Exome-wide association study reveals common and rare coding variants shaping chronic pain in 327,642 UK biobank participants.

BACKGROUND: Chronic pain is a major global health burden with a substantial but incompletely understood genetic basis. Although genome-wide association studies have identified multiple common-variant loci for pain-related traits, the contribution of rare coding variants to chronic pain susceptibility, biological mechanisms, and shared architecture across pain phenotypes remains unclear. METHODS: We performed an exome-wide association study across ten chronic pain phenotypes using whole-exome sequencing data from 327,642 European participants in the UK Biobank. Variant-level and gene-level ass

DNA-adsorbing nanozyme hydrogel reprograms the immuno-oxidative microenvironment to support stem cell-based cartilage repair in osteoarthritis.

Osteoarthritis (OA) is a prevalent degenerative joint disease, and stem cell therapy is often limited by a hostile joint microenvironment. Excessive reactive oxygen species (ROS) and damage-associated cell-free DNA (cfDNA) cooperatively drive innate immune activation, inflammatory macrophage polarization, and chondrogenic instability, forming an immuno-oxidative barrier to cartilage repair. Here, we developed a bioactive DNA-adsorbing nanozyme hydrogel (MNKB/HA) to remodel this niche and enhance BMSC-mediated cartilage repair. MNKB/HA integrates Mn3O4 nanozymes with Kartogenin (KGN)-engineered

Mitochondrial Metabolic Reprogramming in Macrophages: A Key Driver of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent synovitis and progressive cartilage and bone destruction. Synovial macrophages are central coordinators of inflammatory signaling, stromal activation, and osteoclastogenesis, yet their pathogenic functions cannot be explained by extracellular cytokine networks alone. Increasing evidence indicates that macrophage activation is coupled to cell-intrinsic mitochondrial metabolic reprogramming. Within the hypoxic, nutrient-variable, and inflammatory RA synovium, macrophages undergo coordinated changes in glycolysi

Etanercept-Induced Cutaneous Necrotizing Vasculitis in a Patient With Rheumatoid Arthritis: A Case Report and Literature Review.

Tumor necrosis factor-alpha (TNF-α) inhibitors are effective therapies for rheumatoid arthritis (RA) but may rarely induce paradoxical autoimmune complications, including vasculitis. We report the case of a 53-year-old woman with seropositive RA and secondary Sjögren's syndrome who developed bilateral dependent petechial and purpuric lesions of the lower extremities after approximately 33 months of etanercept therapy at a dose of 25 mg twice weekly. At symptom onset, her RA was well controlled, with no clinical or ultrasonographic synovitis and a Disease Activity Score in 28 joints of 2.1. Ski

Variation in Intrinsic Aerobic Capacity Reveals Distinct Differences in the Maintenance of Rat Knee Joint Integrity.

The objective was to test how intrinsic aerobic capacity affects the articular cartilage, infrapatellar fat pad (IFP), subchondral bone, and synovial membrane of the rat knee joint. Thirty-five 24-week-old male N:NIH rats selectively bred for high aerobic capacity (high-capacity-runner [HCR]; n = 17) and low aerobic capacity (low-capacity-runner [LCR]; n = 18) were fed either a high-fat/high-sucrose (HFHS) diet or a chow diet for 12 weeks. Alterations in articular cartilage, subchondral bone microarchitecture, IFP hypertrophy, and synovial membrane cytokine levels were quantified. Body composi

USMSIT/NMUSIT Fundamental Guide on Diagnostic and Interventional Knee Ultrasonography.

Knee pain is one of the most prevalent musculoskeletal complaints and a leading cause of disability, particularly in middle-aged and older adults. Owing to the knee's complex anatomy and its essential role in weight bearing and locomotion, an accurate diagnosis of knee disorders remains challenging. Although radiography and magnetic resonance imaging are indispensable for structural evaluation, musculoskeletal ultrasound (US) has gained recognition as an effective first-line imaging modality because of its real-time visualization, dynamic assessment capability, absence of ionizing radiation, a

Clinical case series: Emergence of psoriatic arthritis during biologic IL-23 pathway inhibition therapy for psoriasis.

Psoriasis (PsO) and psoriatic arthritis (PsA) are increasingly recognized as manifestations within the spectrum of psoriatic disease, and emerging observational data suggest that biologic therapy targeting interleukin-23 (IL-23) may reduce the risk of incident PsA. This finding remains controversial given the potential for channeling and information bias. Here, we describe five patients with long-standing PsO who developed new-onset PsA while achieving excellent skin clearance with IL-23 or IL-12/23 inhibitor therapy (risankizumab in four patients and ustekinumab in one). The time from the ini

Ocular Hypertension and Glaucoma in Graves' Orbitopathy: A Single-Center Retrospective Cross-Sectional Study.

Background/Objectives: Elevated intraocular pressure (IOP) in Graves' orbitopathy (GO) is a recognized risk factor for glaucomatous optic neuropathy, but the extent to which it translates into measurable structural and functional damage remains unclear. This study investigated structural and functional correlations in patients with GO stratified by IOP and glaucoma status. Methods: In this retrospective, cross-sectional study, 165 adult patients with GO were stratified into three groups of 55 each: normal IOP, ocular hypertension, and clinically diagnosed glaucoma. All patients underwent clini

Stellate ganglion block for ventricular arrhythmia disease states: Emerging treatment options.

