Evaluation of EVT401, a P2X7 Receptor Antagonist, in Healthy Chinese Participants: a first-in-human, double-blind, randomized, placebo-controlled, dose-escalation trial.

PURPOSE: As a critical hub between extracellular danger signals and intracellular inflammation, the P2X7 receptor centrally drives synovitis and joint destruction in rheumatoid arthritis, making it an attractive therapeutic target. This study reported the first-in-human data for EVT401, an orally administered P2X7 receptor antagonist. METHODS: A phase I trial was conducted in healthy Chinese participants aged 18 to 50 years, encompassing a single ascending dose study (40-1920 mg), a food-effect study (960 mg), and a multiple ascending dose study (480 mg-960 mg). This randomized, double-blind,

[IGFBP-4 improves synovitis by reducing M1 polarization of macrophages via the Wnt/β-catenin pathway].

Objective To investigate the mechanism by which insulin-like growth factor binding protein 4 (IGFBP-4) ameliorates synovial inflammation in osteoarthritis (OA) by inhibiting M1-type polarization of synovial macrophages via the Wnt/β-catenin signaling pathway. Methods The OA model was established in male C57BL/6 mice by intra-articular injection of sodium iodoacetate. Adenoviral vectors carrying IGFBP-4 overexpression or knockdown sequences were injected into the knee joint cavity for in vivo intervention. Micro-computed tomography (Micro-CT) was used to observe the subchondral bone changes in

Parvovirus B19 and inflammatory arthritis: from viral mimicry to infection-associated autoimmunity.

INTRODUCTION: Parvovirus B19 is a clinically important viral mimic of systemic inflammatory rheumatic diseases, frequently presenting as an abrupt-onset inflammatory polyarthritis in adults. While typically self-limiting, B19V infection can induce transient autoantibodies, cytopenias, and phenotypes resembling rheumatoid arthritis or systemic lupus erythematosus. In specific contexts, it may also lead to persistent or atypical musculoskeletal manifestations. AREAS COVERED: This narrative review summarizes the epidemiology, immunopathogenesis, clinical spectrum, diagnostic approach, and manage

Metabolic-Redox Remodeling of Inflammatory Synovium by Mannose-Modified Selenium Nanoparticles for Osteoarthritis Therapy.

Osteoarthritis (OA) progression is driven by persistent synovial inflammation, amplified by oxidative stress and enhanced glycolysis. Our previous study has demonstrated that glucose transporter 1 (GLUT1) plays a key role in initiating OA synovitis via regulation of glycolysis. Single-cell RNA sequencing and tissue-specific fluorescent reporter mice (Prg4GFPCreERt2 and Lyz2-iCre;tdTomato) reveal that GLUT1 marks a hyperinflammatory synovial state coupled to impaired selenoprotein-dependent redox homeostasis. Accordingly, mannose-modified selenium nanoparticles (M-SeNPs) are developed as an int

Multi-omics analysis unveils inflammatory and metabolic alterations between the synovium and cartilage in patients with knee osteoarthritis.

BACKGROUND: KOA (knee osteoarthritis) is a degenerative disease affecting the entire joint, involving synovial inflammation and cartilage degeneration, but their molecular interplay remains unclear. This study integrated proteomics and metabolomics to investigate cross-tissue alterations in KOA. METHODS: Paired synovial and cartilage tissues from 12 KOA patients and 12 controls were analyzed using DIA proteomics and untargeted metabolomics. The major molecular abnormalities in OA synovium and cartilage were evaluated through differential expression analysis, GO and KEGG functional enrichment

Transcriptomic Identification and Validation of NDUFS2 in Acute Rat TMJ Synovitis and Its Functional Role in LPS-Induced Synovial Fibroblast Inflammation.

PURPOSE: Temporomandibular joint (TMJ) synovitis persistently drives TMJ osteoarthritis (TMJOA) progression. Aberrant reactive oxygen species (ROS) accumulation triggers chronic oxidative stress, amplifying synovial inflammation. This study aimed to identify candidate ROS-related genes through transcriptomic screening and explore their potential functional involvement in TMJ synovitis. METHODS: Acute TMJ synovitis was induced in male Sprague-Dawley rats via intra-articular injection of Complete Freund's Adjuvant (CFA). RNA sequencing of inflamed and normal synovial tissues was performed to sc

Early arthritis signs and symptoms revisited-is synovitis what we are assessing?

