Primary Psoas Pyomyositis Presenting as Subluxation of the Hip: A Case Report.

CASE: An eight-year-old girl presented with a septic subluxation of the right hip secondary to spontaneous drainage of a psoas pyomyositis. The course of the infection was protracted, and the symptoms were mild relative to the magnitude of the psoas abscess and the volume of purulence in the hip that caused subluxation of the femoral head. At the time of surgical drainage, the hip capsule directly communicated with the psoas abscess along the course of the iliopsoas tendon sheath. Methicillin-sensitive Staphylococcus aureus (MSSA) was the microorganism that was isolated from the abscess and fr

Migration of a toothpick along the flexor tendon sheath in a lower extremity.

The most common foreign bodies seen in the foot are sewing needles, toothpicks, glass, and materials such as sand or silica. Foreign bodies in the foot are usually embedded, and surgical exploration and removal is usually necessary. Penetrating foreign bodies in the foot-particularly of organic origin, like wood-can cause cellulitis, osteomyelitis, abscess formation, and pseudotumor formation. Identification of foreign bodies in the foot can be challenging because they are often not radiopaque. However, foreign bodies in the foot do not migrate, in contrast to upper extremities, where foreign

[Complicated course of a felon: prevention is better than surgical reconstruction].

Infections of the hand can result in significant limitation of the function of the hand, and may even require partial amputation of a digit. A felon is an abscess of the distal pulp of the fingertip, which can spread rapidly through the flexor tendon sheath when not treated appropriately. Knowledge of the anatomy and the correct method of physical examination are essential when assessing a patient suspected of having a felon or flexor tendon sheath infection. Here we present two patients with a complicated felon resulting in palmar necrosis of the distal phalanx of the thumb, in which a groin

Point-of-Care Ultrasound Diagnosis of Deep Space Hand Infection.

BACKGROUND: Point-of-care ultrasound is emerging as an important imaging modality for characterizing soft-tissue infections and provides advantages over physical examination and magnetic resonance imaging (MRI). CASE REPORT: A 30-year-old man presented to the emergency department with extensive left upper extremity cellulitis. Magnetic resonance imaging of the left arm was preliminarily interpreted as soft-tissue swelling without evidence of deep-space infection. Point-of-care ultrasound revealed pockets of fluid with sonographic fluctuance tracking along the tendon sheath that were concernin

Tenosynovitis with rice body formation presenting as a cutaneous abscess.

A 62-year-old woman with a past medical history of rheumatoid arthritis was referred to the Department of Dermatology because of an enlarging cutaneous lesion on the right thumb which resembled a soft tissue infection. She had received antibiotics without significant improvement. Clinical examination revealed an erythematous nodule involving almost the whole surface of the distal phalanx with spontaneous drainage of countless of small yellowish ovoid granules. Histopathologic study of these structures showed an inner core of amorphous acidophilic material with some interspersed chronic inflamm

Changing patterns of septic tenosynovitis of the distal extremities.

The medical literature reports that human pyogenic tenosynovitis occurs almost exclusively in flexor tendons of distal extremities with only rare case reports in extensor compartments. We report a series of patients with septic extensor compartment tenosynovitis of the extremity. Twenty cases of septic tenosynovitis tendons of the wrist, hands, feet, and ankles were presented to our emergency department over a 4.17-year period, 15 men (one, twice over a 2.5-year span) and 4 women with an average age of 39 years. Diagnosis was made using CT (n = 6), MRI (n = 14), and in one case ultrasound (US)

The use of magnetic resonance imaging to better define hoof pathology in the reticulated giraffe (Giraffa camelopardalis reticulata).

A 22-yr-old bull giraffe (Giraffa camelopardalis reticulata) with severely altered hoof conformation in both forelimbs was presented for necropsy following acute mortality. Due to multiple challenges that prevented safe immobilization, corrective hoof trimming procedures were never performed on this animal. To better define the extent of the damage of the soft tissue structures and bone within the hoof, a magnetic resonance imaging (MRI) system was used to obtain images of these structures. The MRI study found evidence of severe osteolysis, phalangeal fractures of both forelimbs, and tenosynov

Horseshoe abscess associated with acute carpal tunnel syndrome: somebody wake up the hand surgeon.

