99mTc-MDP-scintimetry was performed in 25 consecutive cases of radiographically silent transient synovitis of the hip in children. Fourteen cases had normal scintimetry; seven cases had an increased uptake in the epiphysis; four cases had markedly defective uptake in the epiphysis, indicating interrupted vascular supply. At repeat scintimetry 6 weeks later, the uptake was normal or increased in three of these four cases; the one case with a persistent defect was the only case in this series who later developed radiographic evidence of Legg-Calvé-Perthes' disease. Fourteen consecutive children with symptoms of transient synovitis of the hip were examined with sonography regarding intracapsular effusion, with scintimetry regarding blood-flow in the proximal femoral epiphysis (PFE) and with intracapsular pressure recording and aspiration. Intra-articular pressure was measured in 94 hips in 81 children with Perthes' disease and various forms of synovitis of the hip. The interferon system of 20 children aged 2 to 11 years (mean 5 years), diagnosed as having transient synovitis of the hip by clinical criteria, was studied. The mean blood interferon concentration was significantly higher than that of normal children, and the incidence of an antiviral state of cells (in 78% of patients) was also significantly higher than in the control group. These findings are compatible with the hypothesis that the aetiology of transient synovitis is an acute, possibly unusual, viral infection.

Forty-one children with transient synovitis of the hip and 10 children with Legg-Calve-Perthes disease were followed. All 41 cases of transient synovitis had a benign outcome. The efficacy of magnetic resonance imaging (MRI) in the assessment of pediatric hip disease was tested by scanning the hips of 24 children (30 scans). The development of transient synovitis in the hips of young children occurs quite frequently. This experiment examined the effects of a model synovitis on the deformability of articular cartilage of the immature rabbit hip. At 1, 2, 3, and 4 weeks following synovitis, there was an increase of cartilage deformability on both the acetabular and femoral sides of the joint. This increased deformability may alter force transmission to the underlying bone and its contained vascular structures.

A study of isotope scanning in children with irritable hip syndrome has demonstrated its role as a discriminator between Perthes disease and transient synovitis. In a 7-year-old boy with clinical symptoms of transient synovitis of the hip, an intracapsular effusion was diagnosed with computed tomography and ultrasonography. An intracapsular pressure of 50 mmHg was recorded prior to aspiration of 5.5 ml of clear fluid. Repeated ultrasonography immediately after aspiration verified a reduction in effusion, and serial examinations on the following days did not reveal any recurrence of the effusion.

Sixty children with Legg-Perthes disease (19), bone tumor (27), osteogenesis imperfecta (7), osteomyelitis (5) and transient synovitis (2) were studied using 99mTc labeled diphosphonate. A number of benign or malign bone diseases of children need early detection in order to institute the best form--the fine form--of treatment. We recommend the bone scintigraphy in the initial screening of children with signs and symptoms of bone pathology.

An important problem in investigating food allergic diseases is to adduce evidence that specific diseases of unknown cause may be attributable to food allergy.
