[Coxitis fugax. The sonographic and radiologic picture in 65 cases].

Sonography is a valuable technique for the detection of hip joint effusion in children with transient synovitis. In a retrospective study of 65 patients distension of the anterior recess was found to be increased by 2 mm or more in all patients investigated. A sonographic follow-up examination was carried out in 30 patients. These patients showed complete regression of hip effusion after 4 weeks. The importance of attention to sonographic changes of the adjacent bony outline and femoral head deformity in connection with other hip diseases is emphasized. In 45 patients in this study (62% of the

Scintimetry in transient synovitis of the hip in the child.

Fifty-five consecutive children presenting with transient synovitis of the hip were examined with 99mTc-MDP scintigraphy and pin-hole collimator technique. Quantitative assessment was performed along a profile of interest across the hip joint. The criteria for the normal scintimetric pattern in the child's hip were established and the pathologic pattern of uptake in the acute phase, as well as in the follow-up after synovitis, was described. A decrease in isotope uptake in the proximal femoral epiphysis was observed in 13 children. This was correlated with a reduced uptake in the growth-plate,

Ultrasonography of hip joint effusions.

In order to evaluate ultrasonography in patients with suspected hip joint effusions, 123 consecutive patients were examined prospectively. Twenty healthy subjects were used as a control group. The normal sonoanatomy, the recommended scanning approach, and the diagnostic features of intra-articular joint effusions are presented. Even minor fluid collections of 1 or 2 ml could be accurately detected. Transient synovitis and fresh hemorrhagic effusions proved to be echofree, whereas clotted hemorrhagic collections or septic arthritis showed non-echofree effusions. Based on intraoperative and comp

Infections and related risk factors of arthritis in children. A case-control study.

We used a parent-completed questionnaire to record the clinical signs of infection preceding the onset of joint symptoms by 1 month or less in 334 children with arthritis or other joint disease. An upper respiratory tract infection (URTI) often preceded the onset of arthritis (51%) but also arthralgia (56%) and even trauma or orthopedic disease (43%). The results of a case-control analysis of 165 patients and community controls matched for age and sex were in favour of a preceding URTI having an etiologic or triggering role in acute transient arthritis of other joints than the hip only (odds r

The influence of joint posture on intra-articular pressure. A study of transient synovitis and Perthes' disease.

The pathogenesis of Perthes' disease has been related to increased intra-articular pressure secondary to a joint effusion. The pressure within the hip in different positions was measured in eight children with transient synovitis and four with the synovial stage of Perthes' disease. In the position of comfort for the hip this pressure was always less than the arteriolar blood pressure and in a supine position it did not exceed the systolic blood pressure. However, in extension with medial rotation the intra-articular pressure always became several times the systolic blood pressure. This high p

Conventional radiography in transient synovitis of the hip in children.

Conventional radiography was performed at diagnosis and at follow-up 5 to 9 months later in 70 children with transient synovitis of the hip. Twenty-four of the patients also had CT examination at diagnosis. The cartilaginous and osseous reaction in the conventional antero-posterior radiographs was studied as was the relation between the radiographic and CT findings concerning fat planes, joint effusion, and position of the hip joint. At diagnosis there was significant increase in the medial joint space and at follow-up examination there was significant increase in medial and cranial joint spac

Transient synovitis of the hip in children: role of US.

Transient synovitis of the hip remains a common diagnostic problem for the clinician. The physical signs are not pathognomonic of the condition, and the classic technical examinations are of little help. Therefore, the authors retrospectively studied the value of hip arthrosonography in 46 children with clinical symptoms suggesting pathologic hip conditions. In 20 of the 21 patients with a final diagnosis of transient synovitis, articular effusion was detected on ultrasound (US). Conventional radiography showed an increased medial joint space in only eight of these patients. Increased echogeni

Transient synovitis of the hip. Its incidence, epidemiology and relation to Perthes' disease.

In a prospective five-year study, 294 episodes of acute transient synovitis of the hip were diagnosed in 275 children. The average annual incidence was 0.2% and the accumulated risk of suffering at least one episode was 3%. The risk of recurrence was 20 times greater than the risk of having a single episode. Perthes' disease was diagnosed from one to five months after the acute attack of synovitis in 10 cases (3.4%). Review of the initial radiographs revealed signs of avascular necrosis in three of the 10 cases, and an increased joint space in five. Only two cases had had completely normal rad

The validity of radiographic assessment of childhood transient synovitis of the hip.

The diagnostic significance of some radiographic signs (abnormal hip 'joint space' and periarticular fat layers) as indicators of hip joint effusion or hip complaints without effusion was studied with ultrasonography and radiography in 47 children (58 examinations), of whom 40 had acute unilateral transient synovitis. It was found that 'joint depth' was not influenced by presence of intra-articular fluid collections; blurring and/or displacement of the periarticular fat pads medial and lateral to the hip joint occurred more frequently when joint effusion was present than in symptom-free hips o

[Value of ultrasonics in painful hip in children].

