Two-stage flexor tendon reconstruction continues to be an important option for delayed or missed Zone II flexor tendon injuries when direct repair is no longer feasible. Although the palmaris longus is commonly used as a donor tendon, it may be absent, unavailable, or unsuitable in some patients. In this retrospective comparative study, we evaluated the feasibility of split flexor carpi radialis tendon grafts as an alternative donor source and described their outcomes in relation to palmaris longus grafts. Thirty patients treated between December 2022 and February 2025 were included; 14 receiv

OBJECTIVES: Subcutaneous methotrexate (SC MTX) provides more reliable bioavailability than oral MTX; however, longitudinal real-world data incorporating musculoskeletal ultrasound (MSUS) remain limited. We aimed to evaluate longitudinal changes in clinical disease activity and ultrasound-detected synovitis, particularly power Doppler (PD) activity, after switching from oral to SC MTX in patients with rheumatoid arthritis (RA).
METHODS: This two-center retrospective cohort study included 57 patients with RA who switched from oral to SC MTX in routine clinical practice. Clinical disease activit

OBJECTIVE: The molecular mechanisms contributing to posttraumatic ankle osteoarthritis (PTAOA) remain unclear. Here, we developed a rat model of PTAOA induced by joint instability and evaluated both pain and pathological changes. We paid special attention to the synovial tissue, which acts as a critical, pain-generating structure in OA.
METHODS: We induced ankle instability by resection of the anterior talofibular ligament (ATFL) and calcaneofibular ligament (CFL) (RAC group). Safranin-O and hematoxylin & eosin staining were conducted to evaluate histological changes. Pain-related behavior wa

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by chronic inflammation within joints, where macrophage-driven inflammatory cascade caused by mitochondrial dysfunction significantly contributes to the pathogenesis. Through recruitment and activation of neutrophils from circulation, these macrophages promote the release of neutrophil extracellular traps (NETs) and generation of reactive oxygen species (ROS), leading to synovitis exacerbation and tissue damage. To disrupt this closely linked cycle, we present a mitochondria-modulating nanosystem (NM@APDAase) targeting th

Osteoarthritis (OA) is characterized by persistent synovial inflammation and disturbed redox homeostasis, yet most redox-directed interventions predominantly target reactive oxygen species (ROS) and provide limited control over the interconnected ROS-nitric oxide (NO)-reactive nitrogen species (RNS) network. Here, we developed Mn@GaMOF, a gallium-based metal-organic framework bearing highly dispersed mixed-valence Mn sites, to coordinately regulate the synovial nitro-oxidative microenvironment. Post-synthetic Mn incorporation markedly enhanced the superoxide dismutase (SOD)- and catalase (CAT)

OBJECTIVES: Rheumatoid arthritis (RA) is a prototypical autoimmune disease, characterised by prolonged systemic autoimmunity before organ-specific tissue inflammation. Understanding immune mechanisms that drive the transition from systemic autoimmunity to joint-specific pathology is key to arthritis prevention. In this study, we define immune signatures in anticitrullinated protein antibodies-positive (ACPA+) at-risk individuals who imminently progress to RA, comparing those with and without ultrasound (US)-detectable subclinical synovitis.
METHODS: We performed single‑cell RNA sequencing and

Synovial chondromatosis can lead to proliferation of the synovium followed by the appearance of multiple cartilaginous nodules, most commonly in large joints. Extra-articular involvement of the subacromial-subdeltoid bursa could resemble an inflammatory or infectious process, especially if the chondromatosis is present in the form of rice bodies. A 37-year-old right-hand-dominant male presented with left shoulder pain for three months, with restricted shoulder motion and pain during overhead activities, with no past medical history of trauma or surgery. He presented to the orthopedic outpatien

BACKGROUND: Bone scintigraphy is sensitive to increased osteoblastic activity but is not disease specific. Diffuse idiopathic skeletal hyperostosis (DISH) may therefore be mistaken for synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome when uptake involves the anterior chest wall and spine.
CASE PRESENTATION: A 50-year-old woman presented with a 1-year history of painless left sternoclavicular prominence. She reported that her daughter had SAPHO syndrome with anterior chest wall pain and palmoplantar pustulosis. Whole-body 99mTc-MDP scintigraphy and single-photon emissio

OBJECTIVE: Although synovial inflammation is recognized as a factor in the progression of osteoarthritis (OA), the specific role of fibroblast-like synoviocytes (FLS) in cartilage degradation has yet to be fully elucidated. This study sought to identify key regulators that connect inflammatory FLS to chondrocyte dysfunction.
METHODS: An integrated re-analysis of bulk and single-cell RNA sequencing data from OA synovial tissues was conducted to identify consistently upregulated genes. Functional assays, including migration and invasion tests, were performed on inflammatory FLS. FLS with silenc

Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disorder primarily affecting the synovial joints, characterized by persistent synovitis, progressive joint destruction, and systemic complications that contribute to significant morbidity. Despite advances in disease-modifying anti-rheumatic drugs and biologic therapies, many patients fail to achieve sustained disease control or experience treatment-related adverse effects, highlighting the need for safe adjunctive strategies. Keratin-positive giant cell-rich tumor (KPGCT) is a newly described bone and soft tissue tumor. The tumor is characterized by scattered keratin-positive cells and the presence of HMGA2::NCOR2 fusions. It is not known if the HMGA2::NCOR2 fusion is located in the keratin-positive cells, and little is known about how KPGCT develops. KPGCT shares some histologic features with tenosynovial giant cell tumor (TGCT), a soft tissue tumor with CSF1 rearrangements. Single-nuclei RNA sequencing and Xenium spatial transcriptomics were used to elucidate the most likely mechanisms driving KPGCT and compare K

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a rare autoinflammatory disorder characterized by combined cutaneous and osteoarticular involvement. Its etiology and pathogenesis remain incompletely understood, and an ideal animal model capable of fully recapitulating its phenotype is still lacking. Pstpip2-deficient mice exhibit sterile osteitis, inflammatory bone destruction, abnormal bone remodeling, and varying degrees of extraosseous inflammation, accompanied by myeloid immune abnormalities and IL-1β-associated inflammatory dysregulation. These mice have theref

Osteoarthritis (OA) is a prevalent degenerative joint disease, and stem cell therapy is often limited by a hostile joint microenvironment. Excessive reactive oxygen species (ROS) and damage-associated cell-free DNA (cfDNA) cooperatively drive innate immune activation, inflammatory macrophage polarization, and chondrogenic instability, forming an immuno-oxidative barrier to cartilage repair. Here, we developed a bioactive DNA-adsorbing nanozyme hydrogel (MNKB/HA) to remodel this niche and enhance BMSC-mediated cartilage repair. MNKB/HA integrates Mn3O4 nanozymes with Kartogenin (KGN)-engineered

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent synovitis and progressive cartilage and bone destruction. Synovial macrophages are central coordinators of inflammatory signaling, stromal activation, and osteoclastogenesis, yet their pathogenic functions cannot be explained by extracellular cytokine networks alone. Increasing evidence indicates that macrophage activation is coupled to cell-intrinsic mitochondrial metabolic reprogramming. Within the hypoxic, nutrient-variable, and inflammatory RA synovium, macrophages undergo coordinated changes in glycolysi

Tumor necrosis factor-alpha (TNF-α) inhibitors are effective therapies for rheumatoid arthritis (RA) but may rarely induce paradoxical autoimmune complications, including vasculitis. We report the case of a 53-year-old woman with seropositive RA and secondary Sjögren's syndrome who developed bilateral dependent petechial and purpuric lesions of the lower extremities after approximately 33 months of etanercept therapy at a dose of 25 mg twice weekly. At symptom onset, her RA was well controlled, with no clinical or ultrasonographic synovitis and a Disease Activity Score in 28 joints of 2.1. Ski

The objective was to test how intrinsic aerobic capacity affects the articular cartilage, infrapatellar fat pad (IFP), subchondral bone, and synovial membrane of the rat knee joint. Thirty-five 24-week-old male N:NIH rats selectively bred for high aerobic capacity (high-capacity-runner [HCR]; n = 17) and low aerobic capacity (low-capacity-runner [LCR]; n = 18) were fed either a high-fat/high-sucrose (HFHS) diet or a chow diet for 12 weeks. Alterations in articular cartilage, subchondral bone microarchitecture, IFP hypertrophy, and synovial membrane cytokine levels were quantified. Body composi

Knee pain is one of the most prevalent musculoskeletal complaints and a leading cause of disability, particularly in middle-aged and older adults. Owing to the knee's complex anatomy and its essential role in weight bearing and locomotion, an accurate diagnosis of knee disorders remains challenging. Although radiography and magnetic resonance imaging are indispensable for structural evaluation, musculoskeletal ultrasound (US) has gained recognition as an effective first-line imaging modality because of its real-time visualization, dynamic assessment capability, absence of ionizing radiation, a

Psoriasis (PsO) and psoriatic arthritis (PsA) are increasingly recognized as manifestations within the spectrum of psoriatic disease, and emerging observational data suggest that biologic therapy targeting interleukin-23 (IL-23) may reduce the risk of incident PsA. This finding remains controversial given the potential for channeling and information bias. Here, we describe five patients with long-standing PsO who developed new-onset PsA while achieving excellent skin clearance with IL-23 or IL-12/23 inhibitor therapy (risankizumab in four patients and ustekinumab in one).
