Earlier Initiation of Treatment Following HIV Acquisition Reduces Non-AIDS-defining Malignancy Risk.

BACKGROUND: Starting antiretroviral treatment (ART) at higher CD4 cell counts is associated with reduced non-AIDS-defining malignancy (NADM) risk. We studied whether starting ART within 1 year after acquiring human immunodeficiency virus type 1 (HIV-1) reduces this risk while also adjusting for socioeconomic status. METHODS: We included individuals (≥18 years) from the Dutch national ATHENA cohort diagnosed with human immunodeficiency virus (HIV) and having started ART between 2000 and 2022 without prior NADM with ≥6 months of follow-up. "Early ART" initiators were defined as starting ART <36

Risk of Hepatocellular Carcinoma After Direct-Acting Antiviral Treatment for Hepatitis C Virus Infection in People With HIV.

BACKGROUND: To inform hepatocellular carcinoma (HCC) surveillance after hepatitis C virus (HCV) cure with direct-acting antivirals (DAA), we estimated HCC risk post-DAA in people with human immunodeficiency virus (HIV) with advanced liver fibrosis or cirrhosis under universal DAA. METHODS: We used data from HepCAUSAL, a collaboration of cohorts of HIV-HCV coinfection from Europe and North America. Eligibility criteria were HIV-HCV coinfection, advanced liver fibrosis or cirrhosis, DAA naïve, HIV-RNA < 50 copies/mL, on antiretroviral therapy, no HBV coinfection, and no prior HCC diagnosis or l

Missed opportunities for anal cancer (AC) screening in women living with HIV: results from a survey across the European region.

OBJECTIVE: The incidence of anal cancer (AC) is higher in women with HIV than in women without HIV due to immunosuppression and persistence of human papilloma virus (HPV). Since 2024, the International Anal Neoplasia Society's and European AIDS Clinical Society (EACS) guidelines recommend annual AC screening of cisgender women (CW) of ≥45 years old, transgender women (TW) of ≥35 years old and women with previous vulvar high-grade squamous intraepithelial lesion (HSIL)/cancer regardless of age. This study describes current clinical practices and protocols for AC screening in women with HIV with

The impact of HIV on cervical cancer elimination in KwaZulu-Natal: a comparative modeling analysis.

BACKGROUND: Achieving cervical cancer (CC) elimination requires addressing disparities in CC burden for women living with HIV (WLHIV) and how disparities evolve in the context of antiretroviral therapy (ART) scale-up. To inform CC elimination for high HIV prevalence regions, we modeled the impact of HIV, HIV interventions, and CC interventions in KwaZulu-Natal, South Africa. METHODS: We used 2 independently developed, dynamic compartmental transmission models of HIV and human papillomavirus (DRIVE and Policy1-Cervix-HIV) calibrated to KwaZulu-Natal. We simulated: a counterfactual without HIV

Population Analysis and Immunologic Landscape of Melanoma in People Living with HIV.

PURPOSE: To dissect the clinical and immunologic features of people living with human immunodeficiency virus (HIV; PLWH) diagnosed with melanoma, who have consistently shown worse outcomes than HIV-negative individuals (PLw/oH) with the same cancer. EXPERIMENTAL DESIGN: We analyzed electronic health records from 1,087 PLWH and 394,437 PLw/oH with melanoma. Demographic and clinical characteristics were compared. Spatial immune transcriptomics (72 immune-related genes) was performed on melanoma tumor samples (n = 11), with downstream validation using multiplex immunofluorescence (mIF; n = 15 PL

Health-related quality of life among adult cancer patients with and without HIV: Analysis of SEER-MHOS data (2007-2017).

INTRODUCTION: People with HIV (PWH) are more likely to die due to cancer compared to people without HIV. Disparities in cancer care, including access to palliative care, for PWH contribute to poor health-related quality of life (HRQoL) and survival. However, limited research exists examining patient-reported HRQoL among PWH with cancer. We examined HRQoL among patients diagnosed with non-AIDS defining cancers with and without HIV. MATERIALS AND METHODS: We used the 2007-2017 Surveillance, Epidemiology, and End Results and the Medicare Health Outcomes Survey (SEER-MHOS) linkage data to assess

Trends in the Incidence of HPV-Associated Anal Cancer by Risk Group, 1990-2019: A Population-Based Cohort Study in British Columbia, Canada.

BACKGROUND: Anal squamous cell carcinoma caused by human papillomavirus disproportionately affects people living with human immunodeficiency virus (HIV; PLWH), particularly gay, bisexual, and other men who have sex with men (GBM). New guidelines recommend screening and treatment of precancerous lesions. We aimed to estimate anal cancer incidence by HIV status, sex, and GBM status in British Columbia, Canada. METHODS: Using administrative health databases, we assessed anal cancer stratified by HIV status, sex, and sexual orientation from 1990 to 2019. A phenotypic algorithm was used to classif

Point-of-care HPV testing for cervical cancer screening in Sub-Saharan Africa: platform diversity, diagnostic performance, implementation feasibility, and determinants-a scoping review with contextual considerations for Ethiopia.

