Flexor Tendon Sheath Ganglion Causing Concurrent Trigger Wrist and Carpal Tunnel Syndrome.

INTRODUCTION: Trigger wrist is a rare condition often misdiagnosed as trigger finger or carpal tunnel syndrome (CTS). It is characterized by painful clicking, snapping, or locking of the wrist during movement and can result from space-occupying lesions, such as ganglion cysts. This case highlights an unusual presentation of trigger wrist associated with CTS caused by a ganglion cyst in the flexor tendon sheath. CASE PRESENTATION: A 44-year-old male presented with paresthesia, severe pain, and triggering of the right wrist. Imaging and histopathology confirmed a ganglion cyst between the flexo

Updates in ultrasound-guided musculoskeletal interventions.

Musculoskeletal ultrasound (MSK-US) has evolved from a diagnostic tool into a key modality for guiding minimally invasive interventions across the musculoskeletal system. Its real-time imaging, portability, and absence of ionizing radiation make it a cost-effective and safe alternative to fluoroscopy and CT for numerous procedures, including biopsies, aspirations, injections, and percutaneous releases. Recent technological advances in ultrasound, including high-frequency probes, microvascular imaging, needle-tracking systems, and robotic probe steering, have improved precision and expanded its

Incidence, Timing, and Predictors of Staged Contralateral Carpal Tunnel Release at Minimum Ten-Year Follow-Up from Unilateral Surgery.

PURPOSE: Carpal tunnel syndrome is one of the most prevalent upper-extremity disorders. It may present unilaterally, bilaterally, or on the contralateral hand at a subsequent time to initial carpal tunnel release (CTR). We aimed to assess the incidence, timing, and factors associated with contralateral CTR at the time of an index unilateral surgery. METHODS: We performed a retrospective chart review of 1,291 primary unilateral CTR surgeries performed from 2008 to 2013 with a minimum follow-up period of 10 years within our integrated health network. In total, 742 (58%) patients met our inclusi

The Role of the Hand Surgeon in the Diagnosis of Amyloidosis.

Amyloidosis is a disorder characterised by progressive extracellular deposition of misfolded proteins, which can result in significant cardiac and neurologic morbidity. Traditionally, haematologists and cardiologists have served as primary diagnosticians for this disease when patients present with symptomatic systemic disease. However, musculoskeletal manifestations of the disease, such as carpal tunnel syndrome, trigger finger, spontaneous distal biceps tendon rupture and rotator cuff disease, are now recognised to frequently precede cardiac or neurologic involvement by several years. With th

Trigger finger and the risk of systemic inflammatory and autoimmune rheumatic diseases: real-world evidence.

BACKGROUND: Trigger finger (TF) is a common hand disorder traditionally viewed as a localized mechanical condition. Recent evidences suggests potential links between tenosynovial pathology and systemic inflammatory diseases, especially rheumatoid arthritis, though large-scale longitudinal data remain limited. METHODS: We conducted a retrospective cohort study using the TriNetX Global Collaborative Network. Adults diagnosed with TF between 2005 and 2018 were identified and compared with individuals undergoing routine health evaluations without TF. Patients with prior malignancy, systemic rheum

Postoperative pain after carpal tunnel or trigger digit release using local anaesthesia with tourniquet or WALANT.

INTRODUCTION: Local anaesthesia with tourniquet (LA-T) and wide-awake local anaesthesia no tourniquet (WALANT) are both commonly used in hand surgery. However, the differences in postoperative pain between these methods in various procedures are unclear. METHODS: We conducted a retrospective single-surgeon case series of 356 procedures (184 LA-T; 172 WALANT) between 2015 and 2019. Included cases were unilateral carpal tunnel release and single-digit A1 pulley release. Postoperative pain was assessed on days 1-3 and at 2 weeks using patient-reported pain scores, sleep disturbance, hospital vis

Synovial Biopsy as a Diagnostic Clue for Transthyretin Amyloidosis With the Rare Tyr98Phe Variant Presenting Predominantly With Severe Musculoskeletal Manifestations.

Hereditary transthyretin amyloidosis (ATTRv), the most common form of hereditary amyloidosis, results from autosomal dominant mutations in the TTR gene. Clinical manifestations of ATTRv are typically driven by extracellular amyloid deposition in multiple organs, most notably the peripheral nerves, heart, gastrointestinal tract, and musculoskeletal (MSK) system. We report the case of a 69-year-old man presenting with a two-year history of paresthesias and hypoesthesia in the lower limbs and a complex MSK history, including bilateral carpal tunnel syndrome, multiple trigger finger surgeries, rig

Screening for Transthyretin Amyloidosis During Carpal Tunnel Release: An Opportunity for Early Diagnosis.

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive infiltrative disease of the myocardium. Early diagnosis is essential for the timely initiation of disease-modifying therapies. Carpal tunnel syndrome (CTS) often precedes cardiac manifestations by several years, making carpal tunnel release (CTR) a unique window for tissue-based screening. This review summarizes current evidence on the prevalence of transthyretin (TTR) deposits during CTR, explores clinical risk factors, and proposes a multidisciplinary management pathway for biopsy-positive patients. Reported prevalence of TTR de

Trigger Digits in Mucopolysaccharidosis Type I: Clinical Characteristics, Surgical Outcomes, and Histopathological Findings.

PURPOSE: Mucopolysaccharidosis type I (MPS-I), a genetic lysosomal storage disorder, often leads to musculoskeletal deformities such as trigger digits (TDs). Despite systemic therapies, connective tissues such as bone, muscle, and tendons remain difficult for the enzyme to reach, and TDs develop. We performed a retrospective study to assess the prevalence of TDs, evaluate surgical outcomes and recurrence, and explore accumulation of foam cell macrophages contributing to TD development in patients with MPS-I. METHODS: Data were obtained retrospectively from a cohort of 33 children born between
