Surgical Treatment of Anchor-induced Heel Pain after Minimally Invasive Repair of Acute Achilles Tendon Rupture Using Percutaneous Achilles Repair System Achilles Midsubstance SpeedBridge™ Repair: A Case Report.

INTRODUCTION: Minimally invasive repair of acute Achilles tendon rupture (ATR) using the percutaneous Achilles repair system (PARS) represents a reliable surgical option, with complication rates comparable to other techniques. Heel pain at the anchor insertion site has been reported as a post-operative complication but is rarely discussed in the literature. This report describes the successful treatment of persistent heel pain through surgical anchor excision. CASE REPORT: A 24-year-old man sustained a football-related injury to his left ankle in May 2023. He was referred to the emergency dep

Incidence of Heterotopic Ossification Following Double-Incision Technique for Distal Biceps Tendon Repair.

Background Heterotopic ossification (HO) is a postoperative finding that may occur after distal biceps tendon repair using the double-incision technique; however, its incidence, temporal evolution, and clinical relevance remain incompletely defined. Objectives The objectives of this study are to determine the incidence and temporal evolution of heterotopic ossification (HO), evaluate variables associated with its development, and assess clinical outcomes following double-incision distal biceps tendon repair performed under a standardized postoperative protocol including non-steroidal anti-infl

Spontaneous Quadriceps Tendon Rupture Associated With Severe Secondary Hyperparathyroidism in a Patient on Chronic Hemodialysis: A Case Report.

Spontaneous or minimal-trauma ruptures of the quadriceps tendon are an exceptional clinical entity, representing a highly unusual presentation even among patients with underlying metabolic conditions, including uremic patients on chronic hemodialysis. We report the case of a 40-year-old male patient on chronic hemodialysis with severe secondary hyperparathyroidism, who was admitted for a rupture of the left quadriceps tendon following minimal trauma. Treatment consisted of the surgical reconstruction of the extensor apparatus and management of his hyperparathyroidism.

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Multifocal Osseous and Soft Tissue Blastomycosis Infection.

A 15-year-old male with no significant medical history presented with a 4-month history of bilateral wrist pain and a 2-month history of rapidly growing, spontaneously ulcerating bilateral volar wrist nodules. He denied fevers, fatigue, weight loss, illicit drug use/injection, or local trauma including self-harming behavior. Ultrasound and MRI demonstrated strikingly symmetric ill-defined mass-like tissue underlying each wound invading the bilateral distal radii but without mass effect or invasion of traversing bilateral radial arteries and flexor carpi radialis tendons. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Drug-Associated Tendinopathies and Ligament Disorders: Results from a Retrospective Pharmacovigilance Study Using Disproportionality Analysis.

Background: Tendinopathies and ligament disorders are significant musculoskeletal adverse events associated with various drugs, leading to restricted mobility and reduced quality of life. Although certain drug classes such as fluoroquinolones and corticosteroids have established links to these conditions, there is limited research on other potential drug associations. This study aimed to comprehensively evaluate drugs associated with tendinopathies and ligament disorders using data from the USFDA Adverse Event Reporting System (AERS). Methods: A retrospective pharmacovigilance study utilizing

Attritional rupture of the extensor pollicis longus tendon secondary to an osteochondroma of the distal radius.

The extensor pollicis longus (EPL) tendon, given its complex anatomical course, is at a higher risk of rupture when compared with other extensor tendons of the hand. This is most often seen in the trauma setting, and up to 60% of cases are related to an underlying distal radius fracture. Spontaneous atraumatic ruptures are uncommon and often attributable to prolonged steroid use and tenosynovitis.Attritional ruptures from osseous lesions of the distal radius are not well described, and we are unaware of any cases related specifically to distal radius osteochondroma. This may be due to the rare

[Amyloid and orthopedic pathology : Early diagnosis may allow live-saving treatment of systemic amyloidosis].

Amyloidosis denotes a group of rare, but often fatal diseases which result from abnormal folding and deposition of insoluble proteins with subsequent organ damage especially of the heart and the kidneys. Histologic identification of amyloid by Congo red staining and by immunohistochemistry in the musculoskeletal system by orthopedic pathology especially in specimens from surgery for carpal tunnel syndrome, lumbar spinal stenosis and spontaneous tendon rupture, but also from other sites of the musculoskeletal system may facilitate early diagnosis and treatment of amyloidosis. Appropriate techni

Prevalence of amyloid deposition in distal biceps tendon ruptures: a retrospective review of intraoperative biopsy.

BACKGROUND: In a previous study, one-third of patients with heart failure secondary to wild-type transthyretin amyloidosis were found to have distal biceps tendon (DBT) ruptures, and a large proportion of these ruptures were spontaneous or asymptomatic. This rate is higher than the 2.5% incidence of DBT ruptures seen in patients with other causes of heart failure. DBT ruptures can be used as a screening opportunity for early detection of systemic amyloidosis. However, when a subsequent study investigated the rate of amyloid deposition in 30 patients with acute DBT ruptures, only 3% had evidenc

Unexplained Groin Pain in the Elderly: Spontaneous Iliopsoas Tendon Rupture With Haematoma in a 90-Year-Old Woman.

