Longitudinal clinical and musculoskeletal ultrasound assessment after switching from oral to subcutaneous methotrexate in rheumatoid arthritis: a two-center real-world study.

OBJECTIVES: Subcutaneous methotrexate (SC MTX) provides more reliable bioavailability than oral MTX; however, longitudinal real-world data incorporating musculoskeletal ultrasound (MSUS) remain limited. We aimed to evaluate longitudinal changes in clinical disease activity and ultrasound-detected synovitis, particularly power Doppler (PD) activity, after switching from oral to SC MTX in patients with rheumatoid arthritis (RA). METHODS: This two-center retrospective cohort study included 57 patients with RA who switched from oral to SC MTX in routine clinical practice. Clinical disease activit

FDG Uptake Extending From the Dorsal Root Ganglion to the Sciatic Nerve After Herpes Zoster.

A woman in her 50s receiving paclitaxel, carboplatin, and pembrolizumab for recurrent cervical cancer developed herpes zoster and received antiviral therapy. 18F-FDG PET/CT performed 22 days after zoster onset, when the skin lesions had crusted, demonstrated continuous uptake extending from the left L5 dorsal root ganglion to the sciatic nerve. To our knowledge, this is the first reported case of FDG PET findings indicating ganglioneuritis extending from the dorsal root ganglion to a peripheral nerve after herpes zoster. Recognition of this pattern is important in patients receiving chemoimmun

Dual-tissue single-cell analysis reveals a shared interferon-stimulated gene program and a predominantly non-neuronal trigeminal response in experimental periodontitis.

Periodontitis destroys the tooth-supporting tissues, yet whether a local periodontal lesion is accompanied by changes in the distal trigeminal sensory system at cell-type resolution is poorly defined. In ligature-induced periodontitis (male mice), we profiled the gingiva by single-cell and the trigeminal ganglion (TG) by single-nucleus RNA sequencing, integrating gene-set enrichment, regulon (SCENIC), communication (CellChat) and ligand-target (NicheNet) analyses with micro-computed tomography and histology, and tested the principal findings in individual mice by RT-qPCR (n = 8 per group) and

Development and Characterization of a Rat Model of Post-traumatic Ankle Osteoarthritis.

OBJECTIVE: The molecular mechanisms contributing to posttraumatic ankle osteoarthritis (PTAOA) remain unclear. Here, we developed a rat model of PTAOA induced by joint instability and evaluated both pain and pathological changes. We paid special attention to the synovial tissue, which acts as a critical, pain-generating structure in OA. METHODS: We induced ankle instability by resection of the anterior talofibular ligament (ATFL) and calcaneofibular ligament (CFL) (RAC group). Safranin-O and hematoxylin & eosin staining were conducted to evaluate histological changes. Pain-related behavior wa

When Swallowing Triggers VT.

We describe a case of swallowing-induced ventricular tachycardia in a young, otherwise healthy male with a structurally normal heart which was refractory to multiple anti-arrhythmic medication and was successfully treated with ablation of a cardiac ganglion in the crux of the heart. We use this case to describe an unusual disease presentation and highlight current evidence related to evaluation and management of ventricular arrhythmias and add to the limited body of evidence related to swallowing induced tachycardia.

Incision-triggered neuroimmune remodeling of nociceptor ion channels: from postoperative inflammation to pain chronification.

Chronic postsurgical pain (CPSP) arises after a known tissue insult, yet no intervention reliably prevents its development. This Review examines whether incision-triggered inflammation remodels nociceptor voltage-gated ion channels in ways that sustain peripheral input and favor pain chronification. We integrate human genetic and clinical evidence, incision-specific perturbation studies, functional work in non-surgical models, and expression-level associations distinguishing mechanistic strength from directness to CPSP. We focus on NaV1.7, NaV1.8, NaV1.9, and CaV3.2 within a broader network th

A Mitochondria-Modulating Nanosystem for Rheumatoid Arthritis Immunotherapy via Inflammatory Cascade Disruption.

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by chronic inflammation within joints, where macrophage-driven inflammatory cascade caused by mitochondrial dysfunction significantly contributes to the pathogenesis. Through recruitment and activation of neutrophils from circulation, these macrophages promote the release of neutrophil extracellular traps (NETs) and generation of reactive oxygen species (ROS), leading to synovitis exacerbation and tissue damage. To disrupt this closely linked cycle, we present a mitochondria-modulating nanosystem (NM@APDAase) targeting th

Microvasculopathy in spinal muscular atrophy: The endothelium as a target of SMN deficiency and an amplifier of disease.

