When Swallowing Triggers VT.

We describe a case of swallowing-induced ventricular tachycardia in a young, otherwise healthy male with a structurally normal heart which was refractory to multiple anti-arrhythmic medication and was successfully treated with ablation of a cardiac ganglion in the crux of the heart. We use this case to describe an unusual disease presentation and highlight current evidence related to evaluation and management of ventricular arrhythmias and add to the limited body of evidence related to swallowing induced tachycardia.

Incision-triggered neuroimmune remodeling of nociceptor ion channels: from postoperative inflammation to pain chronification.

Chronic postsurgical pain (CPSP) arises after a known tissue insult, yet no intervention reliably prevents its development. This Review examines whether incision-triggered inflammation remodels nociceptor voltage-gated ion channels in ways that sustain peripheral input and favor pain chronification. We integrate human genetic and clinical evidence, incision-specific perturbation studies, functional work in non-surgical models, and expression-level associations distinguishing mechanistic strength from directness to CPSP. We focus on NaV1.7, NaV1.8, NaV1.9, and CaV3.2 within a broader network th

Microvasculopathy in spinal muscular atrophy: The endothelium as a target of SMN deficiency and an amplifier of disease.

Spinal muscular atrophy (SMA) is caused by deficiency of the ubiquitous survival motor neuron (SMN) protein. Degeneration of anterior horn motor neurons defines the disease, but many organs are affected, including heart, liver, kidney, and bone. The vascular compartment is among them: digital necrosis in infants with the fewest SMN2 copies, capillary rarefaction in SMA mice, and decreased von Willebrand factor with fibrinogen extravasation in human post-mortem spinal cord. Whether these reflect endothelial injury by SMN deficiency itself or capillary adaptation to atrophic, denervated tissue h

Characterizing NaV1.7 antisense analgesia through antagonist binding, mRNA expression, and neuraxial distribution.

Genome-targeting strategies to address NaV1.7-mediated signaling in nociceptive afferents are of great clinical interest given its high selectivity and persistent analgesic outcomes. In the present work, we analyze concentration-dependent effects of suppressing primary afferent NaV1.7 using the intrathecal delivery of an antisense oligonucleotide (ASO) on pain behaviors and demonstrate that NaV1.7 ASOs are likely to modulate pain traffic by blocking afferent signaling at the dorsal root ganglion (DRG) level. Intrathecal NaV1.7 ASOs induced a concentration-dependent gradient of knockdown in DRG

Neuroimmune activation of the TRPV1-RAMP1 axis drives type 2 immunity to repair intestinal barrier and alleviate metabolic disease.

INTRODUCTION: Obesity is closely associated with intestinal barrier dysfunction and chronic inflammation. Group 2 innate lymphoid cells (ILC2s) can receive and integrate signals from neuropeptides, neurotransmitters, and epithelial cells, thereby regulating local tissue inflammatory responses and promoting repair. However, the specific mechanism by which the nociceptor Transient Receptor Potential Vanilloid 1 (TRPV1) interacts with ILC2s to regulate intestinal homeostasis in obesity remains unclear. OBJECTIVE: This study aimed to investigate whether electroacupuncture (EA) ameliorated obesity

Spinorphin Ameliorates Nitroglycerin-Induced Migraine-Like Responses and Trigeminovascular Neuroinflammation Potentially Through Opioid Receptor-Mediated Mechanisms.

Enhancement of endogenous opioid signaling through inhibition of enkephalin degradation has emerged as a potential strategy for pain control, yet its role in migraine remains unclear. This study investigated whether spinorphin, an endogenous inhibitor of multiple enkephalinases, attenuates trigeminovascular activation and migraine-like responses in experimental migraine. Migraine-like pain was induced in male Wistar rats by nitroglycerin. The effects of spinorphin were evaluated using periorbital and plantar von Frey tests, followed by assessment of c-Fos levels in the trigeminal nucleus cauda

Research Progress on Intraocular Pressure-lowering Drugs, Commonly Used Clinical Adjuvants, and Emerging Neuroprotective Agents for Glaucomatous Optic Neuropathy.

