Integrated single-cell and bulk-tissue transcriptomic analysis reveals an epithelial-stromal inflammatory microenvironment in biliary atresia.

BACKGROUND: Biliary atresia is a severe fibroinflammatory cholangiopathy of infancy. Its pathological process involves biliary epithelial injury, immune activation, and intrahepatic stromal remodeling, but the cellular interactions linking these processes remain incompletely defined. Using public single-cell and bulk-tissue transcriptomic cohorts, this study systematically characterized disease-associated alterations across liver cell populations in biliary atresia and further evaluated their consistency in independent cohorts. METHODS: Public single-cell liver data (GSE176189) and three inde

Extracorporeal shockwave therapy in musculoskeletal conditions: a narrative review.

Extracorporeal shockwave therapy (ESWT) has evolved from its initial application in urologic lithotripsy into an increasingly utilized noninvasive treatment modality for musculoskeletal disorders. This narrative review summarizes the physical principles, mechanisms of action, treatment protocols, contraindications, and clinical applications of ESWT across a range of orthopedic and sports medicine conditions. Two primary forms of ESWT are discussed: focused shockwave therapy (FSWT) and radial shockwave therapy (RSWT), which differ in wave generation, tissue penetration, and energy delivery. Pro

Sex differences in rheumatoid arthritis: results of a cross-sectional study.

INTRODUCTION: Rheumatoid arthritis (RA) shows sex-related differences, but the expression across clinical domains remains inconsistent. We compared women and men with RA and explored characteristics associated with male sex. METHODS: This cross-sectional study included 240 adults with RA (160 women and 80 men). Demographic, clinical, laboratory, radiographic, treatment, and comorbidity data were compared by sex. An exploratory multivariable logistic regression model, with male sex coded as 1, was used to identify distinguishing characteristics. RESULTS: Women were older and had higher body m

The diagnostic signature of diabetic enthesopathy: bilateral symmetry, serological silence, and ultrasound dominance.

OBJECTIVE: To characterize the distinctive bilateral, serological, and radiographic phenotype of diabetic enthesopathy using standardized quantitative ultrasonography with validated cutoff thresholds. METHODS: This cross-sectional extended analysis of a previously published cohort enrolled 60 patients with diabetes (17 T1DM, 43 T2DM). Five bilateral entheseal sites were evaluated using the Glasgow Ultrasound Enthesitis Scoring System (GUESS) methodology with validated thickness cutoffs (> 6.1 mm quadriceps, >4.0 mm patellar, >5.2 mm Achilles, >4.4 mm plantar fascia). Rheumatoid factor (RF), a

Successful endovascular management of popliteal artery adventitial cystic disease with expanded polytetrafluoroethylene stent graft reinforced with interwoven nitinol stent.

Popliteal adventitial cystic disease has traditionally been treated with open surgery of cyst excision, patch repair of artery, or interposition graft. Endovascular angioplasty or stenting has demonstrated poor mid- to long-term results because high-viscosity tension of the adventitial cyst continues to exert extrinsic compression, resulting in early elastic recoil and disease recurrence. To our knowledge, this is the first report in the literature of an effective endovascular strategy using a popliteal stent graft reinforced with an interwoven nitinol stent to treat popliteal cysts, with favo

Senotherapeutic potential against xeroderma pigmentosum.

BACKGROUND: Xeroderma pigmentosum (XP) is an inherited photoaging syndrome caused by mutations in genes involved in the nucleotide excision repair (NER) pathway. XP patients exhibit hypersensitivity to ultraviolet (UV) radiation, leading to accelerated skin aging and requiring lifelong sun avoidance. In this study, we aimed to investigate UV-induced cellular senescence in XP melanocytes and explore potential senotherapeutic strategies. METHODS: Melanocytes differentiated from iPSCs derived from an XP-A patient and a healthy individual, respectively, were used to induce senescence. Senescence

Capsulectomy at Explant: Rethinking Spinal Cord Stimulator Infected Generator Pocket Management in the Immunocompromised-A Case Series.