Autonomic modulation has emerged as an effective therapeutic target in the management of ventricular arrhythmias and electrical storm (e.g., refractory ventricular arrhythmias), in a background of growing evidence of the link between the autonomic nervous system and arrhythmias. Stellate ganglion blockade (SGB) is a minimally invasive procedure targeting the stellate ganglion with an anesthetic agent that has emerged as a temporizing treatment option for management of refractory and hemodynamically unstable ventricular arrhythmias and electrical storm (ES). SGB which can be performed with fluo

Robot-Assisted Gasless Trans-Hairline Excision of Submandibular Gland Tumors.

Benign submandibular gland tumors are usually resected through a transcervical incision, which provides direct exposure but leaves visible cervical scarring. Remote-access endoscopic techniques improve scar concealment but can be limited by two-dimensional visualization, assistant-dependent camera control, and rigid instrument collision in the narrow submandibular gland space. This article presents a standardized video protocol for robot-assisted submandibular gland excision via a gasless trans-hairline approach for benign tumors. The procedure begins with postauricular trans-hairline incision

Physiological Postnatal Neurogenesis, Development, and Maturation in the Regenerable Retina.

The retina enlarges postnatally by ∼440% in axolotls and ∼150% in humans, predicting a reduction in neuron densities by 81.5% and 60%, respectively, if neurogenesis is absent. The concurrent retinal remodeling is poorly characterized. We studied retinas of 2-15 cm axolotls. Results showed that hatchling (∼2 cm) retinas, particularly the peripheral (Peri) had 1-2 more soma rows in the ganglion cell (GCL) layer and outer (ONL) and inner nuclear (INL) layers. Cell density reduced in GCL (GCs, -49%) in juveniles (4-12 cm) but increased in INL (74%) and ONL (55.5%) in young juveniles (4-6 cm), demo

Retinal layer thickness as a potential marker for future cardiovascular events - a UK Biobank cohort study using optical coherence tomography.

AIMS: Cardiovascular disease (CVD) is the leading cause of death worldwide. For CVD prevention, effective screening is needed to identify high-risk individuals. Optical coherence tomography (OCT) provides a noninvasive, low-cost "quick look" into the brain and has recently shown thinning of certain retinal layers in patients with previous CVD. We explored if OCT-derived retinal layer thicknesses could predict future major adverse cardiovascular events (MACE: stroke/myocardial infarction/cardiovascular death). METHODS: In this retrospective cohort study, data of 51,293 participants from the UK

Sacral Neural Crest-Derived Extracellular Vesicles Promote Pelvic Ganglion Repair Through Anti-Pyroptotic Effects Involving ANXA1-FPR2 Signaling.

Pelvic autonomic nerve injury is a major cause of postoperative bowel dysfunction, yet effective strategies for restoring damaged pelvic ganglia remain lacking. Here, we demonstrate that extracellular vesicles derived from human pluripotent stem cell-derived sacral neural crest cells (Sacral NC-EVs) promote pelvic neuroregeneration by suppressing oxidative stress-induced pyroptosis. In a rat model of major pelvic ganglion (MPG) injury, local administration of Sacral NC-EVs improved gastrointestinal motility and partially restored rectal neural innervation and epithelial barrier integrity. Peripheral nerve injury alters metabolic function and neuronal excitability through the Lin28/let-7 pathway.

Sensory neurons require efficient metabolic processes to reliably transmit information including touch, temperature, and noxious stimuli from peripheral to central nervous systems. Peripheral nerve injury is known to disrupt metabolic function resulting in aberrant sensory nervous system excitation, but molecular pathways linking nerve injury to persistent alterations in metabolism and neuronal activity have been incompletely understood. We find that a conserved metabolic regulator, RNA binding protein Lin28, is elevated in the somas of dorsal root ganglion (DRG) nociceptive neurons following

Early-life high-fat diet accelerates the development of vagal satiation signaling and predisposes females to overconsumption.

Overconsumption of foods rich in fats, sugars, and calories is a major contributing factor to increased risk for cardiometabolic disease. Ingestive experience with these foods can begin early in children, yet there is limited understanding of the impact of early life nutrition on the development of vagal afferent neurons necessary for coordinating appetitive and satiating behaviors. To address this gap, mice reared on a chow diet (control) were compared to those reared on a high-fat diet (HFDEARLY). We demonstrate that the vagally mediated satiation response to cholecystokinin (CCK) does not m

Synovial inflammation and immune-stromal crosstalk in osteoarthritis-related synovial fibroblast research, 2006-2025: a bibliometric analysis.

BACKGROUND: Osteoarthritis (OA) is increasingly understood as a whole-joint organ disease involving cartilage, synovium, subchondral bone, and the local immune-stromal microenvironment. Synovial fibroblasts and fibroblast-like synoviocytes (FLS) participate in synovitis, matrix remodeling, cartilage-synovium crosstalk, pain-associated microenvironments, and disease heterogeneity. However, the knowledge structure, main knowledge sources, and frontier evolution of OA-related synovial fibroblast research remain incompletely mapped. METHODS: We retrieved publications on OA-related synovial fibrob

Mitochondrial Genetic Background Shapes Baseline Retinal Function, Age-Related Bioenergetic Remodeling, and Microglial STING Phenotypes.

Mitochondria are central regulators of retinal bioenergetics and innate immune signaling, yet how mitochondrial genetic background and aging jointly influence immunometabolic responses in the retina remains poorly defined. Here, we investigated the effects of mitochondrial DNA (mtDNA) variation on retinal function and the combined effects of mtDNA variation and age on bioenergetic composition, mtDNA damage, and the microglial expression and intracellular localization of stimulator of interferon genes (STING) protein. Using mitochondrial-nuclear eXchange (MNX) mice harboring identical C57BL/6J
