Comparison of Ultrasonographic Features Among Healthy Individuals and Patients With Trigger Finger.

PURPOSE: The trigger finger is one of the leading causes of finger pain and triggering. The flexor tendons are stabilized by a fibrous band called the annular pulley. The first annular pulley (A1 pulley) is located at the level of the metacarpophalangeal joint. Trigger finger is characterized by thickening of the A1 pulley. There is a lack of clinical and ultrasonographic risk factors that may help clinicians. The main purpose of the study was to measure the thickness of the flexor tendon and A1 pulley in patients with trigger finger compared to healthy volunteers and to reveal risk factors of

Closed Intratendinous Rupture of the Flexor Digitorum Profundus: A Rare Cause of Pediatric Trigger Finger.

Paediatric trigger finger (PTF) is a rare condition, far less common than paediatric trigger thumb, and often associated with anatomical anomalies or systemic disease. Unlike trigger thumb, which may resolve spontaneously, PTF usually requires surgical treatment. Trauma is an uncommon cause, and closed intratendinous rupture of the flexor digitorum profundus (FDP) has not previously been reported in a child following direct injury. We present the case of a healthy 12-year-old boy who developed triggering of the right middle finger eight weeks after blunt trauma during rugby. Examination reveal

From Glycemia to Grip: A Comprehensive Review of Musculoskeletal Complications in Diabetic Patients.

Diabetes mellitus is a chronic metabolic disorder with well-established vascular complications. Its impact on the musculoskeletal (MSK) system is often underrecognized in clinical practice. MSK disorders contribute significantly to pain, functional limitations, and decreased quality of life in individuals with diabetes. However, they remain inadequately addressed in primary care settings. This narrative review aims to synthesize current evidence on the prevalence, assessment, pathophysiology, clinical features, and management of MSK complications in individuals with diabetes, with an emphasis

Clinical Review of Trigger Finger in Pediatric and Adult Patients: Evaluation and Management Strategies.

Trigger finger (TF), also known as stenosing tenosynovitis, is a condition characterized by pain, locking, and restricted motion of the digits due to narrowing and inflammation of the flexor tendon sheath, most commonly at the A1 pulley. This review examines both adult and pediatric TF (PTF), emphasizing pathophysiology, diagnosis, and evidence-based treatment strategies. In adults, TF is strongly associated with repetitive finger movements, diabetes mellitus, and inflammatory arthritides. Conservative treatments such as nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroid injections,

Effect of percutaneous release versus steroid injection among adults with trigger fingers: a randomized clinical trial.

BACKGROUND: Trigger finger (TF) causes pain and impaired hand function. Percutaneous release of the A1 pulley demonstrates a better outcome than steroid injection in the treatment of TF; however, evidence remains limited. Therefore, this study aimed to compare the effect of percutaneous release of the A1 pulley compared to local steroid injection in the treatment of trigger fingers in Nepal. METHODS: A hospital-based randomized clinical trial among 92 patients aged 18 years and above suffering from trigger fingers that were unresponsive to conservative treatment was conducted to evaluate the

Dorsal Approach for Corticosteroid Injection in Trigger Finger Management: A Scoping Review.

Introduction: Stenosing tenosynovitis (trigger finger) is a common condition caused by inflammation and hypertrophy of flexor tendons. Corticosteroid injection (CSI) is an effective and safe treatment option. A palmar approach for CSI is typically used; however dorsal injection, which may be less painful, is not well-studied. Methods: A 6-stage scoping review was conducted to characterize outcomes associated with dorsal CSI for trigger finger management. We searched Ovid MEDLINE, EMBASE, and Web of Science for eligible articles in English from inception to July 2024. Data regarding study chara

Ultrasound-Guided Percutaneous Versus Open A1 Pulley Release for Trigger Finger: A Randomized Controlled Trial.

Background/Objectives: Trigger finger is a common cause of hand pain and functional limitation. Although open A1 pulley release remains the standard surgical treatment, ultrasound-guided percutaneous needle release (UGPR) has emerged as a minimally invasive alternative. This study aimed to compare the clinical effectiveness and safety of UGPR with open surgery. Methods: In this prospective, randomized controlled trial, 146 patients with Green stage 2-4 trigger finger were randomly assigned to UGPR (n = 75) or open release (n = 71). Pain (VAS), functional status (QuickDASH), and symptom severit

Influence of Sociodemographic Factors on Patient Completion of Patient-Reported Outcome Measures in Hand Surgery Patients.

PURPOSE: Patient-reported outcome measures (PROMs) are standardized surveys that assess health outcomes, symptoms, functional status, and quality of life. Although prior studies have evaluated completion and/or participation across orthopedic subspecialties, no current studies have examined both completion and participation outcomes specifically in hand surgery. This study investigates the association between sociodemographic factors and PROM completion and participation in hand surgery patients. METHODS: A retrospective review was conducted of patients who underwent carpal tunnel release, tr

Effectiveness of steroid injection on functional outcomes in adults with trigger finger: a systematic review and meta-analysis protocol.

INTRODUCTION: Corticosteroid injections are widely used as first-line treatment for trigger finger, but their comparative efficacy against other non-surgical and surgical interventions remains unclear. While previous meta-analyses have explored this topic, many were limited by a narrow scope or methodological constraints. This systematic review and meta-analysis aims to comprehensively evaluate the effectiveness and safety of corticosteroid injections in adult trigger finger management compared with alternative treatment modalities, using robust methodology and updated evidence to guide clinic

Comparative effectiveness of corticosteroid injections for trigger finger: A systematic review of randomized controlled trials.

