Optical coherence tomography angiography of the macular retina and choroid in glaucoma: a narrative review.

Glaucoma is a progressive optic neuropathy characterized by retinal ganglion cell degeneration, structural remodeling of the retina and choroid, and irreversible visual field loss. Increasing evidence indicates that microvascular dysfunction plays a crucial role in its onset and progression. Optical coherence tomography angiography (OCTA), as a noninvasive, high-resolution imaging technique, enables quantitative visualization of retinal and choroidal microcirculation, providing new insights into the structural-vascular-functional relationships in glaucoma. This narrative review summarizes rece

[Special surgical anatomy and pathology of the 6th extensor compartment of the wrist and reconstruction of extensor carpi ulnas dislocations].

The article describes the special surgical anatomy of the extensor carpi ulnaris tendon (ECU) and the sixth extensor compartment, including the most common pathological conditions. The sixth extensor compartment is only loosely covered and not connected by the extensor retinaculum. The ECU has its own retinaculum at the ulnar head, for which the Latin name "retinaculum privatum" (English: subsheath) is proposed. Dislocation of the extensor tendon (ECU), first described by Schlesinger in 1907, can be treated using various techniques for reconstructing the retinaculum privatum (from the extensor

Chronic periodontitis induces cognitive decline in young adult mice.

PURPOSE: This study investigated whether chronic periodontitis induces cognitive impairment in young adult mice and examined the mechanisms underlying periodontitis-associated cognitive decline. METHODS: Chronic periodontitis was induced in 4-month-old female C57BL/6N mice by bilateral ligature placement around the maxillary molars. Learning and memory were assessed using the Morris water maze (MWM) and novel object recognition tests at 4 and 8 weeks after ligation. At 2 and 12 weeks post-ligation, tissues from the brain, including the hippocampus and prefrontal cortex, as well as the trigemi

Imaging in Rheumatic Diseases of the Head and Neck.

Multimodality imaging is essential for evaluating head and neck rheumatologic disease. Computed tomography (CT) can demonstrate osseous erosions, mineralization, and structural complications, including temporomandibular joint and cervical spine involvement in JIA/RA, crowned dens in pseudogout, and sinonasal or airway disease in giant cell arteritis (GCA) and relapsing polychondritis (RP). Ultrasound is useful for detecting synovitis in JIA/RA, auricular chondritis in RP, salivary gland abnormalities in Sjögren's syndrome, and vascular changes in GCA. FDG-PET/CT can identify metabolically acti

Anti-RANKL antibody treatment mitigates subchondral bone loss and cartilage degeneration without affecting synovial inflammation in a mouse model of rotator cuff tear arthropathy.

AIMS: Enhanced osteoclast activity in subchondral bone is implicated in osteoarthritis (OA) progression, but the contribution of osteoclasts versus synovial inflammation in cuff tear arthropathy (CTA) remains unclear. This study investigated whether inhibition of osteoclasts using an anti-receptor activator of nuclear factor κB ligand (RANKL) antibody can suppress joint degeneration in a mouse CTA model without affecting synovial inflammation. METHODS: A modified CTA (mCTA) model was established in mice by resecting the supraspinatus and infraspinatus tendons, the superior joint capsule, and

Endogenous Trigeminal Neurons Induce Schwann Differentiation of Donor Stem Cells in Dental Pulp Regeneration via Reciprocal Extracellular Vesicles Interplay.

The potential of stem cells transplantation to restore organ function in recipients is hindered by the poor integration of donor cells into host tissues. Here, we successfully regenerated tooth pulp in the root canal of minipigs by transplanting human deciduous tooth stem cells (SHED). The implanted SHED differentiated into several functional cells, particularly Schwann cells (SCs), which attracted and enwrapped regrown host axons in regenerated pulp. The reciprocal extracellular vesicles (EVs) communication between SHED and the host trigeminal ganglion (TG) regulated the regeneration of denta

4-Octyl itaconate inhibits synovial fibroblast activation by targeting Nrf2 to exert anti-inflammatory effects in rheumatoid arthritis.

OBJECTIVES: Fibroblast-like synoviocytes (FLS) are pivotal for the expansive growth and invasiveness of rheumatoid synovitis, and patients with a fibroblastic-rich pathotype respond poorly to currently approved antirheumatic drugs. The aim of this project was to examine the anti-inflammatory effects of the nuclear factor erythroid 2-related factor 2 (Nrf2) activator 4-octyl itaconate (4-OI) on inflammatory rheumatoid arthritis (RA)-FLS-dominated pathology. METHODS: Rheumatoid arthritis fibroblast-like synoviocytes (RA-FLS) and autologous cocultures of RA-FLS and peripheral blood mononuclear c

Elderly-onset rheumatoid arthritis versus polymyalgia rheumatica: A systematic review and meta-analysis on pathophysiology, diagnosis and clinical progression.

Elderly-onset rheumatoid arthritis (EORA) and polymyalgia rheumatica (PMR) are among the most frequent inflammatory rheumatic diseases in patients over 50, yet their clinical differentiation, particularly when EORA presents with polymyalgic onset, poses a common diagnostic challenge. This systematic review aimed to identify the pathophysiological, clinical, imaging and treatment-response features that distinguish EORA from PMR. MEDLINE, CENTRAL and Scopus (inception to January 2026), with supplementary searches of LILACS, Google Scholar and trial registries (June 2026) were searched, including

Official incidence and trends of occupational musculoskeletal disorders in Turkey in 2014-2024: implications of possible underreporting.

