Multimodal high-resolution ultrasound, including B-mode, Power Doppler, MicroVessel Doppler, spectral Doppler, and strain elastography, was used to assess concomitant dactylitis and hidradenitis suppurativa (HS) in a 46-year-old woman with severe hidradenitis suppurativa (IHS4 = 28), who was diagnosed 1.5 years ago and has been using adalimumab. Axillary ultrasound demonstrated abscess cavities and draining fistulous tracts with marked structural distortion, increased vascular signal on advanced Doppler modalities, and heterogeneous stiffness patterns on elastography, consistent with active de

Palmar bursitis and tenosynovitis of the flexor tendon sheath around the wrist is a rare manifestation of musculoskeletal tuberculosis. It has an indolent course and diagnosis is often delayed especially when it is an isolated finding. Timely diagnosis can prevent complications like bony involvement, tendon rupture and neuropathy. Here, we report a case of a 42-year-old male patient who presented with gradually progressive painful swelling of the left palm, wrist and distal forearm. Ultrasound and MRI revealed synovial thickening and distension of the palmar bursae and flexor tendon sheath wit

INTRODUCTION: Chronic volar wrist swelling is a diagnostic challenge, particularly in tuberculosis-endemic regions, where both infectious and inflammatory causes must be considered. Rheumatoid tenosynovitis, an extra-articular manifestation of rheumatoid arthritis (RA), can mimic infectious tenosynovitis, especially in seronegative patients.
CASE REPORT: We report a case of a 44-year-old male presenting with progressive pain and swelling over the volar aspect of the right wrist for 2 months. Clinical findings of a local rise in temperature, tenderness, and limitation of range of motion sugges

Gonococcal tenosynovitis is a rare but serious manifestation of disseminated Neisseria gonorrhoeae (N. gonorrhoeae) infection. It presents with localized inflammatory symptoms, including joint and tendon sheath involvement, and requires early recognition to prevent complications, including abscess formation, tendon rupture, and long-term functional impairment. We report the case of a 40-year-old male who developed progressive swelling, erythema, and pain in his left wrist over 20 days, without significant response to oral anti-inflammatory treatment. Laboratory tests showed marked leukocytosis

PURPOSE: This study aimed to describe the clinical and imaging features of patients with wrist tuberculosis (TB) and to explore the importance of magnetic resonance imaging (MRI) in the evaluation and treatment of wrist TB.
METHODS: The clinical and imaging data of 47 patients with wrist TB, diagnosed through a combination of pathological (microbiological culture, polymerase chain reaction, and histopathological examination) and clinical methods, were retrospectively analyzed. The demographic characteristics, clinical symptoms, laboratory tests, and imaging findings of these patients were rec

There has been little investigation into the health patterns of the eventing horse population outside veterinary clinics. To target health problems in the eventing horse population, it is important to determine the relative impact of different health problems. The objectives were to describe the prevalence and patterns of lameness/illness in registered British eventing horses. An online survey was released for all horses registered with British eventing in 2018. Data were collated and descriptive analyses undertaken. A total of 1677 surveys were completed, among which 49.4% reported a previous

BACKGROUND: Tenosynovial giant cell tumour (TGCT) is a locally aggressive neoplasm for which few systemic treatment options exist. This study evaluated the efficacy and safety of vimseltinib, an oral, switch-control, CSF1R inhibitor, in patients with symptomatic TGCT not amenable to surgery.
METHODS: MOTION is a multicentre, randomised, double-blind, placebo-controlled, phase 3 trial done in 35 specialised hospitals in 13 countries. Eligible patients were adults (aged ≥18 years) with a histologically confirmed diagnosis of TGCT for which surgical resection could potentially worsen functional

CASE: A 53-year-old man presented with a puncture wound to his right thumb, resulting in a horseshoe abscess, its infectious clinical picture muddled by dense paresthesias from the ensuing acute carpal tunnel syndrome. He was treated with irrigation and debridement of the thumb and small finger flexor tendon sheaths, carpal tunnel release, and bootlacing and dermal substitute application to the proximal forearm with eventual split-thickness skin grafting.
CONCLUSION: Although a horseshoe abscess resulting in acute carpal tunnel syndrome is a rare entity unquantified in the literature, the tre

Soft tissue and osseous musculoskeletal infections are common but can be difficult to diagnose clinically. Signs, symptoms, and physical examination findings may be nonspecific, and laboratory values can be inconclusive. The extent of disease may also be underestimated on physical examination. Soft tissue infections most commonly occur secondary to direct inoculation from broken skin and less frequently due to the seeding of the soft tissues from hematogenous spread, while osseous infections are more commonly due to hematogenous seeding. Infections may also be iatrogenic, following surgery or

