A Case of Aortic Mural Thrombus Presenting as Recurrent Shoulder Pain Discovered by the Presence of Splinter Hemorrhages.

Progressive claudication of the upper extremity is uncommon and can be misattributed to cervical radiculopathy or even shoulder pathology, such as tendonitis or bursitis. We report a patient presenting to the emergency department with several months of worsening left upper-extremity pain initially managed as suspected radiculopathy, but, in retrospect, was upper-extremity claudication. This was discovered through the presence of lesions beneath the fingernails of the left hand, splinter hemorrhages. A computed tomographic angiography (CTA) of the chest for aortic and vascular evaluation demons

Adding reciprocal inhibition to conventional physiotherapy for adhesive capsulitis of the shoulder in Bangladesh: a study protocol for a randomised controlled trial evaluating pain, function and range of motion.

INTRODUCTION: Adhesive capsulitis (frozen shoulder) is a common condition characterised by pain, stiffness and reduced function. While conventional physiotherapy (typically comprising joint mobilisation, stretching, strengthening and electrotherapy) is widely used, there is limited evidence for the effectiveness of advanced neuromuscular techniques such as the reciprocal inhibition technique (RIT). This study aims to evaluate the impact of incorporating RIT into standard physiotherapy compared with standard physiotherapy alone in individuals with adhesive capsulitis. METHODS AND ANALYSIS: Thi

Individualized Deltoid Landmark and Needle Length for Safe Intramuscular Vaccination in Southeast Asian Adults: An Ultrasound Study.

Background/Objectives: An incorrect intradeltoid injection technique can cause shoulder injury related to vaccine administration, including bursitis, septic arthritis, and axillary nerve injury, particularly when Western-derived landmarks and needle-length tables are applied to smaller-framed Southeast Asian adults. We aimed to define an individualized deltoid injection landmark and needle length that avoid the axillary nerve while ensuring reliable intramuscular delivery in Southeast Asian adults. Methods: In this cross-sectional ultrasound study of adults aged ≥18 years, four vertical landma

Neurotized Free Anterolateral Thigh Flap for Large Tissue Defect and Joint Exposure in the Setting of Olecranon Bursitis: A Case Report.

Elbow wounds secondary to septic olecranon bursitis are a challenging problem to treat. Neurotized fasciocutaneous free anterolateral thigh (ALT) flaps provide a reliable soft tissue coverage option with the opportunity to provide protective sensation to a common pressure point. Free flaps can provide coverage when alternative options lower on the reconstructive ladder are unavailable. A 69-year-old man with rheumatoid arthritis presented with septic olecranon bursitis requiring serial debridement resulting in significant soft tissue loss and bone exposure. A free ALT, with fascial tacking sut

Ultrasound-guided double-needle lavage for acute calcific periarthritis of the hand: technical description and preliminary experience in five patients.

This study describes a standardized ultrasound-guided double-needle lavage technique for acute calcific periarthritis of the hand and reports its technical feasibility and preliminary clinical outcomes. Five consecutive patients underwent ultrasound-guided percutaneous lavage. Clinical characteristics, affected joint, calcification size, sonographic features, complications, and visual analog scale (VAS) scores at baseline, 1 week, and 1 month were recorded. Four cases involved the metacarpophalangeal joints, whereas one involved the interphalangeal joint. Mean calcification size was 14.4 ± 6.0

Morphological Characteristics of the Distal Iliopsoas Tendon and Its Relationship to Adjacent Osseous Structures.

Background The iliopsoas muscle influences lumbo-pelvic posture and gait stability, with its distal portions contributing to common anterior hip pathologies, including iliopsoas bursitis, impingement, and tendinopathy. Despite its role in such pathologies, the anatomical variation of the iliopsoas musculotendinous morphology is poorly described. This project, therefore, evaluated distal iliopsoas tendon morphology and its relationship to pelvic osseous structures. Methods Data on the iliopsoas tendon and osseous morphology were collected from 49 body donors. Linear measurements were collected

An Unusual Presentation of a Left Extra-acetabular Trochanteric Bursa Abscess.

Trochanteric bursitis is a common cause of lateral hip pain, particularly among elderly and postmenopausal females, but abscess formation within the trochanteric bursa is exceedingly rare. We report an unusual extra-acetabular trochanteric abscess identified during cadaveric dissection of an elderly female, characterized by irregular fibrotic and proteinaceous tissue with multiple inclusion bodies but no evidence of malignancy or granulomatous inflammation. Although limited by an absent clinical history, the gross and histopathologic findings suggest a chronic inflammatory or degenerative proc

MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

Driving Safety After Hip Injections: Brake Response Times.

