Targeted tenosynovial biopsy during carpal tunnel release for detection of transthyretin amyloid deposits.

INTRODUCTION: Carpal tunnel syndrome is increasingly recognized as an early musculoskeletal manifestation of transthyretin amyloidosis. Tenosynovial biopsy during carpal tunnel release may detect transthyretin amyloid deposits, but the yield of a targeted strategy remains uncertain. METHODS: We conducted a single-centre comparative before-after study of 347 patients undergoing carpal tunnel release with tenosynovial biopsy. The non-targeted cohort was recruited between October 2021 and July 2023 and the targeted cohort between October 2023 and May 2026. Targeting was based on predefined clini

Occupational Therapy at the First Visit: A Case for Joint Consultation in Hand Care.

For many common hand conditions, such as carpal tunnel syndrome, trigger finger, and thumb base arthritis, both surgery and nonsurgical care can lead to good outcomes. The best choice depends on the patient's occupations and goals, so the patient should help make it. This is what shared decision-making, choosing together after weighing the options, is meant to do. In practice, though, patients often meet an occupational therapist only after deciding on surgery, so the nonsurgical option gets little attention when the choice is made. We argue that the occupational therapist belongs at the first

Management of Pregnancy-related Hand and Upper-extremity Conditions and Trauma: Pain Control, Anesthesia, and Other Perioperative Considerations.

Upper-extremity conditions, including carpal tunnel syndrome, de Quarvians disease, trigger finger, and joint laxity, are common during pregnancy and are related to hormonal fluctuations and fluid shifts. These conditions generally respond well to conservative management with medications, bracing, and corticosteroid injection. However, surgery should be offered during pregnancy if nonoperative treatment fails. Regional and local anesthesia decreases fetal anesthetic exposure, minimizes airway issues, and assists postoperative pain control. Pregnant patients with trauma should be evaluated for

Concordance analysis of common hand pathology referrals from primary care to plastic surgery: a retrospective cross-sectional study.

BACKGROUND: Hand conditions are commonly referred from primary care to plastic surgery; however, no prior studies have assessed diagnostic concordance between family physicians and plastic surgeons. OBJECTIVES: This study aimed to evaluate the accuracy and agreement of referring diagnoses for common hand pathologies. METHODS: We conducted a retrospective cross-sectional study of 141 adult referrals from family physicians to plastic surgery for hand conditions at a tertiary care center over a 1-year period. Referrals were classified into five categories: correct, incorrect, missed main diagno

miR‑324‑5p in oncology: Molecular mechanisms and clinical outlook (Review).

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation

[Early Detection of Systemic Amyloidosis through Routine Histological Examination in Risk Patients Undergoing Carpal Tunnel Surgery].

Carpal tunnel syndrome (CTS) may represent an early manifestation of systemic amyloidosis. Musculoskeletal manifestations such as CTS, trigger finger, or Dupuytren's disease may precede cardiac or neurological symptoms by many years, thereby opening a potentially relevant diagnostic window. We conducted a retrospective-prospective analysis of all carpal tunnel release procedures performed at our center between 2013 and 2025. For the period from 2013 to 2023, demographic data and information on tissue excision and histological examination were collected. For 2024, an additional retrospective his

Juvenile Hyaline Fibromatosis Presenting as Progressive Flexion Contracture of the Middle Finger in a Child: A Case Report.

Juvenile fibromatosis is a rare benign fibroproliferative condition that may cause functional impairment when affecting the upper extremity. Deep involvement of flexor tendons in the hand is particularly unusual. An 8-year-old girl presented with a 6-year history of progressive flexion contracture of the left middle finger, causing inability to extend or grasp objects. Examination showed severe proximal interphalangeal and distal interphalangeal contractures of the middle finger and mild proximal interphalangeal contracture of the index finger. Radiographs revealed deformity without bony abnor

Screening for amyloidosis in trigger finger release and Dupuytren's fasciectomy.

INTRODUCTION: This study investigated the prevalence of amyloid deposition in tissue obtained during trigger finger release and limited fasciectomy for Dupuytren's disease and whether these procedures may allow early detection of cardiac amyloidosis. METHODS: This prospective, explorative study enrolled men aged >50 years and women >60 years undergoing trigger finger release or limited fasciectomy at two centres, excluding patients with known amyloidosis. Surgical specimens (A1 pulley or Dupuytren cord/nodule) were histologically analysed using Congo red staining. Positive samples underwent i

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Ultrasound-guided trigger finger release with a minimally invasive knife: A retrospective analysis of 297 releases.

Trigger finger, or stenosing tenovaginitis, is a common condition characterized by impaired flexor tendon sliding due to thickening of the A1 pulley. While open surgical release remains the gold standard for the treatment of persistent trigger finger, there is increasing interest in minimally invasive ultrasound-guided techniques to improve precision and outcomes. The purpose of this study is to evaluate the outcomes, safety, and complications associated with ultrasound-guided trigger finger release using a minimally invasive surgical knife. We performed a retrospective analysis of 297 trigger

A collaborative interdisciplinary approach for trigger finger management.

BACKGROUND: Stenosing tenosynovitis, or trigger finger, is a common cause of hand disability. This study outlines a trigger finger management protocol that redirects referrals for surgical consultations to conservative management first. PURPOSE: The primary outcome variable was the protocol endpoint based on the resolution of trigger finger symptoms (i.e. pain and triggering). Secondary outcome measures were to identify which patient characteristics were associated with an increased need for (i) surgical management and (ii) corticosteroid injections. STUDY DESIGN: Retrospective chart review.

Concurrent Perioperative Benzodiazepine and Opioid Utilization in Opioid-Naive Patients Undergoing Soft Tissue Hand Surgery.

BACKGROUND: While concomitant opioid and benzodiazepine use is discouraged due to an increased risk of sedation/overdose, the extent of perioperative opioid utilization in hand surgery patients already using benzodiazepines is unknown. METHODS: Using an administrative claims database, we identified adults undergoing carpal tunnel, DeQuervain, or trigger finger release, palmar fasciectomies, ganglion/mucoid cyst removals, and hand/wrist soft tissue mass excisions from 2011 to 2021. We identified opioid-naive patients with a benzodiazepine prescription within 90 days before surgery. Outcomes in

A1 pulley cutting potential and safety of three ultrasound-guided percutaneous A1 pulley release techniques for trigger finger: a cadaveric study.

BACKGROUND: There is no consensus as to the best technique for percutaneous trigger finger release. METHODS: This assessor-blinded study compared three ultrasound-guided percutaneous trigger finger release techniques using a needle (N), a needle-knife (NK), and a specially designed knife (K). Three physicians simulated A1 pulley release surgery on 56 fingers of 14 fresh-frozen hand cadaver body donors. Both the physicians and the fingers included were randomly selected. RESULTS: The results of repeated-measures ANOVA revealed significantly longer cuts for the NK and K techniques, than for th
