OBJECTIVE: To conduct a retrospective multicenter study to assess the ability of a predictive algorithm to differentiate between children with Kingella kingae infection of the hip and those with transient synovitis.
STUDY DESIGN: Medical charts of 25 Israeli and 9 Spanish children aged 6-27 months with culture-proven K kingae arthritis of the hip were reviewed, and information on the 4 variables included in the commonly used Kocher prediction algorithm (body temperature, refusal to bear weight, leukocytosis, and erythrocyte sedimentation rate) was gathered.
RESULTS: Patients with K kingae ar

Transient synovitis, septic hip, and Legg-Calvé-Perthes disease are common conditions in children. Distinguishing between these disorders can be a diagnostic challenge. Similar presentations, in an age group difficult to examine, coupled with literature that is confusing creates difficulty. It is important to make the correct diagnosis of septic hip in a timely fashion to avoid serious and potentially crippling consequences. As there is no single test for discriminating between these conditions, knowledge of the nuances of clinical presentation, physical examination, laboratory investigations,

OBJECTIVES: To estimate the prevalence of Lyme infection among children presenting with acute, nontraumatic hip pain in a Lyme endemic region and to investigate predictors of Lyme disease among children with suspected transient synovitis.
STUDY DESIGN: Retrospective cross-sectional study of children with unilateral hip pain who were brought to an academic pediatric emergency department. Cases were identified by specific discharge diagnoses or radiologic imaging. Lyme infection was determined by serologic criteria, and a minimum prevalence was estimated for the entire study population; maximum

PURPOSE: To comprise current knowledge on morphology, embryology and pathophysiology of synovial plicae as well as on clinical and therapeutic aspects of the plica syndrome.
METHODS: Review of the literature combined with a meta-analysis of studies assessing the outcome of open or arthroscopic plica excision including the author's own series.
RESULTS: The term synovial plica has been devised to describe a number of intra-capsular folds thought to represent remnants of a membranous knee joint partition present during foetal development. Although four such folds have been defined, it is mainly

INTRODUCTION: Hip transient synovitis (TS) is a common pediatric orthopaedic problem. Although a self-limiting illness, it often makes the patient temporarily disabled and poses a diagnostic difficulty because of its similarity to septic arthritis in clinical manifestations. The aim of this study was to evaluate the use of a single ultrasound-guided hip aspiration as a treatment modality for TS.
METHODS: Between the years 1984 and 1989, 112 children with TS were treated through bed rest and using nonsteroidal anti-inflammatory drugs (group 1). Between the years 1990 and 1999, 119 children dia

Pediatric hip pathology varies depending on patient age. Newborns are often screened for developmental dysplasia of the hip, whereas toddlers and older children can present later with pain, leg length discrepancy, or limp. Young children may have hip pain related to transient synovitis, septic arthritis, or Legg-Calvé-Perthes disease. Older children are more likely to suffer from slipped capital femoral epiphysis or apophyseal avulsion fractures. Knowledge about how the growing skeleton differs from adults as well as the classic imaging findings in many of these diagnoses is paramount when tak

UNLABELLED: Hip pain, in the paediatric population, is a common complaint and a cause for admission. There are several diagnoses one has to keep in mind, when making the differential diagnosis, as the treatment for each can be quite different. Often, the radiographs obtained for these patients turn up normal. Hence one has to rely on clinical findings, laboratory parameters, and possibly an ultrasound investigation in order to clinch the diagnosis and initiate the appropriate treatment.
KEY WORDS: Paediatric, hip, pain, normal radiographs.

A misdiagnosed septic arthritis (SA) of the paediatric hip is a nightmare for many physicians. The authors tried to trace the most recent information about this problem. A Medline search, using PubMed interface, was focused on the period from 01.01.2002 to 31.08. 2012. A total of 53 papers were included in the study. They led to the following statement, among others, about the differential diagnosis between SA and transient synovitis: "CRP > 20 mg/L, non-weight-bearing, temperature >38.5 degrees C and peripheral white blood cell count > 12 x 10(9) cells /L offers a predictive probability for s

The hip and proximate tissues are implicated in a variety of childhood conditions, and in the differential diagnosis of many more. To a large extent the possible diagnoses are limited by the child's age, an accurate history and thorough examination. Developmental dysplasia of the hip (DDH) is a spectrum disorder of joint development and/or instability. It is a major cause of morbidity in children and adults. It can be classified into three types: neuromuscular; teratological; and idiopathic (the most common type). Examination of the hips is carried out neonatally using Ortolani and Barlow mano

OBJECTIVE: To investigate the value of B ultrasonography in the early diagnosis of transient synovitis of hip (TSH) in dogs, and provide the valid base and data for the clinic early diagnosis of TSH.
METHODS: Eighty 2-3 month old dogs were injected 0.2% noradrenalin (NA) into the hip joint induced TSH. We observed 5 assessments that included the 99mTC-MDP triphasic imaging, X ray, B ultrasonography, the synovial fluid and the pathological tissue check in different time.
RESULTS: Early course of TSH presented the synovium of joint hemangiectasis, hyperaemia, synovium villus hyperplasia, edema

