Ultrasound-Guided Needling Without Lavage Followed by Platelet-Rich Plasma or Corticosteroid in Rotator Cuff Calcific Tendinitis: A Comparative Cohort Study.

Background/Objectives: This study aimed to compare the effects of platelet-rich plasma (PRP) versus corticosteroid injection administered after ultrasound-guided needling without lavage on pain, shoulder function, and radiographic resorption in rotator cuff calcific tendinitis. Methods: This retrospective single-centre cohort included 93 patients. All underwent ultrasound-guided needling of the calcific deposit without lavage. Treatment allocation reflected routine clinical practice and was non-randomised. Patients with Gartner type 1-2 calcifications received either PRP or corticosteroid inje

Editorial Commentary: Good Outcomes Do Not Necessarily Mean Effective Treatment: A Critical Appraisal of the Causal Relation Between Treatment and Outcome in Calcific Tendinopathy.

Calcific tendinopathy of the shoulder can be treated using a variety of approaches, including noninvasive, minimally invasive, and arthroscopic procedures. Although numerous studies have reported favorable outcomes, many of them lack appropriate controls, making it difficult to differentiate between the effects of treatment and the natural history of the disease. Because calcific tendinitis has a strong tendency toward spontaneous resolution, clinical improvement after treatment alone does not establish causality. Furthermore, placebo effects may substantially contribute to symptom relief. Con

There is a relationship between the acromial index, the critical shoulder angle, and rotator cuff tears.

Understanding anatomical parameters is essential not only for the treatment but also for the prevention of new rotator cuff tears. There are still controversies and conflicting results regarding the association or causal inference between the acromion index, the critical shoulder angle, and this disease. The study aimed to (1) investigate the relationship between the acromial index, the critical shoulder angle, and rotator cuff tears, as well as (2) analyze the intra- and inter-rater reliability of the measurements performed. This was a cross-sectional study conducted at a single diagnostic ce

Calcific Tendinitis of the Linea Aspera with Bilateral Deposits and Inflammatory Asymmetry Presenting as Unilateral Pain: A Case Report and Review of Literature.

INTRODUCTION: Calcific tendinitis (hydroxyapatite deposition disease) most commonly affects the shoulder and peri-trochanteric hip region. Involvement of the linea aspera is uncommon and may mimic aggressive pathology due to associated cortical reaction and marrow edema. CASE REPORT: A 53-year-old female presented with predominantly right-sided upper thigh pain. Radiographs demonstrated amorphous calcification along the lateral proximal femur at the linea aspera. Magnetic resonance imaging showed a calcific focus with marked adjacent marrow and soft-tissue edema on the right, while the contra

Rheumatoid Arthritis Mimickers Presenting With Polyarticular Arthritis: A Case Series Highlighting Diagnostic Pitfalls.

Rheumatoid arthritis (RA) is the most common chronic inflammatory arthritis and a leading cause of disability worldwide. However, several diseases may mimic RA, particularly in patients presenting with polyarticular pain or seronegative inflammatory arthritis. Misdiagnosis may result in inappropriate immunosuppressive therapy and delayed treatment of the underlying disease. Recognizing these mimickers remains a critical challenge in rheumatology practice. We conducted a retrospective case series of patients evaluated at a tertiary rheumatology center between January 2021 and December 2025. Pat

Update of guideline for diagnosis and treatment of subacromial pain syndrome: a multidisciplinary review by the Dutch Orthopedic Association Part 2: Operative considerations and treatment of various conditions related to subacromial pain syndrome.

BACKGROUND AND PURPOSE: In 2013, the first clinical practice guideline for subacromial pain syndrome (SAPS) was developed in the Netherlands to support healthcare professionals. SAPS refers to non-traumatic, non-rheumatologic shoulder complaints that are particularly painful during arm elevation. It includes conditions such as supraspinatus tendinosis, calcific tendinitis, and degenerative supraspinatus tears. Over 50,000 patients annually consult orthopedic surgeons for these issues. In response to new evidence and clinical needs, an updated guideline was developed. Part 2 focuses on supraspi

Exceptional Massive Calcification of the Medial Collateral Ligament: A Case Linked to Underlying Lung Neoplasm.