Hip and groin pain are frequent presenting complaints among elderly patients. These symptoms are most commonly related to fractures, which are usually identifiable on X-rays. Spontaneous iliopsoas tendon injury, however, is a rare and under-recognised cause of atraumatic hip or groin pain in this population. We report the case of a 90-year-old woman with a history of multiple comorbidities on long-term warfarin for atrial fibrillation who presented with dull left groin pain radiating to the lower back and marked limitation of mobility in the absence of recent trauma. On admission, the internat

Spontaneous Idiopathic Bilateral Sagittal Band Rupture: A Case Report.

INTRODUCTION: Extensor tendon subluxation or dislocation resulting from sagittal band rupture can be associated with snapping, catching, locking, pain, and swelling of the finger. Several etiologies were reported in the literature, with frequent mention of a traumatic rupture caused by a direct blow at the level of the metacarpophalangeal joint (MCPJ). The other causes of sagittal band injuries are degenerative disease, congenital infection, and rheumatoid arthritis. To our knowledge, we report the first case of spontaneous idiopathic bilateral sagittal band rupture. CASE REPORT: We, herein,

[Long head of the biceps pathologies : diagnosis and treatments].

The long head of the biceps (LHB) tendon is a common source of shoulder pain. Often associated with other injuries, the tendon can alone be responsible of the symptoms. Spontaneous rupture has been observed to allow for pain relief. The spectrum of LHB lesions is broad. Repetitive motion, carrying heavy loads, have been associated with diagnosis. Specific tests have been described to diagnose biceps injuries, their implementation can remain difficult in the presence of associated pathologies. The combination of a history and clinical examination with the use of targeted imaging is required to

Avulsive Achilles Tendon Rupture in a Patient With Alkaptonuria: A Case Report.

Alkaptonuria is a rare autosomal recessive disease caused by a mutation in the homogentisate 1,2-dioxygenase (HGO) gene, leading to the accumulation of homogentisic acid (HGA). HGA polymerizes to form a black pigment that accumulates in connective tissue and joints (ochronosis), causing their destruction. In this work, we report a case of Achilles tendon rupture in a patient with a prior diagnosis of alkaptonuria. A 71-year-old man presented to the emergency department reporting pain in his posterior right ankle and dysfunction, evolving over three weeks after falling down a short flight of st

Spontaneous Midsubstance Rupture of the Left Little Finger's Flexor Tendon in the Absence of Trauma.

Spontaneous midsubstance rupture of the flexor tendon without underlying pathology is rare. This case report describes a healthy 45-year-old man who experienced spontaneous midsubstance flexor tendon rupture at zone III. He presented with pain and disability in flexing his left little finger after carrying a plastic bag containing 20 kg and only sought medical treatment 10 days after the injury. Two-stage tendon reconstructive surgery was performed, involving the insertion of a nasogastric tube in the first stage followed by ipsilateral palmaris longus tendon grafting in the second stage. A Case Report of Extensor Pollicis Longus Tendon Rupture: Delayed Diagnosis in a Professional Rugby Player.

Spontaneous rupture of the extensor pollicis longus (EPL) tendon has been reported to be uncommon. Several possible causes and precipitating factors have been reported for this rupture, including prior surgery to the distal radius. We report the case of an 18-year-old male professional athlete who presented with an inability to extend their left thumb. They presented with this condition after open reduction and internal fixation (ORIF) for a fractured left distal radius. Findings were consistent with the rupture of the EPL tendon. The typical presentation of such a rupture is one to four month

Spontaneous Rupture of the Extensor Pollicis Longus Tendon: A Systematic Review.

BACKGROUND: Extensor pollicis longus (EPL) rupture and tenosynovitis of the third dorsal compartment is often described in association with a history of rheumatoid arthritis or in the setting of a distal radius fracture. However, the literature suggests multiple other potential factors that may lead to a seemingly spontaneous rupture. METHODS: We performed a systematic review following guidelines set forth by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. The search consisted of headings and keywords related to tendon injuries, tendinopathy, hand surgery, te

Amyloidosis and Considerations for the Hand Surgeon.

Amyloidosis can lead to cardiac, renal, and other multiorgan failure. New treatments have become available that can prolong survival but rely on early diagnosis. Manifestations of amyloidosis in hand surgery include carpal tunnel syndrome, trigger finger, peripheral neuropathy, and spontaneous distal biceps rupture. Often, these can predate systemic amyloidosis, offering hand surgeons an opportunity to diagnose patients with amyloidosis before systemic disease, refer them for treatment, and potentially alter disease course and prolong survival. In this review, we describe the pathophysiology a

Bilateral Spontaneous Supraspinatus Tendon Rupture Under Prolonged BRAF/MEK Targeting Treatment in a Melanoma Patient.

The B-Raf proto-oncogene, serine/threonine kinase (BRAF)/ mitogen-activated protein kinase kinase (MEK) targeting agents have become the treatment of choice for BRAF-mutated melanoma during the last decade. However, it is possible that some long-term adverse events of these drugs have not yet been reported. A case of bilateral spontaneous, non-traumatic, supraspinatus tendon rupture in a 65-year-old Caucasian male suffering metastatic melanoma under prolonged and successful combination treatment with dabrafenib plus trametinib is presented.