AIMS: Hip inflammatory conditions such as trochanteric bursitis and osteoarthritis significantly impact functional ability. While hip arthroplasty is common, hip injections are a less invasive pain management option for these conditions. Concerns about driving safety post-injection have led to guidelines advising against driving for 12-24 hours. This study assesses brake response time (BRT) before and after hip injections to determine driving fitness. METHODS: In this prospective study, 55 patients scheduled for steroid and local anaesthetic hip injections at a National Orthopaedic Hospital w

Efficacy of Pericapsular Platelet-Rich Plasma Injection Combined With Brachial Plexus Block-Assisted Manipulation in Adhesive Capsulitis: A Randomized Controlled Trial.

Adhesive capsulitis causes severe shoulder pain and functional limitation, and single treatment modalities often fail to prevent relapse after manipulation under anesthesia. In this randomized controlled trial, we examined whether adding ultrasound-guided multitarget platelet-rich plasma injections to manipulation under brachial plexus block could improve pain, function, and health-related quality of life. We enrolled 68 adults with adhesive capsulitis confirmed by magnetic resonance imaging and randomly assigned them to manipulation plus standardized rehabilitation or to the same regimen plus

Fungal Prepatellar Bursitis in an Immunosuppressed Kidney Transplant Recipient: A Rare Case of Phaeohyphomycosis.

INTRODUCTION: Fungal musculoskeletal infections are rare and often present diagnostic and therapeutic challenges, particularly in immunocompromised individuals. Phaeohyphomycosis is a dematiaceous fungal infection with a wide clinical spectrum, but isolated prepatellar bursitis due to this organism has not been previously reported. This case highlights the importance of maintaining a high index of suspicion for unusual pathogens in transplant recipients presenting with atypical joint or periarticular infections. CASE REPORT: We report the case of a 55-year-old female renal transplant recipien

Arthroscopic Treatment of a Large Traumatic Morel-Lavallée Lesion of the Knee in a Young Female Athlete: A Case Report.

INTRODUCTION: Morel-Lavallée lesions (MLLs) are uncommon soft-tissue injuries that can be easily mistaken for other conditions, especially around the knee. CASE REPORT: We present the case of an 18-year-old female athlete who developed a large collection of fluid around the knee after a direct blow to the leg during a wrestling competition. The injury first appeared to be a simple prepatellar bursitis but was later found to be a large fluid collection on magnetic resonance imaging, which measured 9.0 × 2.2 × 18.1 cm and extended from the distal quadriceps to below the tibial tubercle. The pat

Retrospective analysis of guideline-based massage therapy in primary care for musculoskeletal disorders.

BACKGROUND: Musculoskeletal disorders (MSDs) impose a significant burden on primary healthcare systems and the economy. Massage therapy (MT) may represent a useful tool for addressing these issues. However, the lack of robust evidence confirming its effectiveness makes this therapy controversial. OBJECTIVES: This study aimed to assess the feasibility of implementing guideline-based MT into routine primary care practice. MATERIAL AND METHODS: In this retrospective study, records of 258 primary care patients (median (Me) = 51.5 years; 1st and 3rd quartiles (Q1-Q3) = 42-66) were analyzed. These

Severe Diverticulitis With Streptococcus anginosus Bacteremia Complicated by Native Hip Septic Arthritis and Subacromial Bursitis.

A 38-year-old man was admitted for severe diverticulitis complicated by Streptococcus anginosus bacteremia. A CT of the abdomen and pelvis on admission was significant for the additional finding of several hepatic abscesses and a small left hip effusion. On examination, the passive and active range of motion of his left hip and shoulder was limited and painful. A hepatic abscess drain was placed, with aspirate cultures yielding Streptococcus anginosus. Additionally, a left hip arthrocentesis was performed with cultures yielding Streptococcus anginosus. A left shoulder arthrocentesis did not yi

Brodie's Abscess in a Pediatric Patient: A Diagnostic Pitfall.

Brodie's abscess is a localized form of subacute osteomyelitis that often presents without systemic illness and may be misdiagnosed due to its indolent course. We describe a pediatric patient in whom Brodie's abscess initially mimicked a less severe, superficial condition, resulting in delayed definitive diagnosis. The patient was initially treated for presumed septic infrapatellar bursitis, with transient clinical improvement under antibiotic therapy before imaging revealed proximal tibial osteomyelitis consistent with a Brodie's abscess. This case highlights the risk of misdiagnosis in child