The horseshoe abscess is a rare variant of flexor tendon sheath infections. It involves the tendon sheaths of the first and fifth digits, which communicate through the deep potential space of the wrist. Neural compression is possible, leading to an acute carpal tunnel syndrome. The approach to such patients should include empirical antibiotic coverage and urgent consultation with a hand surgeon to facilitate operative management.

Mycotic cysts: report of 21 cases including eight pheomycotic cysts from Saudi Arabia.

BACKGROUND: Mycotic cysts are subcutaneous cystic granulomas caused by either dematiaceous (pigmented) fungi (pheomycotic cysts) or eumycotic (nonpigmented fungi) present in soil, wood, and decaying plant material. These fungi gain access to the tissues via a wooden splinter or thorn. No deep tissue involvement or extension to bone is known to occur. METHODS: We reviewed our surgical pathology files for the last 32 years. All cases with the diagnosis of cysts with fungi, thorns, or splinters and associated granulomatous and acute inflammation were reviewed. Gomori's silver and periodic acid-S

Imaging of wrist masses.

The wrist is a complex structure with an extensive differential diagnosis for a presenting mass. However, the vast majority of hand and wrist masses are benign, and many of these have a distinctive radiographic appearance. In this article, the imaging characteristics of the most common entities are reviewed with particular attention to magnetic resonance appearance. The 3 most common hand and wrist lesions include ganglion cysts, giant cell tumors of the tendon sheath, and hemangiomas. Other common lesions that can be diagnosed radiographically include lipomas, neural sheath tumors, infection

Common acute hand infections.

Hand infections can result in significant morbidity if not appropriately diagnosed and treated. Host factors, location, and circumstances of the infection are important guides to initial treatment strategies. Many hand infections improve with early splinting, elevation, appropriate antibiotics and, if an abscess is present, incision and drainage. Tetanus prophylaxis is indicated in patients who have at-risk infections. Paronychia, an infection of the epidermis bordering the nail, commonly is precipitated by localized trauma. Treatment consists of incision and drainage, warm-water soaks and, so

[The importance of sonography in orthopedics for dogs].

For the ultrasonographic examination of the musculoskeletal system in dogs a 7,5 MHz linear scanner is used best, in some cases with a standoff pad. Radiography is the technique of choice to detect bone abnormalities. For certain circumstances the sonography can be used to give additional information (e.g. surrounding tissue with osteomyelitis or bone tumors). For the diagnostic of joint diseases ultrasonography is an excellent completion for the radiographic examination. Joint effusion and thickened capsule can be detected. Defects of articular cartilage like osteochondrosis dissecans in the

Hand infections secondary to fish bone injuries.

Hand injuries associated with fish bones and fin spines are not common but can cause morbidity out of proportion to the original injury. This is because such injuries often leave residual fragments of foreign organic matter in the tissues, leading to troublesome secondary infections. This report details 1 year's experience with nine hand infections after fish bone injury in a busy regional hand surgery referral centre. There were five males and four females with an average age of 45 years. Radiographs taken on presentation in five patients revealed a radiolucent foreign body in only two patien

Mycobacterium terrae tenosynovitis.

Atypical mycobacteria causing extra-pulmonary disease in man are well documented. These infections are manifested by the presence of ulcers, abscesses and lymphadenitis. Mycobacterium marinum is particularly noted for infections involving the synovium, tendon sheaths, bursae and bone. Of lesser note is Mycobacterium terrae (radish bacillus), a nonchromogen also associated with tenosynovitis. We are not aware of any previous report of the association of M. terrae with synovitis in Australia. This case report describes a culture-proven case of tenosynovitis caused by M. terrae.

[Infections of the hand].

Sometimes severe bacterial infections of the hand arise quickly and lead to damage of anatomical structures and loss of function. In this paper pathophysiology and clinical signs are presented. The most important priority in therapy is to prevent the infection from extending to deeper structures in the hand, such as tendon sheaths, and along them to the mid palmar space and Parona's space. The therapy most often indicated is therefore surgical treatment at the very beginning, depending on the severity of pain and swelling.