Classic signs of joint inflammation at physical examination of patients with rheumatoid arthritis (RA) are joint swelling and joint tenderness. In the hand, additional characteristic signs include a positive squeeze test, reduced grip strength, and incomplete fist closure. Typical symptoms that are presumed to be inflammatory are pain and morning stiffness. Traditionally, these signs and symptoms have been thought to reflect underlying synovitis, ie, inflammation of the intra-articular synovial lining. Over the past 15 years, however, we have recognised the involvement of other inflamed tissue

The osteoarthritis inflammatory ecosystem: a node-edge-boundary-emergent framework toward precision stratification and therapeutic hypothesis generation.

Osteoarthritis (OA) is a heterogeneous whole-joint disorder in which pain, structural progression and treatment response often diverge from radiographic severity. This mismatch has limited disease-modifying OA drug development and highlights the need for biologically informed patient stratification. Chronic low-grade inflammation contributes to OA progression, but it is embedded within mechanical loading, metabolic dysfunction, synovitis, osteochondral remodeling, cellular senescence and pain sensitization rather than acting as a single-cytokine process. Here, we propose a Node-Edge-Boundary-E

Time-dependent synergistic pathogenicity of sequential avian reovirus and Mycoplasma synoviae co-infection in SPF chickens.

Avian reovirus (ARV) and Mycoplasma synoviae (MS) are major pathogens associated with tarsal synovitis and growth suppression in broilers. Yet the effect of infection order on their combined virulence remains poorly understood. A survey of 202 swollen joint samples from northern Chinese farms (2024-2025) revealed 35.1% ARV-MS co-detection, identifying this dual infection as a frequent co-infection in broiler arthritis. To assess timing effects, we inoculated 240 SPF chickens with ARV, MS, or both simultaneously or sequentially at 12- or 24 h intervals. All co-infection protocols were associate

Uveitis despite adalimumab therapy in Blau syndrome.

Blau syndrome is a rare granulomatous autoinflammatory disease caused by NOD2 mutations and characterised by dermatitis, arthritis and uveitis. We report a middle childhood boy initially diagnosed with oligoarticular juvenile idiopathic arthritis (JIA) because arthritis was the predominant early manifestation. He had papular skin lesions in infancy and periarticular swelling and tenosynovial cysts, later developed persistent arthritis and recurrent fever. Genetic testing identified a heterozygous NOD2 variant (c.1001G>A, p.Arg334Gln [R334Q]), confirming Blau syndrome. Despite ongoing methotrex

Precision medicine in rheumatoid arthritis: advances and clinical applications of multi-omics biomarkers.

Rheumatoid arthritis (RA) is a highly heterogeneous autoimmune disease characterized by the presence of synovitis, joint destruction, and systemic inflammation. Despite the substantial improvement in patient outcomes brought about by the advent of targeted therapies, the traditional "one-size-fits-all" treatment model still faces numerous challenges. These include individual variability in treatment response, a "trial-and-error" process in drug selection, and the emergence of refractory RA. Precision medicine is the development of optimal prevention and treatment strategies based on patients'

LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli

Functional and biochemical inflammatory responses to a 75 ng intra-articular recombinant equine interleukin-1β (reIL-1β) synovitis model.

Recently, a synovitis model using 75 ng intra-articular recombinant equine interleukin-1 (reIL 1β), showed promise for inducing a mild, self-limiting inflammatory response without overtly altering upper-body movement asymmetry in n = 3 horses. This study aimed to evaluate the effect of this 75 ng reIL-1β model on gait biomechanics and synovial fluid (SF) inflammatory, pro-resolution, and cartilage turnover biomarkers in a larger equine sample. SF samples were collected from the left intercarpal joint of n = 11 horses at baseline and at 6- and 12-hours following 75 ng reIL-1β injection and were

Advances in traditional Chinese medicine for modulating macrophage polarization in the treatment of rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disorder characterized by persistent synovitis and progressive joint destruction, wherein macrophage polarization constitutes a critical pathogenic mechanism governing both the initiation and perpetuation of synovial inflammation. The conventional M1 and M2 dichotomy has evolved into a continuum model encompassing disease-stage-specific synovial subsets, including remission-associated MerTK-positive populations and pro-inflammatory clusters, reflecting the functional heterogeneity of macrophages in the rheumatoid microenvironment. Modulating ma