BACKGROUND: Trigger finger is a common condition marked by painful catching or locking of a digit during motion. Although corticosteroid injections (CSIs) are widely used, there is limited consensus on the optimal corticosteroid type, dose, injection technique, or use of adjunctive therapies. This systematic review examined the effectiveness and safety of methylprednisolone and triamcinolone in achieving symptom remission. METHODS: A systematic search was conducted across PubMed, Embase, and MEDLINE from inception to May 2024. Inclusion criteria focused on randomized controlled trials (RCTs)

Efficacy of Splinting in Managing Adult Trigger Finger: A Systematic Review of Short-Term Outcomes.

PURPOSE: Trigger finger is a common hand condition often managed conservatively with splinting, which reduces pain and improves function by immobilizing the affected digit. Splinting is a viable alternative for patients wishing to avoid corticosteroid injections or surgery. Short-term studies suggest it effectively relieves symptoms; however, adherence can be challenging. This systematic review evaluates the short-term efficacy of splinting for trigger finger and aims to identify the most effective splint. METHODS: A systematic review was conducted following Preferred Reporting Items for Syst

A Two-Year Retrospective Comparison of Clinical Outcomes Between Open Release and Percutaneous Release of the Trigger Finger.

BACKGROUND: Trigger finger is often found in adults, accompanied by pain, swelling, and limited movement. If conservative treatment fails, surgical release of the A1 pulley will be performed. Percutaneous trigger finger release is a new surgery method with rapid recovery that includes a good outcome, but there is also more reluctance to use this standard technique than open surgery. This article, therefore, presents a comparison of the long-term outcomes of both methods across different factors, similarities, and patient groups. The study hypothesized that the percutaneous trigger finger relea

Predictors and Incidence of Requiring Subsequent Injection or Surgery in the Same Digit Following Trigger Finger Release.

BACKGROUND: Currently, there is limited literature that highlights the rate of reoperation or requiring additional corticosteroid injections following surgical trigger finger release. This study determined the incidence of additional corticosteroid injection or reoperation of the same digit in patients who underwent trigger finger release. Risk factors for requiring subsequent treatment in the same digit following trigger finger release were also identified. METHODS: This retrospective cohort study evaluated patients who underwent primary trigger finger release from 2015 to 2017. The incidenc

Updates in non-neoplastic orthopaedic pathology: what you don't know can hurt you!

Even though the average surgical pathologist reviews far more non-neoplastic orthopaedic pathology on a daily basis, most current research focuses on rare tumours and their even less frequent molecular events. Our experiences among consults and focused conferences strongly suggest that there remains a practice gap regarding knowledge and diagnosing specific non-neoplastic orthopaedic conditions. One of the most frequent intraoperative consultations performed in the USA, among both academic and private institutions, relates to revision arthroplasty and the determination of infection in peripros

The Effect of Tourniquet Deflation Timing on Outcomes of Common Upper Extremity Surgeries.

Background Historically, upper extremity procedures have relied on tourniquets to achieve hemostasis and provide a bloodless surgical field. However, a shift away from routine tourniquet use occurred in the early 2000s with the popularization of the wide-awake local anesthesia with no tourniquet (WALANT) technique. Despite this trend, many surgeons still prefer pneumatic tourniquets, which may improve visualization and operative efficiency. The purpose of this study was to compare early postoperative pain and function between procedures where the tourniquet was deflated after final wound closu

A systematic review of the use of corticosteroid injections with and without local anaesthetic for trigger finger.

BACKGROUND: Trigger finger causes pain, catching, and locking of the finger, with corticosteroid injection being the first-line treatment. Injections with local anaesthetic may reduce immediate pain, but it has been suggested that added volume worsens pain. This systematic review aimed to evaluate studies investigating pain following corticosteroid injection for trigger finger, with or without local anaesthetic. Evaluation of the topic: We performed a prospectively registered systematic review (PROSPERO ID: CRD42024502361) following PRISMA guidelines. We searched PubMed, Cochrane Library, and

Short-Term Effects of Capacitive and Resistive Electric Transfer Therapy and Static Splinting in the Management of Trigger Finger: A Non-Randomized Clinical Study.

Trigger finger (TF) is a common debilitating hand disorder that often results in pain and functional limitations. Currently accepted conservative treatments include splinting, nonsteroidal anti-inflammatory drugs, and corticosteroid injections. Recently, transfer energy capacitive and resistive (TECAR) therapy is emerging as a promising intervention for its potential to enhance microcirculation, reduce pain and inflammation, and stimulate tissue regeneration in various musculoskeletal conditions. However, its effectiveness for TF remains unclear. This study aims to compare the outcomes between

Ultrasound-Guided Percutaneous Tenotomy for Trigger Finger: A Minimally Invasive Office-Based Case.

Trigger finger is frequently managed with conservative therapy and corticosteroid injections, while surgery is reserved for refractory cases. Ultrasound-guided percutaneous tenotomy has emerged as a minimally invasive alternative that allows direct visualization of the A1 pulley, flexor tendons, and surrounding neurovascular structures. We present the case of a 70-year-old man with Grade 3 stenosing tenosynovitis of the left middle finger and Grade 2 involvement of the left index finger who experienced only partial improvement after a landmark-based corticosteroid injection.