BACKGROUND: Septic arthritis is an emergency. In 1999 Kocher et al. identified four clinical criteria to distinguish hip septic arthritis from transient synovitis in children (nonweightbearing, erythrocyte sedimentation rate ≥ 40 mm/L, white blood cell count > 12 × 10(9)/L, temperature > 38.5°C). Subsequent authors evaluating the same criteria produced conflicting results. This calls into question the use of such diagnostic algorithms. The reasons for the differences remain unclear.
QUESTIONS/PURPOSES: To what degree do studies, evaluating the predictive ability of diagnostic algorithms for s

BACKGROUND: Transient synovitis of the hip (TS) is considered to be a self-limiting disease in childhood. However, because the etiology is unclear and some cases precede Legg-Perthes' disease, data on follow-up are important. Our aim was to summarize the knowledge on the clinical course of TS in children.
METHODS: The study design was a systematic review and a literature search was conducted in Medline and Embase. Studies describing short and/or long-term follow-up of TS in children were included. Case reports, reviews and studies describing traumatic hip pain were excluded. Study quality was

OBJECTIVE: The purpose of this study was to show the usefulness of dynamic contrast-enhanced MRI (DCE-MRI) and to determine the optimal time window in MRI for differentiating between septic arthritis and transient synovitis in painful hip joints.
MATERIALS AND METHODS: Eighteen patients who underwent DCE-MRI were enrolled, and the final diagnoses were septic arthritis (n = 7) and transient synovitis (n = 11). The enhancement patterns of DCE-MRI were dichotomized according to the shape of the time-signal intensity curves. The time at the maximal difference in the signal intensity between two t

Epidural morphine is widely used in veterinary medicine, but there is no information about the anti-hyperalgesic and anti-inflammatory effects in acute inflammatory joint disease in horses. The analgesic, anti-hyperalgesic and anti-inflammatory effects of epidural morphine (100mg/animal or 0.17 ± 0.02 mg/kg) were therefore investigated in horses with acute synovitis. In a cross-over study, synovitis was induced in the talocrural joint by intra-articular lipopolysaccharide (LPS). The effect of epidural morphine was evaluated using physiological, kinematic and behavioural variables. Ranges of mo

The appropriate imaging for pediatric patients (ages 0-5 years) being evaluated for limping depends on the clinical presentation, specifically, the presence of signs of infection, any localization of pain, and history of or suspected trauma. Common diagnoses causing limping in children are briefly reviewed, and recommended imaging techniques are discussed, including toddler's fracture, transient synovitis, septic arthritis, Legg-Calvé-Perthes disease, and osteomyelitis. The ACR Appropriateness Criteria(®) are evidence-based guidelines for specific clinical conditions that are reviewed every 2

Intra-articular injection of opioids provides analgesia in painful equine joints and μ-opioid receptors (MORs) have been demonstrated in equine synovial membranes. The aim of this study was to determine whether acute inflammatory conditions will lead to up-regulation of MOR in equine synovial membranes and whether anti-inflammatory treatment can prevent any such upregulation. In a two-period, blinded, placebo-controlled randomised cross-over design, lipopolysaccharide (LPS, 1.0 ng) was injected into the left or right middle carpal joint of seven healthy ponies. Arthroscopy and synovial membran

OBJECTIVES: Group A streptococcus (GAS) is a frequent cause of pediatric musculoskeletal infections including septic arthritis, acute rheumatic fever (ARF), and a more benign arthritis called post-streptococcal reactive arthritis. Children with painful joints are frequently evaluated in the acute care setting, and because the presentation of each of these entities is similar, the diagnosis can be difficult to make. Five cases of children with GAS arthridities are presented to demonstrate the spectrum of GAS-associated joint pathologies encountered in the acute care setting and also to discuss

Chickenpox has many rare complications; transient synovitis is one of the very painful and self-limiting rare complications. This patient suffered from transient synovitis associated with chickenpox. After being diagnosed, it was managed by diclofenac sodium suppositories. Physicians must be able to correlate all of the possible complications of chickenpox in order to avoid misdiagnoses.

Limp is a common clinical presentation in children, seen both in acute and community practice. The incidence is thought to vary between countries with suggested figures between 1.5 and 3.6 cases of non-traumatic limp per 1000 children. The authors present a case of a 6-year-old girl who attended our accident and emergency department with limp. It has been suggested that the most common diagnoses in our patient's age group are transient synovitis, septic arthritis, osteomyelitis, Perthe's disease and fractures or soft tissue injuries. Surprisingly our patient was diagnosed with haemophagocytic