Glaucomatous optic neuropathy (GON) is the leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss. Current clinical treatment relies primarily on IOP-lowering drugs, but a subset of patients continues to experience progressive optic nerve damage. Adjuvant therapies such as antioxidants and calcium channel blockers have limited efficacy. Emerging neuroprotective strategies including NMDA receptor antagonists, neurotrophic factors, and gene therapy remain in preclinical or early-stage trials. This review systematically summarizes the mech

Potential sphenopalatine ganglion block efficacy as an alternative to epidural bloodpatch for post-dural puncture headache: a monocentric french study.

BACKGROUND: Post-dural puncture headache (PDPH) is a common complication of lumbar puncture caused by cerebrospinal fluid leakage and intracranial hypotension. Epidural blood patch (EBP) is considered the gold standard treatment but remains invasive. Sphenopalatine ganglion block (SPGB), a non-invasive technique, has shown promising results in recent studies, but it was not evaluated as an alternative of EBP. This study aimed to evaluate the effectiveness of SPGB in PDPH by determining whether it could reduce the need for EBP in clinical practice. METHODS: We conducted a prospective monocentr

Tissue-specific peripheral sensory remodeling in chronic low back pain: established and emerging nociceptive niches.

Chronic low back pain arises from heterogeneous peripheral and central mechanisms, yet the sensory biology of individual lumbar tissues is rarely considered within a unified framework. This Mini Review compares peripheral sensory remodeling and sensitization across four candidate nociceptive niches: the intervertebral disc, vertebral endplate, thoracolumbar fascia, and paraspinal muscle. Disc pathology is characterized by nociceptive ingrowth, endplate pathology by osteochondral sensory remodeling and afferent sensitization, and thoracolumbar fascia by inflammation-associated remodeling of a p

Identification of TREK channels in mouse intracardiac ganglion neurons.

The intracardiac nervous system is a complex neural network formed by a diffuse plexus, the so-called intracardiac ganglion (ICG). These ganglia locally innervate and control heart function, but little is known about how ICG neuronal activity is regulated. Although various ionic currents have been characterized in cardiac tissue, the expression and role of TWIK-related K+ (TREK) channels in ICG neurons remain unexplored. TREK channels, members of the two-pore domain potassium channel family, are essential regulators of neuronal membrane resting potential and excitability, with recognized neuro

Orofacial neuropathic pain and enhanced excitability of non-nociceptive-like and nociceptive-like trigeminal Aβ-afferent neurons in mice following infraorbital nerve chronic constrictive injury.

Orofacial neuropathic pain is a debilitating pain condition. It is poorly treated, partially due to an incomplete understanding of its underlying neurophysiological mechanisms. Orofacial neuropathic pain is often caused by injury or dysfunction of trigeminal nerve branches, including the infraorbital nerve (IoN). Clinically, a main symptom of orofacial neuropathic pain is mechanical allodynia, excruciating pain induced by gentle touch. While previous studies on neuropathic pain have mainly focused on the involvement of C- and Aδ-afferents, the potential contribution of Aβ-afferents to orofacia

ABCC1 Deficiency Mediates Susceptibility to Cd-Induced Hearing Loss via the NOX2/ROS Axis.

Cadmium (Cd), a prevalent environmental contaminant, is associated with auditory dysfunction. The cochlear blood-labyrinth barrier (BLB) protects the inner ear from toxic substances, a process in which the ATP-binding cassette subfamily C member 1 protein, ABCC1, plays a critical role in expelling toxins from its cells. To understand the specific contribution of ABCC1 to Cd-induced hearing damage, we employed both an in vivo model (ABCC1 knockout mice, Abcc1 -/- mice) and an in vitro endothelial model (ABCC1-deficient human umbilical vein endothelial cells, ABCC1 -/-HUVECs). These models were

Superior cervical sympathetic ganglion enlargement in idiopathic intracranial hypertension: An exploratory case-control study.

AimIdiopathic intracranial hypertension (IIH) may involve bidirectional interactions between elevated intracranial pressure (ICP) and sympathetic nervous system activity along an ICP-sympathetic axis. We hypothesized that superior cervical sympathetic ganglion (SCG) volume is increased in IIH and investigated whether SCG enlargement on MRI may serve as a potential imaging covariate of disease severity associated with cerebrospinal fluid opening pressure (OP).MethodsIn this retrospective case control study, we selected IIH patients with a brain MRI within six weeks prior to lumbar puncture, and
