BACKGROUND: Residual synovial inflammation may persist in patients with rheumatoid arthritis (RA) despite clinical remission or low disease activity. Conventional inflammatory markers such as C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) may not adequately reflect subclinical synovitis detected by musculoskeletal ultrasound. Ferritin, an acute-phase reactant involved in inflammatory signaling, may represent a complementary biomarker of ongoing synovial inflammation.
OBJECTIVE: To evaluate the association between serum ferritin levels and Power Doppler (PD)-detected synovit

BACKGROUND: Psoriatic arthritis (PsA) develops in up to approximately 30% of patients with psoriasis (PsO) and can cause irreversible joint damage. For dermatologists, identifying factors independently associated with PsA may help identify patients who may benefit from earlier musculoskeletal monitoring and specialist referral.
OBJECTIVE: To identify demographic, clinical, and laboratory factors independently associated with clinical and subclinical PsA in patients with psoriasis.
METHODS: This retrospective study at Shandong Provincial Hospital included 75 clinical PsA, 22 subclinical PsA,

INTRODUCTION: Rheumatoid arthritis (RA) is a chronic, autoimmune, destructive inflammatory synovitis that ultimately affects metabolic function, the vascular system, and systemic bone. Many researchers have demonstrated that free radicals induce oxidative stress and are involved in several disease conditions. It was observed that a physiological equilibrium between oxidants and antioxidants is being disrupted by oxidative stress. Free radicals are molecules, ions, or atoms with unpaired electrons. These elements tend to be stabilized. For stabilization, they attack other molecules to get elect

OBJECTIVES: To undertake an early economic evaluation of anti-cyclic citrullinated peptide (anti-CCP) antibody testing scenarios in primary care, to improve identification of individuals at risk of rheumatoid arthritis (RA).
METHODS: A discrete event simulation (DES) model was developed to track a hypothetical cohort of 10 000 patients from primary care presentation with musculoskeletal (MSK) symptoms without synovitis through to secondary care rheumatology services. Multiple anti-CCP testing scenarios were evaluated, including testing all patients on first presentation and targeted testing u

Autoimmune diseases have traditionally been conceptualized as systemic disorders that secondarily involve the lung. This roadmap review proposes a complementary framework in which the lung is considered an autoimmune organ: a mucosal site where tolerance may be breached, a vulnerable target of systemic autoimmunity, and a sentinel from which future disease trajectories can be inferred. Across connective tissue diseases, the lung is also a major determinant of prognosis through airway disease, interstitial lung disease, progressive fibrosis, pulmonary vascular involvement and acute alveolar inj

BACKGROUND: Hand and upper extremity complaints frequently prompt evaluation for systemic inflammatory conditions, leading surgeons to refer patients to rheumatology. However, referral patterns are highly variable and not standardized, and access to rheumatologic care is often limited by prolonged wait times and workforce constraints. Existing literature on rheumatology referrals has largely focused on access and disease-specific pathways, with little attention to referral patterns within surgical subspecialties. Concordance between referral indication and confirmed rheumatologic diagnosis is

BACKGROUND: The Kocher criteria are widely used to differentiate septic arthritis (SA) from transient synovitis (TS) in children. However, diagnostic performance has since been proven to be inconsistent. Furthermore, the criteria were originally developed to distinguish SA from TS and have not been validated for differentiating osteomyelitis (OM), which frequently presents with overlapping clinical features. The purpose of this prospective study was to evaluate the diagnostic performance of the Kocher criteria and other clinical variables in differentiating SA from both TS and OM.

BACKGROUND: Effusion is a frequent finding after anterior cruciate ligament (ACL) injury and reconstruction, but the reliability of its quantification on routine [non-contrast] magnetic resonance imaging (MRI) is not established. This study sought to evaluate the intra-rater and inter-rater reliability of volumetric quantitative measurements (VQM) of knee effusion volumes (EV) using axial proton density-weighted, fat-suppressed (PD-FS) sequences after ACL injury and reconstruction.
METHODS: Twenty deidentified MRIs (10 ACL-injured knees and 10 uninjured knees) were randomly selected from a pr

TRIM28, a member of the tripartite motif (TRIM) family, functions as a transcriptional coregulator involved in maintaining genome stability during mitosis. In this study, we explored the role of TRIM28 in mouse oocyte meiotic maturation, where transcriptional activity is barely detectable. We found that TRIM28 was constitutively expressed during the early stages of oocyte meiotic maturation, with predominant nuclear localization in germinal vesicle (GV)-stage oocytes. TRIM28 depletion caused defective germinal vesicle breakdown (GVBD), but oocytes that successfully underwent GVBD displayed uni

