[Ligament ruptures of the lower extremity in the elderly].

There is an increasing incidence of ligament ruptures of the lower extremities in older patients. This higher incidence is caused by the typical current demographic changes in the population and the higher level of activity and athletic motivation of the older people in our society. In this review we address the most important ligament ruptures of the lower extremity in the old patient. Quadriceps tendon and Achilles tendon ruptures are mostly the result of degenerative and abrasion changes. The ACL rupture on the other hand occurs spontaneously after adequate trauma and without former degener

Vaughan-Jackson-like syndrome as an unusual presentation of Kienböck's disease: a case report.

INTRODUCTION: Kienböck's disease is a condition of osteonecrosis of the lunate bone in the hand, and most patients present with a painful and sometimes swollen wrist with a limited range of motion in the affected wrist. Vaughan-Jackson syndrome is characterized by the disruption of the digital extensor tendons, beginning on the ulnar side with the extensor digiti minimi and extensor digitorum communis tendon of the small finger. It is most commonly associated with rheumatoid arthritis. We describe a case of a patient with an unusual presentation of Kienböck's disease with symptoms similar to t

Rupture of gastrocnemius tendon in broiler breeder hens.

Lameness and mortality of 0.55% to 1.00% per week occurred in 32-to-33-wk-old broiler breeder hens in a complex involving eight houses. Males were not affected. Laboratory evaluation of 19 birds, over 2 wk, revealed complete rupture or partial tear, primarily of the gastrocnemius tendon; the fibularis longus, fibularis brevis, and flexor muscles and their tendons were injured in a few birds. Rupture of the gastrocnemius tendon has been observed in broiler breeder chickens older than 12 wk of age and it is characterized by lameness secondary to unilateral or bilateral rupture of the tendon. The

Spontaneous totally thrombosed pseudoaneurysm mimicking a tendon tear of the wrist.

Wrist pseudoaneurysm is commonly caused by iatrogenic procedures such as blood gas puncture, arterial-venous shunting for hemodialysis, or noniatrogenic causes such as blunt trauma. The mechanism of pseudoaneurysm formation is arterial wall damage with subsequent blood leakage that collects in surrounding tissue. This article presents a case of a 32-year-old woman who had sudden onset left wrist pain with a snap sound and a mass 1 cm in diameter after pulling a heavy object while working. A wrist flexor tendon tear was diagnosed after evaluation. Musculoskeletal ultrasound revealed a partially

Morphological and biomechanical studies on the common calcaneal tendon in dogs.

Spontaneous rupture at the distal part of the gastrocnemius tendon (GT) is the second most common non-traumatic tendon injury in dogs, whereas the other strands of the common calcaneal tendon do not seem to have a predisposition to rupture. In order to discover why we investigated the common calcaneal tendons of 63 dogs microscopically and biomechanically. Both the gastrocnemius and superficial digital flexor tendon (SFT) had multiple low vascularized fibrocartilaginous areas within their distal course as opposed to regular parallel fibered areas in the proximal tendon areas. Biomechanical tes

Extensor tendon rupture and three-dimensional computed tomography imaging of the rheumatoid wrist.

PURPOSE: Extensor tendon rupture on the dorsum of the wrist is commonly seen in patients with rheumatoid arthritis (RA). The diagnosis of tendon rupture is usually straightforward, but it is sometimes difficult in the hand with complex deformity. The purposes of this study were to investigate the reliability of three-dimensional computed tomography (3DCT) imaging of extensor tendons in the rheumatoid wrist and in the normal wrist and to clarify the validity of its clinical application to the diagnosis of tendon rupture in the rheumatoid wrist. METHODS: Preoperative 3DCT images of 48 wrists of

Bilateral patellar tendon rupture at different sites without predisposing systemic disease or steroid use.

Simultaneous bilateral patellar tendon ruptures are extremely rare, and even more rare in patients without systemic disease. We describe bilateral simultaneous patellar tendon disruptions in the absence of systemic disease or steroid usage, with one tendon disruption at the inferior pole and the other an intrasubstance tear. The different locations of the ruptures are also exceedingly rare, as only two cases of non-identical ruptures have ever been reported. We also review all bilateral patellar tendon rupture case reports from English and German literature.

Clinical and morphological studies on spontaneous rupture of the gastrocnemius tendon in broiler breeders.

1. Clinical and morphological studies were performed in 8 broiler breeder flocks (one hybrid) originating from two different farms in Bulgaria, with an incidence of lameness in female birds between 3 and 4% on one farm and 7 and 8% on the other. Clinical and morphological studies were performed. 2. Ruptured gastrocnemius tendons were observed in 78 broiler breeders, 24 to 31 weeks old with signs of lameness, selected for pathoanatomical study from different flocks. Unilateral ruptures predominated, over 80%. 3. A histological study of 20 samples from one farm and 8 samples from the other revea

[Fluoroquinolone-induced Achilles tendon rupture].

HISTORY AND ADMISSION FINDINGS: A 72-year-old female dialysis patient with insulin-dependent diabetes mellitus who was under long-term medication with oral prednisolone due to chronic obstructive pulmonary disease was given levofloxacin for one week to treat an acute bronchitis (one 500 mg dose on the first day, 125 mg/day orally from second day onwards). One day after the end of levofloxacin treatment, the patient complained about a constant dragging pain above the right heel that receded under local application of diclofenac ointment and inactivity of the right foot. Twelve days after ending

A novel lesion of the infraspinatus characterized by musculotendinous disruption, edema, and late fatty infiltration.

