In the course of a prospective study at our department the shoulder joints of 50 RA-patients were examined. Two criteria have been applied: only righthanded patients have been examined in order to ascertain any prevalence of changes in the dominant extremity, and only patients who were primarily admitted to our department because of complaints other than those of the shoulder. We evaluated specimens obtained from the biopsy of spontaneously ruptured tendons in 891 patients who were treated between 1968 and 1989. The specimens, which were removed at the time of repair, included 397 Achilles tendons, 302 biceps brachii tendons, forty extensor pollicis longus tendons, eighty-two quadriceps tendons and patellar ligaments, and seventy other tendons. A comparative study between percutaneous repair and open surgical repair of acute spontaneous Achilles tendon ruptures in young athletic patients is presented. Twenty-seven patients with acute Achilles ruptures were evaluated objectively and with subjective questionnaires. Fifteen of the patients were treated by reconstruction with a gastrocsoleus fascial graft (followup, 4.6 years) and 12 treated by percutaneous repair (followup, 1.8 years). During the period 1983-1988, 254 patients underwent simple end-to-end repair of closed ruptured Achilles tendons in our institution. During the same period, eight patients were treated with plastic repair on account of old ruptures, and 19 patients were treated conservatively on account of relative contraindication to open repair. The material of simple end-to-end repair is analyzed as regards factors which may be supposed to contributory to the rate of infection, wound healing and repeated rupture. The rate of major wound complication was low (2.4%) as was the rate of repeated rupture (1.6%).

Although patellar tendon rupture after total knee arthroplasty (TKA) is a rare complication, the consistently poor outcome of conventional tendon repair has convinced some to abandon such reconstruction in favor of a prospective protocol using an allograft distal extensor mechanism. The graft consists of a quadriceps tendon, a patella with a cemented prosthesis, a patellar tendon, and a tibial tubercle. The roentgenograms and the "Leclercq's manoeuvre" of 105 patients who had a shoulder arthrography were reviewed. We differentiated two groups: One with a rotator cuff tear. A 26-year-old man incurred bilateral spontaneous rupture of the patellar ligament. This may be the eighth case in previously healthy individuals reported in the literature. Further review of other case reports suggested various etiological factors. Surgical repair generally produces excellent results.

We evaluated the surgical results of 42 consecutive patients with spontaneous rupture of the Achilles tendon treated from 1973 to 1984 to determine the causes of rupture and to evaluate our treatment methods. Tendon lesions have been studied in 35 broiler or broiler breeder fowls. Tendon rupture most commonly affected the gastrocnemius and flexor muscles but was also seen in cranial tibial muscles (four cases) and the patellar tendon (one case). In nine birds tenosynovitis, characterised by a marked inflammatory response, was identified. In others, however, rupture occurred without appreciable inflammation and these cases were concluded to be comparable to spontaneous tendon rupture in other species. An assessment has been made of spontaneous orthopaedic disease in 100 male broiler breeding fowls aged between 51 and 62 weeks. Lesions were identified in the pelvic limbs of 92 birds, 85 of which showed total or partial rupture of ligaments or tendons. Five cases are presented of bilateral simultaneous rupture of the quadriceps tendon and the English literature of six isolated case reports is reviewed. All the patients were men, and most were over 50 years old. The mechanisms of tendon rupture in the hand are reviewed. Attrition ruptures of tendons in the carpal tunnel, other than those due to rheumatoid arthritis, are rare. A case is reported where Kienböck's avascular necrosis of the lunate caused rupture of flexor policis longus. This is the second case reported in the literature and serves as a model of attrition rupture.

The authors found in the material of the National Institute of Traumatology among the 650 cases of spontaneous tendon ruptures seven cases of rupture, when an identified disease localized on the tendon was the precipitating cause. Aetiology of the rupture has been as follows: tendon tuberculosis, rheumatic tendinitis xantoma tuberosum, haemangioma and pseudocyst of the tendon. In two other cases muscle biopsy revealed a concealed myopathy (central core disease and centronuclear myopathy) that may responsible for the rupture of the tendon.
