid Injection for Trigger Finger in Type 2 Diabetes.

BACKGROUND: Patients with type 2 diabetes mellitus (T2DM) have a higher incidence of trigger finger and are more likely to require operative A1 pulley release. The impact of glucagon-like peptide-1 receptor agonist (GLP-1RA) therapy on treatment progression remains unclear. This study evaluated the association between GLP-1RA use and progression to operative A1 pulley release following corticosteroid injection for trigger finger in patients with T2DM. METHODS: A retrospective analysis was conducted using the TriNetX Research Network to identify adults with T2DM and trigger finger who underwen

What Do Patients Want? Outcome Measures in a Diverse World.

PURPOSE: Quantifying outcomes of medical interventions has evolved but challenges remain in hand surgery. Initially, hand surgeons used concrete assessments like range of motion and/or pinch strength. With a shift toward patient-centered care, numerous pathology-specific patient-reported outcome measures (PROMs) were developed. These measures typically used standardized closed-ended outcomes and mostly originated from western countries. However, it is unclear how well measures with specific items capture diverse patients' values. Therefore, we used an open-ended PROM, the Patient-Specific Func
