Myotonic Dystrophy type 2 unmasked by physical activity resumption following COVID-19 lockdown: case discussion and review of the literature.

OBJECTIVE: Myotonic dystrophy type 2 (DM2; PROMM) is characterized by myotonia and muscle dysfunction, episodic muscle pain, proximal and axial weakness of the neck flexors. We describe the case of a young woman affected with a clinically silent form of DM2 disclosed by her return to physical exercise, a 7 km walk, after Covid-19 lockdown. METHODS: The patient underwent neurological examination, serum CK dosage and electromyography after assessing the Emergency Room complaining of cramps and severe myalgia. Molecular screening for CNBP expansions was carried out on the patient and her family.

Adult-Onset Still's Disease Mimicking Myositis: A Case Report.

The combination of severe myalgia, progressive weakness, and blood in the urine often leads a neurologist to consider myositis. Accordingly, reddish urine may be linked to urine myoglobinuria brought about by muscle destruction. Nevertheless, in a young patient with normal creatine kinase complaining of immobility, adult-onset Still's disease (AOSD) should be one of the top differentials. We discuss a case of a 25-year-old Filipino male who presented to our clinic with a month's history of progressive, generalized weakness, joint pains, and rashes, accompanied initially by undocumented fever,

Identification of the PROMs used to assess ICI toxicities and HRQoL in patients receiving immune checkpoint inhibitor treatment in cancer care and their suitability: A systematic review.

BACKGROUND: The implementation of patient-reported outcome measures (PROMs) in the clinical identification of immunotherapy toxicities is a complex intervention. There has been very little work evaluating the clinical utility and generalisability of PROMs used after immune checkpoint inhibitor (ICI) treatment to date. We reviewed evidence on the use of PROMs assessing toxicities and health-related quality of life in patients treated with ICIs. METHODS: PubMed, EMBASE, MEDLINE, PsycInfo, CINAHL, Web of Knowledge, the Cochrane Library were searched (January 2008 - October 2024). Quantitative st

Anti MDA-5 associated rapidly progressive interstitial lung disease complicated by viral pneumonia - a fatal outcome.

Dermatomyositis is a heterogeneous systemic disease, with 7% to 10% of the individuals presenting the Anti MDA-5 antibody. This subset of patients has clinically amyotropic dermatomyositis, presenting with cutaneous ulcer and rapidly progressive interstitial lung disease. We report the case of a 22-year-old male with a six-month history of low-grade fever associated with myalgia, polyarthralgia, and marked weight loss. He had a history of shortness of breath and high-grade fever 15 days before admission. His clinical features and imaging workup were consistent with acute respiratory distress s

IAPSM's Position Paper on the Human Papilloma Virus (HPV) Vaccine for Adult Immunization in India.

Cervical cancer ranks as the second most common cancer in women in India, primarily caused by persistent infection with the human papillomavirus (HPV). Given its long latent period, secondary prevention through screening and early detection is essential. However, fear and stigma associated with cancers and the costs involved in disease management are the prominent barriers to its uptake. HPV vaccination is one of the vital components of the World Health Organization's (WHO) Global Strategy to speed up the elimination of cervical cancer as a public health problem. In India, four prophylactic HP

Hidden in Plain Sight: Antisynthetase Syndrome Masquerading As Acute Coronary Syndrome and Heart Failure.

A middle-aged man presented with fever, generalized myalgia, dyspnea, and worsening chest discomfort. Initial investigations showed elevated troponin and creatinine kinase levels and nonspecific ECG changes. Pulmonary embolism was ruled out, and high-resolution CT (HRCT) revealed ground-glass opacities and nonspecific interstitial pneumonia (NSIP)/organizing pneumonia (OP) overlap pattern. Given the elevated creatinine kinase level, antisynthetase syndrome with rapidly progressive interstitial lung disease (RP-ILD) was suspected. Treatment with pulse-dose methylprednisolone and tacrolimus led

Low serum S100A6 levels are associated RP-ILD risk in anti-MDA5-positive dermatomyositis.