INTRODUCTION: A key element of computational drug discovery is the precise prediction of drug-gene interactions, particularly when working with intricate biological systems where relational dependencies are essential. Because biological networks are graph-structured, traditional machine learning techniques frequently fall short. Graph Neural Networks (GNNs) have emerged as a viable approach for learning meaningful representations from this data type in response to this challenge. In this study, three state-of-the-art GNN architectures Graph Convolutional Networks (GCN), Graph Attention Network

Our study is a retrospective multicenter observational cohort study to investigate effect of the use of antenatal corticosteroids (ACS) in preterm infants on the severity of necrotizing enterocolitis (NEC) and its associated complications. We collected clinical data from 443 preterm infants with gestational age (GA) of less than 37 weeks who were diagnosed with NEC in four hospitals across various provinces in China, covering the period from June 2020 to June 2024. According to whether they received a full course of prenatal corticosteroid treatment in the week before delivery, infants were di

BACKGROUND: Rheumatoid Arthritis (RA) is a chronic autoimmune inflammatory disease that affects synovial membranes, leading to relentless progressive joint damage. This pathological process is regulated by transcription factors, such as NF-κB, STAT3, TGF-β, WNT, p38 MAPK, mTOR, AP-1, TLR-4, SOCS-4, YY-1, IRF, and FGF-20, which enhance the production of matrix-degrading enzymes and proinflammatory cytokines. Dysregulation of these transcription factors amplifies inflammation and accelerates joint damage, making them potential therapeutic targets.
OBJECTIVES: The purpose of this review was to s

Reactive arthritis (ReA) is characterized by immune-mediated sterile synovitis brought on by an infection that enters the body through the gastrointestinal or urogenital tracts from a distance. The diseases known as seronegative spondyloarthropathy (SSA) include undifferentiated arthritis (uSpA) and reactive arthritis (ReA). Cytokines are crucial in orchestrating an effective immune response to eliminate bacterial infections, such as those seen in ReA (Reactive Arthritis) conditions. The balance between Th1 and Th2 cytokines is particularly important in determining the outcome of infections as

BACKGROUND: Intracranially extending temporomandibular joint (TMJ) lesions may be radiologically misinterpreted as primary intracranial or skull base pathologies, leading to diagnostic delays or inappropriate management.
PURPOSE: This systematic review aimed to characterize the clinical and imaging features of such TMJ lesions and evaluate the impact of radiologic misclassification. We also aimed to develop a diagnostic framework for when to consider an intracranially extending TMJ lesion, based on clinical and radiologic features.

OBJECTIVES: TM joint OA (TMJOA) is a multifactorial degenerative disorder involving biomechanical and inflammatory processes. Yet, how mechanical stress causes synovial inflammation and cartilage degradation is unclear. This study aimed to explore the relationship between mechanical stress and inflammatory responses in TMJOA.
METHODS: SF cytokine profiles from TMJOA patients and controls were analysed using high-throughput antibody microarrays and validated by ELISA. Finite element analysis (FEA) was employed to map mechanical stress distributions in the mandibular condyle post-surgery. A uni

Obesity and metabolic syndrome play a significant role in the complexity of chronic inflammatory arthritis. By promoting systemic inflammation and altering immune responses, these conditions can amplify joint-related symptoms, such as pain, synovitis, and enthesitis. This inflammatory and mechanical burden complicates clinical evaluation, as traditional disease activity scores may be skewed by excess weight, leading to inaccurate assessments. Imaging techniques like musculoskeletal ultrasound (MSUS) offer a promising tool to detect subclinical inflammation and improve diagnostic accuracy. This

Radiotherapy is increasingly used for benign diseases, especially in Germany, where over 50% of radiation treatments address nonmalignant conditions such as osteoarthritis, plantar fasciitis, and Dupuytren's disease. Low-dose regimens have shown good efficacy in pain reduction and disease stabilization, with minimal toxicity. For rarer indications like Gorham-Stout disease, pigmented villonodular synovitis, and lymphatic fistulas, evidence is limited to case series and retrospective studies. Randomized trials remain scarce, and further research is needed to evaluate long-term outcomes and pati

OBJECTIVES: To assess whether 3D photoacoustic tomography (PAT) using the Fabry-Perot (FP) scanner can (1) reliably detect synovitis in participants with rheumatoid arthritis (RA), and (2) assess the severity of active inflammation.
METHODS: A total of 247 3D-PAT images of the finger and wrist joint from 11 healthy volunteers and 9 patients with RA were obtained using the FP scanner. Patients underwent power doppler ultrasound (PDUS) assessment of RA disease activity. The 3D-PAT images were acquired over a 15 × 15 × 10 mm3 volume in <15 s. The images were assessed quantitatively by segmentati