Isolated, atraumatic rupture of the infraspinatus (IS) with associated edema of its muscle is a previously undescribed lesion. We retrospectively identified 19 patients with MRI detected, isolated lesions of the IS tendon or musculotendinous junction with associated muscle edema. The average age at the time of presentation was 47.7 years (range, 30-66). There were 15 females and 4 males. Fourteen patients were treated nonoperatively; 5 underwent an open repair. All patients underwent clinical and MRI follow-up at an average of 50.3 months (range, 24-79) after the initial MRI. Constant scores w

Spontaneous rupture of the anterior tibial tendon in a diabetic patient: results of operative treatment.

UNLABELLED: Spontaneous rupture of the tibialis anterior tendon is infrequently seen as a clinical entity. In this report, we describe the case of a diabetic neuropathic patient that underwent successful surgical repair of a spontaneously ruptured tibialis anterior tendon with no other factors that would render the tendon susceptible to injury or rupture. LEVEL OF CLINICAL EVIDENCE: 4.

An in vitro biomechanical study comparing the spiral linking technique against the pulvertaft weave for tendon repair.

A new spiral linking technique for tendon repair in which one end of the tendon is spiralled around the other end has been developed. Using pig trotter extensor tendons, the Pulvertaft weave technique was compared with this new technique. Twenty-five repairs using each technique were tested by tensile loading with an Instron testing machine. The spiral linking technique matched the strength of Pulvertaft method: the mean peak loads were 102 and 105 N, respectively. The Pulvertaft weave was stiffer than the spiral linking technique: mean stiffness of 11.1 and 6.7 N/mm, respectively. The spiral

Musculoskeletal adverse drug reactions: a review of literature and data from ADR spontaneous reporting databases.

The musculoskeletal system can be a target organ for adverse drug reactions (ADRs). Drug-induced muscle, bone or connective tissue injuries may be due to, i), primary direct drug action, or, ii), undirected consequence of generalized drug-induced disease. Musculoskeletal ADRs may be only temporarily disabling, such as muscle cramps, as well as in other cases may be serious and life-threatening, such as rhabdomyolysis. In the last few years there has been an increasing awareness of musculoskeletal ADRs. Some recent drug safety issues dealt with serious or uncommon musculoskeletal reactions like

Effect of wrist and ulna head position on gliding resistance of the extensor digitorum minimi and extensor digitorum communis III tendons: a cadaver study.

While attrition from sharp bony surfaces is the most common cause of extensor digiti minimi (EDM) tendon rupture, the etiology of other cases of spontaneous EDM tendon rupture is still unknown. Friction within the compartment may play a role, especially with ulna dislocation. The purpose of this study was to compare gliding resistance of the EDM tendon with that of a tendon which rarely ruptures spontaneously, the extensor digitorum communis of the middle finger (EDC III) tendon, under various wrist and ulna head positions. Eight fresh frozen cadavers were used. Gliding resistance between the

[Damage to large tendons: Achilles, patellar and quadriceps tendons].

The etiology and mechanisms of Achilles, patellar and quadriceps tendon ruptures are very similar. Age dependent changes in tendon structure and disorders such gout, diabetes, rheumatic diseases and chronic renal failure are associated causes. The main mechanism of rupture is indirect trauma. Although clinical diagnosis is easy, ruptures are still frequently missed. Sonography is the main standard diagnostic tool. MRI is indicated only in special cases. Open operative repair is the most common treatment for quadriceps and patellar tendon ruptures. Treatment of Achilles tendon ruptures is movin

Bilateral, simultaneous rupture of the quadriceps tendon: a diagnostic pitfall? Report of three cases and meta-analysis of the literature.

Bilateral, simultaneous quadriceps tendon rupture (QTR) represents a rare entity and delay in establishing the correct diagnosis is not uncommon. Another three cases are reported here and in all the correct diagnosis was missed initially. A review of the English and German literature retrieved 105 cases of bilateral, simultaneous QTR and in 32 patients (30.5%) the correct diagnosis was established with delay. In 28 cases--representing 25 men (89.3%) and 3 women (10.7%)--sufficient data was available for further analysis. In the majority of patients (n = 19/28; 67.9%) rupture was associated wit

[From diagnosis to treatment of gluteus medius tendon tear of the hip: a case report].

OBJECTIVE: To report a case of spontaneous gluteus medius tear repaired surgically in a 42-year-old woman patient without any other antecedent other than diabetes. PATIENT AND METHODS-RESULTS: Trochanteric pain evolving for 7 months led to scintigraphy objectifying a hyperfixation of the trochanter and ultrasonography showing an inflammatory gluteus medius tendon. These examinations were supplemented by magnetic resonance imaging and tomodensitometry visualising the tendon rupture but no marked fat degeneration of the muscle despite atrophy of this one. Surgical exploration confirmed the prese

Acute rupture of the anterior cruciate ligament and patellar tendon in a collegiate athlete.

In rare incidences of combined ruptures of the ACL and patellar tendon, either the patellar tendon ruptures or the associated ACL tear is often initially missed. Even when recognized, there is no established treatment regimen. We report a case of an intercollegiate football player with a combined rupture of the ACL and patellar tendon that was successfully treated by primary augmented repair of the patellar tendon along with ACL reconstruction. Similar to other reported cases, the mechanism of injury involved forceful eccentric contraction of the quadriceps against a fixed foot. Superior displ

Bilateral spontaneous rupture of Achilles tendons: a case report.

Spontaneous bilateral rupture of Achilles tendon is rare. Rupture of the Achilles tendon has been described in patients on oral corticosteroid therapy. The sudden dorsiflexion of the plantar-flexed foot is the usual mechanism of injury. Spontaneous bilateral rupture is common in the degenerated tendon, which is often seen in patients with long-term corticosteroid therapy. This case is unusual because the patient has never taken steroids. We discuss the mechanism of injury and other probable causes.