INTRODUCTION: Anti-MDA5-positive dermatomyositis (anti-MDA5-DM) is a rare autoimmune disease that often leads to rapid-progressive interstitial lung disease (RP-ILD). The lack of effective prediction and treatment methods makes RP-ILD a major risk factor for death in patients with this condition. S100A6 is a member of the S100 Ca2 + - binding protein family, which plays important roles in inflammation, tumor, injury, and fibroblast reparation. This study aims to explore the correlation between serum S100A6 and RP-ILD in anti-MDA5-DM, and to determine whether S100A6 can be used as a specific bi

Effect on Pain Following One Session of Botulinum Toxin Type A in Patients With Jaw Myalgia: A Randomised Double-Blind Controlled Multicentre Pilot Study.

BACKGROUND: Botulinum toxin Type A (BTX-A) is increasingly used in the management of myofascial pain; however, the evidence to support this treatment modality is still inconclusive. OBJECTIVE: To evaluate the change in days with functional jaw pain after one session of BTX-A or placebo injections into the masseter and temporalis muscles in subjects with jaw myalgia. The hypothesis was that BTX-A is effective in reducing the number of days with functional jaw pain. METHODS: This was a randomised, double-blind, parallel group study in subjects with jaw myalgia. After randomisation, BTX-A or pl

Prevalence of Fibromyalgia and Widespread Pain in Psoriatic Arthritis: Association With Disease Severity Assessment in a Large US Registry.

OBJECTIVE: The classic conception of pain etiology in rheumatologic disease is nociceptive pain-tissue injury and inflammation signaling through peripheral and central nerve fibers. But this can be mixed with other pain etiologies, including nociplastic, which is augmented pain experience due to central sensitization. The pain of fibromyalgia (FM) is nociplastic, occurs in 10% to 30% of patients with rheumatologic disease, and its presence can influence disease severity assessment. The objective of our study was to (1) ascertain the prevalence of FM and widespread pain (WP) in the CorEvitas ps

Fibroblasts with high matrix metalloproteinase 2 expression regulate CD8+ T-cell residency and inflammation via CD100 in psoriasis.

BACKGROUND: Psoriasis is a T cell-mediated chronic inflammatory skin condition characterized by the interaction of T cells with various cell types, forming an inflammatory microenvironment that sustains psoriatic inflammation. Homeostasis of these tissue-resident T cells is supported by fibroblasts, the primary structural cells in the dermis. In psoriasis, there is increased expression of matrix metalloproteinase 2 (MMP2), mediating structural alterations in skin tissues and modulating inflammation. Additionally, the CD100-plexin-B2 (PLXNB2) axis is known to enhance psoriasis inflammation via

Uncover spatially informed variations for single-cell spatial transcriptomics with STew.

MOTIVATION: The recent spatial transcriptomics (ST) technologies have enabled characterization of gene expression patterns and spatial information, advancing our understanding of cell lineages within diseased tissues. Several analytical approaches have been proposed for ST data, but effectively utilizing spatial information to unveil the shared variation with gene expression remains a challenge. RESULTS: We introduce STew, a Spatial Transcriptomic multi-viEW representation learning method, to jointly analyze spatial information and gene expression in a scalable manner, followed by a data-driv

Engineered Self-Regulating Macrophages for Targeted Anti-inflammatory Drug Delivery.

BACKGROUND: Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by increased levels of inflammation that primarily manifests in the joints. Macrophages act as key drivers for the progression of RA, contributing to the perpetuation of chronic inflammation and dysregulation of pro-inflammatory cytokines such as interleukin 1 (IL-1). The goal of this study was to develop a macrophage-based cell therapy for biologic drug delivery in an autoregulated manner. METHODS: For proof-of-concept, we developed "smart" macrophages to mitigate the effects of IL-1 by delivering its inhibi
