Integrated Doppler and Elastography Assessment of Hidradenitis Suppurativa and Dactylitis.

Multimodal high-resolution ultrasound, including B-mode, Power Doppler, MicroVessel Doppler, spectral Doppler, and strain elastography, was used to assess concomitant dactylitis and hidradenitis suppurativa (HS) in a 46-year-old woman with severe hidradenitis suppurativa (IHS4 = 28), who was diagnosed 1.5 years ago and has been using adalimumab. Axillary ultrasound demonstrated abscess cavities and draining fistulous tracts with marked structural distortion, increased vascular signal on advanced Doppler modalities, and heterogeneous stiffness patterns on elastography, consistent with active de

Comparative effectiveness of extracorporeal shock wave therapy and high-intensity laser therapy in trigger finger: a randomized controlled trial.

Trigger finger (TF) is stenosing tenosynovitis causing pain and impaired hand function. To compare extracorporeal shock wave therapy (ESWT) and high-intensity laser therapy (HILT) as adjuncts to home exercise in TF. In this three-arm randomized controlled trial, patients were allocated to ESWT, HILT, or home exercise alone. Active modalities were applied for 3 weeks (nine sessions), and all groups followed the same exercise program. Assessments were performed at baseline, week 3, and week 12. The primary outcome was week 12 QuickDASH; secondary outcomes were VAS, Quinnell grade, grip strength,

Transcultural Adaptation of the Spanish Version of the ABILHAND Scale for Hand Surgery in a Colombian Population of Patients with Hand Pathologies.

PURPOSE: Because manual ability is a latent and complex variable that is closely linked to an individual's well-being and daily functioning, it is important to assess it through patient-reported outcome measures that capture the patient's own perception of dexterity in everyday life. This study aimed to develop a Spanish version of the ABILHAND-Hs scale that is equivalent to the original but adapted to evaluate the most prevalent hand pathologies in Colombia. METHODS: A group of bilingual experts translated the ABILHAND scale from English into Spanish using a forward-backward translation meth

Orthopedic visits and procedures preceding rheumatoid arthritis diagnosis: A nationwide case-control study.

OBJECTIVES: Rheumatoid arthritis (RA) may initially present with nonspecific musculoskeletal symptoms before formal diagnosis, often prompting orthopedic visits and subsequent orthopedic procedures. We aimed to examine whether such procedures are more frequent among individuals prior to RA diagnosis compared to RA-free matched controls. METHODS: We conducted a retrospective case-control study using Leumit Healthcare Services electronic database. Each subject diagnosed with RA between 2002 and 2023 was matched with 5 controls based on age, sex, and socioeconomic status (SES). Orthopedic visits

Fibrotic disease co-occurrence and associated risk of subsequent organ failure: a nationwide matched cohort study.

OBJECTIVES: Shared pathophysiologic features of fibrotic diseases, irrespective of anatomical site, have been suggested from epidemiologic, genetic and mechanistic studies. Improvement in diagnostic capabilities and emergence of antifibrotic medications warrants identification of trajectories of fibrotic diseases and earlier identification of patients at risk of organ failure as they may have a fibrotic component. METHODS: We used Danish nationwide health registries to identify all adults with a first-time diagnosis of any of nine major fibrotic diseases from 1998 to 2024. Co-occurrence betwe

An Unusual Complication of Intralesional Corticosteroid Injections: Two Case Reports.

BACKGROUND: Intralesional corticosteroid injections are widely used for treating various inflammatory and musculoskeletal conditions. While generally safe, they can cause adverse effects, including hypopigmentation, which may have psychosocial implications for affected patients. CASE PRESENTATION: We report two cases of hypopigmentation following intralesional triamcinolone acetonide injections. The first case involves a 65-year-old man who developed a well- defined hypopigmented patch on the dorsum of his right hand following treatment for De Quervain's tenosynovitis. The second case describ

Calcific Tendinitis of the Linea Aspera with Bilateral Deposits and Inflammatory Asymmetry Presenting as Unilateral Pain: A Case Report and Review of Literature.

