LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli

Reassessing the Kocher Criteria: A Prospective Evaluation of Diagnostic Performance in Pediatric Musculoskeletal Infection.

BACKGROUND: The Kocher criteria are widely used to differentiate septic arthritis (SA) from transient synovitis (TS) in children. However, diagnostic performance has since been proven to be inconsistent. Furthermore, the criteria were originally developed to distinguish SA from TS and have not been validated for differentiating osteomyelitis (OM), which frequently presents with overlapping clinical features. The purpose of this prospective study was to evaluate the diagnostic performance of the Kocher criteria and other clinical variables in differentiating SA from both TS and OM. METHODS: A

Ultrasonographic features and recurrence risk of transient synovitis of the hip in children: a retrospective study.

BACKGROUND: Recurrence of transient synovitis of the hip (TSH) is often distressing but difficult to predict. Ultrasonography of the hip is essential for evaluating and monitoring hip effusion in patients with TSH. This study aimed to identify ultrasonographic features of the hip in children with recurrent TSH and identify factors associated with recurrence. METHODS: Medical records and hip ultrasound data, including age, weight, anterior capsule distance (ACD) in both hips, and ultrasound signs from children diagnosed with TSH at a single center between September 2015 and August 2023, were r

[Imaging in hip disorders in childhood and adolescence].

BACKGROUND: Hip disorders in childhood and adolescence require precise imaging to prevent maldevelopment, deformities, and early osteoarthritis. ULTRASOUND: Ultrasound is the primary first-line modality: radiation-free, dynamic, and indispensable in infancy. It provides detailed assessment of cartilaginous and soft-tissue structures and is essential in developmental dysplasia of the hip (DDH) to evaluate maturation and femoral head centration. It also enables rapid detection of joint effusions in transient synovitis or septic arthritis, as well as inflammatory changes in juvenile idiopathic a

Prevalence of Hip Joint Effusion in Children Diagnosed With Transient Synovitis in the Pediatric Emergency Department.

BACKGROUND: Transient synovitis (TS) is a benign condition characterized by acute hip pain and is a common cause of limping in children. Ultrasonography (US) examination frequently reveals hip joint effusion. However, accurate documentation of its prevalence among children with a diagnosis of TS is scarcely documented in the literature. OBJECTIVES: The main objective of our study was to evaluate the prevalence of hip joint effusion among children diagnosed with TS in the pediatric emergency department (PED). METHODS: A Retrospective chart review of children diagnosed with TS in the PED betwe

The return to physical activity for children and adolescents with hip pathologies: a scoping review protocol.

INTRODUCTION: Most orthopedic surgeons believe patients should resume physical activity (PA) after recovering from hip pathologies; however, guidance regarding PA type, intensity, duration, or long-term restrictions is lacking. Most children do not achieve the recommended 60 min of moderate-to-vigorous PA (MVPA). Children recovering from hip pathologies may be at heightened risk of insufficient PA due to prolonged restrictions. OBJECTIVE: This scoping review will map evidence on PA, sedentary behavior, and movement-related outcomes in children and adolescents with hip pathologies. INCLUSION

IgA Vasculitis Presenting As Isolated Monoarthritis Mimicking Transient Synovitis: A Diagnostic Challenge With Delayed Purpura.

IgA vasculitis (IgAV), previously known as Henoch-Schönlein purpura, is a small-vessel vasculitis that may involve the skin, joints, gastrointestinal tract, and kidneys. While purpura is a key clinical feature, presentations without rash at onset can occur and may lead to diagnostic uncertainty. We report the case of a two-year-old boy who presented with a three-day history of acute right knee pain, swelling, and reduced weight-bearing ability following a recent febrile respiratory illness. Ultrasound of the knee demonstrated mild periarticular fluid, and a provisional diagnosis of transient s

Low Kocher-Caird Scores High Clinical Suspicion: A Diagnostic Dilemma in Pediatric Hip Septic arthritis.

INTRODUCTION: The Kocher and Kocher-Caird criteria support differentiation of septic arthritis from transient synovitis in children with an irritable hip, but their performance varies across populations and may be altered by treatment before referral. This study aimed to describe the clinical, laboratory, imaging, operative, microbiological, treatment, and follow-up characteristics of partially treated children with operatively confirmed purulent septic arthritis of the hip despite normal admission inflammatory markers, and to determine their Kocher-Caird score distribution. Missed Volar Intercalated Segment Instability with Distal Radius Fracture: Challenges in Diagnosis, Surgical Management, and Clinical Outcome - Case Report.

INTRODUCTION: Transient synovitis (TS) in the setting of a COVID-19 infection has been reported only a few times in the literature. We report a case of TS 5 weeks following a positive COVID-19 test, in which three modified Kocher criteria were met. We aim to summarize and compare the clinical presentations of prior reported cases, and to expand the current understanding of extrapulmonary manifestations of COVID-19. CASE REPORT: A 4-year-old female presented with right hip pain and refusal to bear weight on the extremity without a history of trauma. Five weeks prior, the patient tested positiv

Comparison of Locations and Frequencies of Referred Pain in Pediatric Hip Disorders: A Retrospective Study.

INTRODUCTION: Children with hip joint disorders such as slipped capital femoral epiphysis (SCFE), Legg-Calvé-Perthes disease (LCPD), and transient synovitis of the hip (TSH) often experience pain in the hip, thigh, and knee. However, few studies have compared the specific locations and frequencies of pain associated with these disorders. Understanding these patterns can aid in diagnosis and management. This study aimed to fill this gap by comparing pain localization and frequency in pediatric hip disorders. METHODS: We retrospectively analyzed the medical records of 77 patients diagnosed with

Inflammatory Knee Reaction Mimicking Septic Arthritis Following Closely Spaced Pneumococcal Vaccines in a Pediatric Patient Undergoing Immunodeficiency Evaluation.

Vaccines in the pediatric population are crucial in preventing infectious diseases, but rare inflammatory responses can arise, particularly in patients with suspected immunodeficiency. This case highlights a unique inflammatory reaction mimicking septic arthritis following closely spaced pneumococcal vaccines (PPSV23) in a child undergoing immunodeficiency work-up. A previously healthy five-year-old Caucasian female with a history of recurrent bacterial infections presented with a two-day history of left knee swelling and pain after receiving two pneumococcal vaccines within a five-week interv