The authors report their experience concerning echography in 40 children. They emphasize its importance in the painful hip. All patients suffered with a painful limping with normal standard front and profile X-Rays. Echography appears to be a test both simple and reliable to discern synovial extravasation (no false positive). Yet, it is not able to predict the kind of effusion. More especially, echography is not limited to the very diagnosis of extravasation: in acute transient synovitis, it shows an enlargement of the joint cavity, with no effusion, giving evidence of the inflammatory synovia

The characterization of "transient synovitis of the hip" in children.

A 30-year retrospective review of 497 cases of transient synovitis of the hip involving 475 children allowed full characterization of the syndrome. Femoral head measurements of 118 patients at follow-up of 6 months showed no significant dimensional changes. Three cases of Legg-Calvé-Perthes disease (2.5%) were found in follow-up, as well as a case of osteoid osteoma of the femoral neck and two cases of juvenile rheumatoid arthritis. Recurrent synovitis with a benign prognosis occurred in 19 patients. In spite of a generally anticipated good prognosis in the vast majority of cases, radiographic

Computed tomography and ultrasonography for diagnosis of hip joint effusion in children.

Nineteen consecutive children with transient synovitis of the hip were examined for intracapsular joint effusion using computed tomography (CT) and ultrasonography (US). The two techniques were highly correlated in measurements of the anterior displacement of the joint capsule, indicating both to be reliable diagnostic tools. Such an effusion is clinically important because of the increase in intracapsular pressure with ensuing disturbance in the vascular supply to the proximal femoral epiphysis, as demonstrated experimentally and clinically in transient synovitis.

The nuclide bone scan in the diagnosis of Perthes' disease.

The prognosis in the management of Perthes' disease depends entirely on appropriate treatment following an early and accurate diagnosis of the condition and on the state of revascularization at the time of diagnosis. Early diagnosis is vital for a congruous femoral head to be obtained at the conclusion of treatment. The nuclide bone scan provides, at present, the most sophisticated investigation to enable such an early diagnosis to be made. A scan should be performed in all patients with a suggestive presentation in routine roentgenograms and in cases of transient synovitis with unusual featur

Incidence of arthritis in urban Finnish children. A prospective study.

The incidence of various types of arthritis in children was estimated by a prospective 1-year study in the greater Helsinki area (population under 16 years of age: 148,362). Patients were sought from primary care physicians, and 71% of the patients studied were seen within 1 week of the onset of symptoms. All patients received followup care for at least 3 months; patients whose symptoms were prolonged received followup care for a minimum of 2 years. The incidence per 100,000 children under 16 years of age was as follows: 108.5 for all cases of arthritis, 6.7 for septic arthritis, 5.4 for enter

Transient synovitis and Perthes' disease. Is there an aetiological connection?

A prospective study was made of 119 children with transient synovitis or any other cause for synovial effusion and elevated intra-articular pressure. During a follow-up of one year not one case of Perthes' disease was diagnosed and the late clinical and radiographic changes were minimal with moderate overgrowth of the femoral head in 33% and widening of the joint space in 14.2%. Our results do not support the widely accepted concept that Perthes' disease develops as a result of the period of elevated intra-articular pressure found in transient synovitis. Further research into this and Perthes'

Isotope bone scans in the assessment of children with hip pain or limp.

Bone scans from 43 children referred with hip pain of uncertain cause were reviewed. The bone scan was abnormal in 36 patients: normal in 7. In 12 the findings were diagnostic: osteomyelitis, osteoid osteoma, osteomyelitis with septic arthritis, Perthes' Disease and juvenile chronic arthritis. Twenty-four patients had abnormal scans including diffuse peri-articular increase and of these 18 had transient synovitis. Immobilisation and trauma accounted for the remainder. Isotope bone scans have been found to be a valuable investigation in children presenting with hip pain or limp, where the x-ray

[Quantitative assessment of bone scintigraphy in hip joint disease].

Quantitative assessment of bone scintigraphy was performed in fifty-six patients with hip joint disease including femoral neck fracture, aseptic necrosis of the femoral head, traumatic dislocation of the hip, Perthes disease, and transient synovitis of the hip. In the static study, bone scintigraphy was obtained 3 hours after injection of 99mTc-MDP by gamma camera equipped with a computer and uptake ratios were calculated. In the dynamic study, bone scintigraphy was performed in one, 3 and 5 hours after injection of radionuclide. Femoral head uptake ratio was markedly decreased in osteonecrosi