Spinal muscular atrophy (SMA) is caused by deficiency of the ubiquitous survival motor neuron (SMN) protein. Degeneration of anterior horn motor neurons defines the disease, but many organs are affected, including heart, liver, kidney, and bone. The vascular compartment is among them: digital necrosis in infants with the fewest SMN2 copies, capillary rarefaction in SMA mice, and decreased von Willebrand factor with fibrinogen extravasation in human post-mortem spinal cord. Whether these reflect endothelial injury by SMN deficiency itself or capillary adaptation to atrophic, denervated tissue h

Characterizing NaV1.7 antisense analgesia through antagonist binding, mRNA expression, and neuraxial distribution.

Genome-targeting strategies to address NaV1.7-mediated signaling in nociceptive afferents are of great clinical interest given its high selectivity and persistent analgesic outcomes. In the present work, we analyze concentration-dependent effects of suppressing primary afferent NaV1.7 using the intrathecal delivery of an antisense oligonucleotide (ASO) on pain behaviors and demonstrate that NaV1.7 ASOs are likely to modulate pain traffic by blocking afferent signaling at the dorsal root ganglion (DRG) level. Intrathecal NaV1.7 ASOs induced a concentration-dependent gradient of knockdown in DRG

Modulating the synovial ROS-NO-RNS network attenuates fibroblast-like synoviocyte pyroptosis and osteoarthritis progression.

Osteoarthritis (OA) is characterized by persistent synovial inflammation and disturbed redox homeostasis, yet most redox-directed interventions predominantly target reactive oxygen species (ROS) and provide limited control over the interconnected ROS-nitric oxide (NO)-reactive nitrogen species (RNS) network. Here, we developed Mn@GaMOF, a gallium-based metal-organic framework bearing highly dispersed mixed-valence Mn sites, to coordinately regulate the synovial nitro-oxidative microenvironment. Post-synthetic Mn incorporation markedly enhanced the superoxide dismutase (SOD)- and catalase (CAT)

Type I interferon activation in classical monocytes and GZMK+ CD8+ T cells characterises the ultrasound-negative phase of imminent rheumatoid arthritis.

OBJECTIVES: Rheumatoid arthritis (RA) is a prototypical autoimmune disease, characterised by prolonged systemic autoimmunity before organ-specific tissue inflammation. Understanding immune mechanisms that drive the transition from systemic autoimmunity to joint-specific pathology is key to arthritis prevention. In this study, we define immune signatures in anticitrullinated protein antibodies-positive (ACPA+) at-risk individuals who imminently progress to RA, comparing those with and without ultrasound (US)-detectable subclinical synovitis. METHODS: We performed single‑cell RNA sequencing and

Neuroimmune activation of the TRPV1-RAMP1 axis drives type 2 immunity to repair intestinal barrier and alleviate metabolic disease.

INTRODUCTION: Obesity is closely associated with intestinal barrier dysfunction and chronic inflammation. Group 2 innate lymphoid cells (ILC2s) can receive and integrate signals from neuropeptides, neurotransmitters, and epithelial cells, thereby regulating local tissue inflammatory responses and promoting repair. However, the specific mechanism by which the nociceptor Transient Receptor Potential Vanilloid 1 (TRPV1) interacts with ILC2s to regulate intestinal homeostasis in obesity remains unclear. OBJECTIVE: This study aimed to investigate whether electroacupuncture (EA) ameliorated obesity

Spinorphin Ameliorates Nitroglycerin-Induced Migraine-Like Responses and Trigeminovascular Neuroinflammation Potentially Through Opioid Receptor-Mediated Mechanisms.

Enhancement of endogenous opioid signaling through inhibition of enkephalin degradation has emerged as a potential strategy for pain control, yet its role in migraine remains unclear. This study investigated whether spinorphin, an endogenous inhibitor of multiple enkephalinases, attenuates trigeminovascular activation and migraine-like responses in experimental migraine. Migraine-like pain was induced in male Wistar rats by nitroglycerin. The effects of spinorphin were evaluated using periorbital and plantar von Frey tests, followed by assessment of c-Fos levels in the trigeminal nucleus cauda

Research Progress on Intraocular Pressure-lowering Drugs, Commonly Used Clinical Adjuvants, and Emerging Neuroprotective Agents for Glaucomatous Optic Neuropathy.

Glaucomatous optic neuropathy (GON) is the leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss. Current clinical treatment relies primarily on IOP-lowering drugs, but a subset of patients continues to experience progressive optic nerve damage. Adjuvant therapies such as antioxidants and calcium channel blockers have limited efficacy. Emerging neuroprotective strategies including NMDA receptor antagonists, neurotrophic factors, and gene therapy remain in preclinical or early-stage trials. This review systematically summarizes the mech

Potential sphenopalatine ganglion block efficacy as an alternative to epidural bloodpatch for post-dural puncture headache: a monocentric french study.

BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice. METHODS: We conducted a prospective monocentr
