Trigger Finger: A Narrative Review of Dynamic Ultrasound and Personalized Therapies.

Trigger finger (TF) is a prevalent hand condition characterized by impaired tendon gliding and pulley thickening, leading to pain and functional limitations. This narrative review explores TF's functional assessment, sonographic findings, and conservative treatments. Tools like the Quinnell Grading System and Jamar Dynamometer effectively quantify severity and function, while ultrasound identifies hypoechoic A1 pulley thickening (> 0.62 mm) and tendon snapping. Conservative treatments, including splinting and tendon gliding exercises, reduce pain by 70% within weeks. Ultrasound-guided corticos

Biomechanical modeling of finger flexion based on imaging data: Implications on trigger finger surgery.

PURPOSE: The risk of bowstringing increases significantly after an excessive release of the A2 pulley in patients with trigger finger. However, the safe extent of this release has not yet been determined. Using a biomechanical model of finger flexion, we examined tendon excursion efficiency and moment arms relative to finger joints. This analysis allowed us to determine the safe extent of A2 pulley release. METHODS: Computed tomography and magnetic resonance imaging data of eight subjects were used for the modeling. We simulated the incremental release of the A1 and A2 pulleys and compared mo

Trigger Finger Caused by a Solitary Osteochondroma of the Proximal Phalanx in an Adolescent: A Case Report.

Trigger finger is a common disease, and stenosing tenosynovitis is the most frequent cause of this condition in middle-aged women. We report a case of surgical excision in a 16-year-old adolescent male with trigger finger symptoms caused by a solitary osteochondroma at the proximal end of the proximal phalanx. Although tumorous lesions are a rare cause of trigger finger, a differential diagnosis is necessary because the treatment strategy and surgical technique employed to treat this condition differ from those employed for common causes, such as stenosing tenosynovitis and pediatric trigger t

Cadaveric Comparison of Work of Flexion Following Flexor Tendon Repair: Knotted Versus Knotless Techniques.

BACKGROUND: Few prior studies have compared work of flexion of digital flexor tendons repaired with barbed suture to those performed with conventional knotted techniques. The available evidence is mixed, with one study indicating no difference between the techniques and another showing increased work of flexion with knotless techniques. This study aimed to bolster the current body of literature by comparing work of flexion and strength of digital flexor tendon repairs using knotted and knotless techniques. METHODS: Two knotted and 2 knotless techniques for flexor tendon repair were assessed.

Prevalence of Musculoskeletal Manifestations and its Associated Factors in Patients with Type 2 Diabetes Mellitus in Ernakulam District: A Cross-Sectional Study.

INTRODUCTION: India is referred to as the "Diabetes Capital of the World" with a weighted prevalence of diabetes of 11.4%. In addition to microvascular and macrovascular complications, musculoskeletal manifestations of diabetes mellitus (MMDMs) also cause significant pain, morbidity, and a decrease in quality of life. Previous studies in our country were from tertiary healthcare centres, and the actual community-based prevalence of MMDMs is unknown. The study aims to determine the prevalence of MMDMs and the factors associated with them in the Ernakulam district of Kerala. METHODS: A communit

Safety and Efficacy of Low-Dose Triamcinolone Injection without Injection Frequency Limitation for Trigger Finger.

PURPOSE: Triamcinolone tendon sheath injection is a useful nonsurgical treatment for trigger finger; however, complications, such as tendon rupture, and infections caused by excessive administration, have been reported. Considering the complication risk, we inject 4 mg triamcinolone into the tendon sheath without limitation on the number of injections at intervals of at least 1 month. This study aimed to retrospectively examine the results of triamcinolone tendon sheath injections for trigger finger at multiple facilities. METHODS: The participants included patients with trigger finger who vi

Outcome of Pulley Release via a Radial Mid-Lateral Approach for the Trigger Finger at the A2 Pulley.

Background: Trigger finger occurs primarily at the A1 pulley. However, triggering of the flexor tendon can also occur at the A2 pulley. The full release of the A2 pulley should be avoided because it may cause bowstringing and flexion weakness. To reduce the incidence of postoperative complications, we have performed a complete release of the A1 pulley via a volar approach and a complete release of the A2 pulley at the radial edge via a radial mid-lateral approach. This study aimed to investigate the outcome of our procedure. Methods: Seventeen fingers underwent A2 pulley release at the radial

Structural and Distribution Analysis of Mechanoreceptors and Free Nerve Endings in the Annular Pulleys of the Flexor Tendons of the Hand.

BACKGROUND: The hand is essential in enabling physical interaction with the environment, with the annular pulleys playing a key role in its functionality. These fibrocartilaginous structures, located along the flexor tendons, are vascularized and may contain nerve endings and mechanoreceptors, aligning tendons with the bones to optimize finger flexion. However, their innervation remains poorly understood, limiting therapeutic advancements for flexor tendon conditions, such as stenosing tenosynovitis (trigger finger). This study aims to analyze the presence and distribution of mechanoreceptors

Black holes in hand anatomy: exploring research areas in hand surgery.

The term 'black holes' has been applied to anatomical areas needing further study. The historical account On Growth and Form (D'AW Thompson, Cambridge University Press, 1917) applies mathematical principles to explain how physical forces influence the morphology of bone, tendon, ligament and fascia. The trabecular structure of bone is maintained by force loading, providing a map of strain concentrations to better understand fracture fixation and bone healing. The flexor tendon is classified in zones of sheath pathology, but the site of tendon injury depends on the posture at the moment of inju

Management of Stenosing Tenosynovitis With the Eastwood Technique: Handling, Advantages, and Disadvantages.

Stenosing tenosynovitis, or trigger finger, is a common condition resulting from repetitive use of the finger tendons, causing inflammation and narrowing of the flexor sheaths. This causes difficulty in sliding the tendon, which can lead to locking or pain when extending the fingers. The condition is associated with chronic diseases, such as diabetes and rheumatoid arthritis, and has a higher incidence in people between the ages of 40 and 60 years. Treatments include conservative approaches, such as the use of anti-inflammatory drugs and corticosteroids, or definitive approaches, such as surge