Calcific tendinitis of the medial collateral ligament (MCL) is an exceedingly rare cause of knee pain. We report an exceptional case of massive, refractory MCL calcific tendinitis, with a unique presentation potentially linked to an underlying malignancy. A 61-year-old female patient presented with a 3-month history of persistent left medial knee pain, unresponsive to conservative therapy. Physical examination revealed medial joint line tenderness and limited range of motion. Imaging-including radiography, 3-dimensional computed tomography, and magnetic resonance imaging-demonstrated an unusua

Avoiding Unnecessary Surgery in Acute Longus Colli Calcific Tendinitis: A Clinical Image of a Retropharyngeal Abscess Mimicker.

Acute calcific tendinitis of the longus colli (LCT) is a rare, benign condition that frequently mimics retropharyngeal abscesses (RPA), a true surgical emergency. We present a 34-yearold woman presenting with 4 days of severe posterior neck pain, stiffness, and odynophagia. Initial evaluation revealed significantly elevated inflammatory markers (erythrocyte sedimentation rate [ESR] 113 mm/hr, C-reactive protein [CRP] 128 mg/L) and leukocytosis, raising immediate concern for deep neck infection. Contrast-enhanced computed tomography (CT) demonstrated a retropharyngeal fluid collection; however,

Calcific Tendinitis: A Pain-Oriented and Site-Specific Narrative Review.

Calcific tendinitis is a common musculoskeletal disorder characterized by deposition of calcium hydroxyapatite within or around tendon tissue. Although it has traditionally been regarded as a structural tendon pathology, growing clinical evidence suggests that symptom severity, particularly pain, is more closely related to inflammatory activity and disease stage than to the size or precise anatomical location of calcific deposits. In routine practice, patients most often seek medical attention during sudden and intense pain episodes, typically occurring in the resorptive phase. Current diagnos

Transcranial ultrasound stimulation of the right globus pallidus interna in schizophrenia: A randomized, sham-controlled pilot study.

In recent decades, there has been a call for change among all stakeholders involved in scoliosis management. Parents of children with scoliosis have complained about the so-called "wait and see" approach that far too many doctors use when evaluating children's scoliosis curves between 10° and 25°. Observation, Physiotherapy Scoliosis Specific Exercises (PSSE) and bracing for idiopathic scoliosis during growth are all therapeutic interventions accepted by the 2011 International Society on Scoliosis Orthopaedic and Rehabilitation Treatment (SOSORT). The standard features of these interventions a

Preoperative Shoulder Stiffness Was Accompanied by Distinct Supraspinatus Tendon Inflammatory Signatures but Not Inferior Long-term Outcomes After Rotator Cuff Repair.

BACKGROUND: Rotator cuff tears with concomitant preoperative shoulder stiffness present a clinical challenge because the rehabilitation needed to restore motion may conflict with the protection typically used to support tendon healing after rotator cuff repair. Although recent studies suggest that preoperative stiffness may not be associated with inferior long-term outcomes, whether it is associated with a different pattern of postoperative improvement and distinct inflammatory features in the torn tendon remains unclear. QUESTIONS/PURPOSES: (1) Do a greater proportion of patients with preope

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Subscapularis tendon (SSC) tears have recently become an area of current research focus. However, the risk factors for atraumatic isolated SSC tears, particularly anatomical factors, remain poorly determined. Therefore, the purpose of this study was to identify the associated factors contributing to isolated SSC tears. METHODS: This study involved single shoulders of 605 subjects drawn from a cohort of rural residents. The inclusion criteria required each subject to sign an informed consent document and to have a complete clinical evaluation. The exclusion criteria comprised the a

Interobserver reliability of classifying shoulder calcific tendinopathy on plain radiography and ultrasound.

INTRODUCTION: Shoulder calcific tendinopathy is a frequent cause of shoulder pain. Diagnosis is usually based on ultrasound (US) and/or x-ray. US is considered an inherently operator-dependent imaging modality and, interobserver variability has previously been described by experts in the musculoskeletal US. The main objective of this study is to assess the interobserver agreement for shoulder calcific tendinopathy attending to the size, type, and location of calcium analyzed in plain film and ultrasound among trained musculoskeletal radiologists. MATERIAL AND METHODS: From June 2018 to May 20