INTRODUCTION: Calcific tendinitis (hydroxyapatite deposition disease) most commonly affects the shoulder and peri-trochanteric hip region. Involvement of the linea aspera is uncommon and may mimic aggressive pathology due to associated cortical reaction and marrow edema. CASE REPORT: A 53-year-old female presented with predominantly right-sided upper thigh pain. Radiographs demonstrated amorphous calcification along the lateral proximal femur at the linea aspera. Magnetic resonance imaging showed a calcific focus with marked adjacent marrow and soft-tissue edema on the right, while the contra

Rheumatoid Arthritis Mimickers Presenting With Polyarticular Arthritis: A Case Series Highlighting Diagnostic Pitfalls.

Rheumatoid arthritis (RA) is the most common chronic inflammatory arthritis and a leading cause of disability worldwide. However, several diseases may mimic RA, particularly in patients presenting with polyarticular pain or seronegative inflammatory arthritis. Misdiagnosis may result in inappropriate immunosuppressive therapy and delayed treatment of the underlying disease. Recognizing these mimickers remains a critical challenge in rheumatology practice. We conducted a retrospective case series of patients evaluated at a tertiary rheumatology center between January 2021 and December 2025. Pat

Update of guideline for diagnosis and treatment of subacromial pain syndrome: a multidisciplinary review by the Dutch Orthopedic Association Part 2: Operative considerations and treatment of various conditions related to subacromial pain syndrome.

BACKGROUND AND PURPOSE: In 2013, the first clinical practice guideline for subacromial pain syndrome (SAPS) was developed in the Netherlands to support healthcare professionals. SAPS refers to non-traumatic, non-rheumatologic shoulder complaints that are particularly painful during arm elevation. It includes conditions such as supraspinatus tendinosis, calcific tendinitis, and degenerative supraspinatus tears. Over 50,000 patients annually consult orthopedic surgeons for these issues. In response to new evidence and clinical needs, an updated guideline was developed. Part 2 focuses on supraspi

Flexor Tendon Sheath Ganglion Causing Concurrent Trigger Wrist and Carpal Tunnel Syndrome.

INTRODUCTION: Trigger wrist is a rare condition often misdiagnosed as trigger finger or carpal tunnel syndrome (CTS). It is characterized by painful clicking, snapping, or locking of the wrist during movement and can result from space-occupying lesions, such as ganglion cysts. This case highlights an unusual presentation of trigger wrist associated with CTS caused by a ganglion cyst in the flexor tendon sheath. CASE PRESENTATION: A 44-year-old male presented with paresthesia, severe pain, and triggering of the right wrist. Imaging and histopathology confirmed a ganglion cyst between the flexo

Recurrent Pregnancy Loss: Novel Insights and Future Directions.

In the last 15 years, the US and UK have fought two major wars in the Persian Gulf region. Controversy has arisen over the nature and causes of health problems among military veterans of these two wars. Toxic exposures have been hypothesised to cause the majority of the long-term health problems experienced by veterans of the 1991 Gulf War. The assessment of these toxic exposures and the resolution of controversy about their health effects provide a unique case study for understanding how toxicological disputes are settled in the US. Neither clinical examination of ill war veterans nor scienti

Exceptional Massive Calcification of the Medial Collateral Ligament: A Case Linked to Underlying Lung Neoplasm.

Calcific tendinitis of the medial collateral ligament (MCL) is an exceedingly rare cause of knee pain. We report an exceptional case of massive, refractory MCL calcific tendinitis, with a unique presentation potentially linked to an underlying malignancy. A 61-year-old female patient presented with a 3-month history of persistent left medial knee pain, unresponsive to conservative therapy. Physical examination revealed medial joint line tenderness and limited range of motion. Imaging-including radiography, 3-dimensional computed tomography, and magnetic resonance imaging-demonstrated an unusua

Updates in ultrasound-guided musculoskeletal interventions.

Musculoskeletal ultrasound (MSK-US) has evolved from a diagnostic tool into a key modality for guiding minimally invasive interventions across the musculoskeletal system. Its real-time imaging, portability, and absence of ionizing radiation make it a cost-effective and safe alternative to fluoroscopy and CT for numerous procedures, including biopsies, aspirations, injections, and percutaneous releases. Recent technological advances in ultrasound, including high-frequency probes, microvascular imaging, needle-tracking systems, and robotic probe steering, have improved precision and expanded its