In Turkey, official incidence figures for work-related musculoskeletal disorders (MSDs) remain notably low despite established ergonomic risks. The aim of this longitudinal study was to determine national incidences of all recognised occupational MSD cases among insured workers included in the publicly available registry of the Social Security Institution of Turkey (SGK) between 2014 and 2024, to calculate annual rates per million insured workers, and to establish temporal, gender-specific, and diagnostic trends. A total of 744 cases were recorded, with recognised incidence increasing from 1.7

Visual field asymmetry indices and optical coherence tomography in the differential diagnosis of glaucomatous and pituitary adenoma-related optic neuropathies.

PURPOSE: Differentiating glaucomatous optic neuropathy from compressive optic neuropathy related to pituitary adenomas remains challenging, particularly in patients with mild visual field loss, as structural OCT findings and functional deficits may substantially overlap. METHODS: This cross-sectional study included 178 eyes of 178 participants with primary open-angle glaucoma, pituitary macroadenomas, pituitary microadenomas, and healthy controls. All subjects underwent standard automated perimetry using the 24 - 2 SITA Standard strategy and spectral-domain optical coherence tomography (OCT).

Isolated Medial Ankle Tenosynovitis of the Flexor Digitorum and Posterior Tibialis Tendons: A Case Report of the Role of Early Musculoskeletal Ultrasound in Identifying Anti-citrullinated Protein Antibody (ACPA)-Positive At-Risk Rheumatoid Arthritis.

Tenosynovitis is a recognized feature of rheumatoid arthritis (RA), yet it typically presents alongside symmetric polyarticular synovitis. Isolated focal tenosynovitis as the primary and initial manifestation of RA is uncommon and frequently leads to diagnostic delays. We present the case of a 62-year-old female who presented with localized right ankle pain in the absence of systemic joint involvement or elevated inflammatory markers. While the rheumatoid factor was negative and both the erythrocyte sedimentation rate and C-reactive protein levels were within normal limits, musculoskeletal ult

A pooled genetic approach provides multimodal access to uncharacterized retinal ganglion cell subtypes.

Retinal ganglion cells (RGCs) are the sole output neurons of the retina, responsible for transmitting visual information to the brain. Recent transcriptomic studies have revealed extensive molecular diversity among RGCs that parallels their known morphological and functional heterogeneity. Although large-scale efforts have unified the transcriptomic, morphological, and physiological features for a limited number of RGC subtypes, the majority of molecularly defined types remain poorly characterized. Developing approaches that can be used to identify RGC subtypes in a reliable and reproducible m

Development of the eye in grass carp (Ctenopharyngodon idella): Ocular histomorphology, corneal surface ultrastructure, and retinal calretinin and tyrosine hydroxylase immunoreactivity.

Eye posthatching development was examined in grass carp (Ctenopharyngodon idella) from 1 day posthatching (1 dph) to 365 dph. Eye histology and morphometry, retinal expression of calretinin (CR) and tyrosine hydroxylase (TH), and corneal ultrastructure were studied using light and scanning electron microscopy. Basic eye structure was detectable at 1 dph, though important features, including retina and iris pigmentation as well as photoreceptor differentiation, were delayed till 4 dph. Differentiated photoreceptors seemed to emerge at 4 dph with evident increases in their count and shape divers

Targeting Inflammation in Osteoarthritis Pain: A Phenotype-Driven Approach.

BACKGROUND: Osteoarthritis (OA) is a chronic joint disease marked by the progressive loss of articular cartilage and associated inflammatory processes, which can lead to significant pain and disability. However, the heterogeneous nature of OA has limited the effectiveness of many anti-inflammatory interventions, highlighting the need for phenotype-driven therapeutic strategies. METHODS: This narrative review was conducted by searching PubMed, Web of Science, and Google Scholar from inception to June 2025. Predefined keywords included "osteoarthritis", "inflammation", "pain", "synovitis", "eff

What have we learned about osteoarthritis from imaging?

Clinical and research imaging have been instrumental in shifting our understanding of osteoarthritis from a structural, cartilage-focused disorder to a whole-joint complex disease with biomechanical and inflammatory components. Initially, the only osteoarthritis imaging modality was x-ray; therefore, the focal point of diagnosis was the bone: osteophytes, sclerosis, but most importantly, the joint space narrowing. Soft-tissue visualization, dynamic imaging, and advanced image analysis techniques have shown that numerous other joint tissues and features are associated with osteoarthritis progre

Cervical Sympathetic Blockade for Psychogenic Nonepileptic Seizures Associated With Post-traumatic Stress Disorder: A Case Report and Neurobiological Framework.

Psychogenic nonepileptic seizures (PNES), also termed functional seizures, are episodes that resemble epileptic seizures but occur without the abnormal ictal electrical activity characteristic of epilepsy. PNES is frequently associated with trauma exposure, post-traumatic stress disorder (PTSD), and abnormalities in autonomic regulation. Emerging evidence suggests that altered interactions among limbic, autonomic, and motor networks may contribute to symptom generation. Cervical sympathetic blockade (CSB), including stellate ganglion block techniques, has been investigated as an intervention f