BACKGROUND: The diagnosis of infectious flexor tenosynovitis (FTS) has historically been made based on physical exam using Kanavel's signs. The specificity of these findings has come into question. We looked to evaluate the use of contrast-enhanced computed tomography (CT) in increasing the successful diagnosis of FTS.
METHODS: Two adult cohorts were formed, one of patients with FTS confirmed in the operating room and the second of patients with ICD.10 identified finger cellulitis (FC), without concomitant FTS. Demographics, laboratory values, CT scans, and examination findings were evaluated

Although superficial infections can often be diagnosed and managed clinically, physical examination may lack sensitivity and specificity, and imaging is often required to evaluate the depth of involvement and identify complications. Depending on the area of involvement, radiography, ultrasound, CT, MR imaging, or a combination of imaging modalities may be required. Soft tissue infections can be nonnecrotizing or necrotizing, with the later having a morbid and rapid course. Infectious tenosynovitis most commonly affects the flexor tendon sheaths of the hand, characterized by thickened and enhan

Infections of the distal phalanx are the most common of all hand infections. There are dorsal localised infections, which develop in the area of the nail and are called paronychia, and palmar infections, which affect the fingertip and are the typical felons. The acute paronychia must be specifically opened depending on the site of infection. This requires precise anatomical knowledge of nail structure. Chronic paronychia usually have other causes and treatment is much more difficult. Felons are often extremely painful. There is a complex system of fibrous septa and swelling is limited. If the

A horseshoe abscess is caused by infection that spreads between the flexor tendon sheath of the thumb or little finger through the radial and ulnar bursae through communication between the two and/or the space of Parona. We present a case of an 80-year-old woman with rheumatoid arthritis who presented with 6 months of right hand and wrist soft tissue swelling, initially treated as a rheumatoid arthritis flare. MRI demonstrates the horseshoe abscess and after surgical irrigation and debridement with synovectomy, cultures demonstrated infection with mycobacterium avium intracellulare (MAI). This

Acute hand infections are often caused by puncture wounds and are generally classified into superficial or deep infections. Superficial infections occur in the skin and subcutaneous tissues, whereas deep infections can involve the tendon sheaths, adjacent anatomic compartments, deep fascial planes, bursae, joint spaces, and bones. Superficial hand infections are more common than deep infections and are typically managed with elevation, warm soaks, splinting in the position of function, analgesics, and empiric antibiotics when indicated. Paronychia, which can be acute or chronic, is an infectio

PURPOSE: Characteristic swelling has been described as a differentiating sign of pyogenic flexor tenosynovitis (PFT) but has not been validated. We conducted a retrospective study of adults with finger infections to compare radiographic parameters of soft tissue dimensions. Our hypothesis was that in patients with digit infections, radiographic soft tissue thickness measurement would differ between PFT and non-PFT infected digits.
METHODS: Patients with a finger infection and radiographic evaluation were identified retrospectively at a large academic medical center and divided into 2 groups:

Magnetic resonance imaging (MRI) is a powerful imaging modality in the evaluation of musculoskeletal (MSK) soft tissue, joint, and bone infections. It allows prompt diagnosis and assessment of the extent of disease, which permits timely treatment to optimize long-term clinical outcomes. MRI is highly sensitive and specific in detecting the common findings of MSK infections, such as superficial and deep soft tissue oedema, joint, bursal and tendon sheath effusions, lymphadenopathy, bone marrow oedema, erosive bone changes and periostitis, and bone and cartilage destruction and sequestration. BACKGROUND: Swellings in the hand and wrist are frequently encountered in clinical practice and fine needle aspiration cytology (FNAC) is used as a primary diagnostic modality in most of them. A wide spectrum of lesions can be encountered at these sites. The aim of this study was to report the prevalence and spectrum of hand and wrist lesions and evaluate the diagnostic role of FNAC.
METHODS: All the patients who presented with palpable lesions in the hand or wrist between January 2011 and July 2016 were reviewed, retrospectively.
RESULTS: 1312 cases were subjected to FNAC of hand and wrist

Hand and wrist infections can present with a spectrum of manifestations ranging from cellulitis to deep-space collections. The various infectious processes can be categorised as superficial or deep infections based on their respective locations relative to the tendons. Superficial hand infections are located superficial to the tendons and are comprised of cellulitis, lymphangitis, paronychia, pulp-space infections, herpetic whitlow, and include volar as well as dorsal subcutaneous abscesses. Deep hand infections are located deep to the tendon sheaths and include synovial space infections, such

For patients with suspected flexor tenosynovitis, the mainstay of diagnosis is a thorough history and physical examination. The examination is guided by evaluating the patient for Kanavel's four cardinal signs. Empiric antibiotics should be started immediately on diagnosis covering skin flora and gram-negative bacteria. Typically, surgery is required. Appropriate exposure is required for adequate treatment and incisions should be tailored to preserve areas of skin compromised from draining sinuses and abscess pressure. Diabetes mellitus and peripheral vascular disease place patients at higher