BACKGROUND: Surgical site infections (SSI) associated with spinal cord stimulation (SCS) implants pose risks of morbidity, mortality, and increased healthcare costs. Immunocompromised patients are especially vulnerable due to impaired immune responses, delaying diagnosis and increasing complications. Current guidelines recommend SCS explantation for deep or inadequately treated superficial SSI, though the role of capsulectomy remains underexplored despite its use in other fields like plastic surgery and cardiology. CASE PRESENTATION: We present two cases of immunocompromised patients with SSI

Deep Chest Wall Abscess Involving the Pectoralis Muscle Caused by Actinomyces turicensis and Bacteroides ovatus Mimicking Necrotizing Soft Tissue Infection.

Deep chest wall abscesses may be difficult to distinguish from primary pyomyositis and necrotizing soft-tissue infection because of overlapping clinical and radiographic findings. Although soft-tissue gas raises concern for necrotizing infection, it is not independently diagnostic and may reflect anaerobic bacterial activity, communication with the skin, or prior instrumentation. Infections of the pectoral region involving uncommon polymicrobial organisms are rarely reported. A 61-year-old man with prediabetes, hypertension, chronic kidney disease, and a prior cerebrovascular accident presente

Predictors of positive culture in NAAT-confirmed rectal N. gonorrhoeae infection among men who have sex with men.

BACKGROUND: Gonorrhoea is a common sexually transmitted infection (STI) among men who have sex with men (MSM). Nucleic acid amplification tests (NAATs) are the recommended diagnostic method; however, the interpretation of positive results in asymptomatic individuals remains uncertain. The aim of this study was to identify factors associated with positive culture results in NAAT-confirmed rectal gonorrhoea. METHODS: This retrospective study included individuals receiving HIV care, pre-exposure prophylaxis or STI screening at the Infectious Diseases Unit of San Raffaele Hospital, Milan, Italy,

Gastrin and cholecystokinin 2 receptor, and gastrin-releasing peptide receptor in cancer: From pathogenesis to precision medicine.

Gastrin regulates gastric acid secretion and mucosal homeostasis via the cholecystokinin-2 receptor (CCK2R). Aberrant signaling via CCK2R (cholecystokinin family) and the distinct class A GPCR, gastrin-releasing peptide receptor (GRPR; bombesin family), drives oncogenesis across diverse GI and non-GI malignancies through calcium mobilization, MAPK/PI3K activation, EGFR transactivation, and constitutive splice variants like CCK2Ri4sv. Furthermore, precursor forms such as progastrin remodel the tumor microenvironment by interacting with non-CCK binding sites (p78, Annexin A2), converting fibrobl

FGFR4-associated APOBEC3 mutagenesis characterizes HER2-enriched subtype of human breast cancer.

BACKGROUND: Characterising somatic mutation profiles in human breast cancer (HBC) is essential for understanding tumour progression and guiding therapeutic strategies. METHODS: We performed genomic and transcriptomic analyses to profile the mutational landscape of HBC. In addition to our primary analysis of HBC, we conducted comparative genomic analyses to evaluate the extent to which these mutational processes are recapitulated in canine mammary tumour (CMT), a widely proposed translational model. RESULTS: APOBEC3 (A3)-associated mutations were extensive in HBC but largely absent in CMT, li

The Aging Microenvironment Is a Determinant of Immune Exclusion and Metastatic Fate in Pancreatic Cancer.

UNLABELLED: Aging is a critical yet understudied determinant in pancreatic ductal adenocarcinoma (PDAC) risk and outcomes. Despite a strong epidemiologic association with age, conventional PDAC preclinical models fail to capture the histopathologic and stromal complexities that emerge in older organisms. Using an age-relevant syngeneic orthotopic model, we demonstrated that organismal aging accelerates PDAC progression and metastasis. Transcriptomic and secretome profiling identified a conserved extracellular matrix gene signature enriched in cancer-associated fibroblasts (CAF) from aged tumor

Motor cortex pain compensation is modifiable: allostatic load degrades its analgesic efficiency and rehabilitation restores it.

The primary motor cortex is a critical hub for pain control, yet its few identified regulatory mechanisms - intracortical inhibition and oscillatory markers - predict symptoms inconsistently. The prevailing view holds that pain results from reduced inhibition, a fixed deficit, rather than from resistance to inhibition that rehabilitation can reverse. Here we show that motor cortex pain compensation is instead a modifiable state whose analgesic efficiency depends on allostatic load. In a fibromyalgia trial and a knee osteoarthritis cohort assessed with parallel EEG and transcranial magnetic sti