BACKGROUND: Cervical cancer remains a leading cause of cancer-related mortality among women in Sub-Saharan Africa (SSA), where access to laboratory-based screening is limited. Point-of-care (POC) HPV testing enables rapid, near-patient diagnostics that support same-day screen- and treat strategies; however, the evidence base across SSA remains fragmented. METHODS: We conducted a scoping review following the Arksey and O’ Malley and the Joanna Briggs Institute (JBI) framework. Ten electronic databases and grey literature sources (2000–2025) were searched for studies evaluating POC HPV tests in

A proimmunotoxin nanodrug targeting AIDS-associated non-Hodgkin lymphoma.

B-cell non-Hodgkin lymphoma (NHL) is the most common hematopoietic malignancy in the United States, with a notably higher incidence and aggressiveness observed in individuals infected with human immunodeficiency virus (HIV), leading to AIDS-associated NHL (AIDS-NHL). The transferrin receptor 1 (TfR1/CD71), a type II transmembrane homodimeric protein, is overexpressed on several cancers, including NHL, providing a meaningful therapeutic target. Our group developed an anti-TfR1 IgG3-avidin fusion protein, ch128.1Av, designed to deliver biotinylated therapeutic agents into cancer cells through re

Exploring variation in research priorities generated by AI tools.

BACKGROUND: Artificial intelligence (AI) tools based on large language models (LLMs) are being increasingly used by researchers and may play a role in health-related research priority-setting exercises (RPSEs). However, little is known about how these tools may differ in the types of research priorities they generate. METHODS: We examined research priorities aimed at improving treatments for four diseases: cancer, COVID-19, HIV, and Alzheimer. We compared the outputs from five AI tools (DeepSeek, ChatGPT, Claude, Perplexity, and Gemini) using SBERT-BioBERT embeddings and cosine similarity sco

DNA Methylation in Cervical Intraepithelial Neoplasia and Cervical Cancer: Triage and Management.

This chapter discusses the role of DNA methylation, an epigenetic mechanism that regulates gene expression, as a key event in cervical carcinogenesis, and highlights its potential as a biomarker for detecting cervical cancer and high-grade precancerous lesions. Methylation levels of specific genes (methylation markers) gradually increase with cervical disease severity, allowing several clinical applications. Methylation markers have been evaluated as alternative triage tools for high-risk (hr) human papillomavirus (HPV)-positive women, including those with borderline or mildly abnormal cytolog

Enhancing cervical cancer screening uptake through self-sampling for HPV DNA testing: insights and results from implementing in routine HIV clinics serving women living with HIV in Nairobi, Kenya, 2021-2024.

BACKGROUND: In sub-Saharan Africa (SSA), only 14% of women aged 30 to 49 years have been screened for cervical cancer even though the region accounts for a quarter of global cervical cancer deaths. Women living with HIV are six times more likely to develop cervical cancer as compared to HIV-uninfected women in SSA. Unlike traditional screening methods such as Visual Inspection with Acetic Acid (VIA), the self-sampling for Human Papillomavirus (HPV) DNA testing offers a promising alternative as it reduces stigma, fear, and discomfort associated with cervical screening. However, evidence on its

Integration of HIV Status in Cancer Surveillance in South Africa: A Call for Action.

BACKGROUND: Human immunodeficiency virus (HIV) increases the risk of developing cancer. We aimed to assign HIV status to cancers diagnosed in public laboratories recorded in the National Cancer Registry (NCR) in South Africa, guided by HIV counselling and testing guidelines. METHODS: We used natural language processing to extract HIV-related information from free-text reports and probabilistic record linkage to match cancers diagnosed between 2004 and 2021 to HIV-related tests from the National Health Laboratory Service Corporate Data Warehouse. We assigned HIV status based on the results of

The relational association between multiple sexual partners and HIV testing on cervical cancer screening among women of reproductive age in Ghana: A national population-based study.

BACKGROUND: Cervical cancer screening remains vital in the early detection of precancerous lesions and promotes better treatment outcomes. Though evidence suggested multiple sexual partners and HIV infection as risk factors for cervical cancer, limited studies have investigated how multiple sexual partners and HIV infection impact cervical cancer screening in Ghana. Therefore, this study assessed the association of multiple sexual partnership and HIV testing on cervical cancer screening among Ghanaian women of reproductive age. METHODS: The study utilized data from the 2022 Ghana Demographic
