Pediatric Patellar Injuries.

BACKGROUND: Pediatric patellar injuries are common causes of anterior knee pain in children and adolescents and are frequently encountered in emergency and acute care settings. These conditions range from load-related overuse syndromes to acute traumatic injuries requiring timely recognition and referral. Unique features of the skeletally immature knee, including evolving ossification centers, open physes, and cartilaginous structures, may complicate diagnosis and contribute to missed or delayed recognition. OBJECTIVE: This review provides an evidence-informed, emergency medicine-oriented app

Primary Cutaneous Aspergillosis in Disguise: A Case Report with Diagnostic Insights from Cytology and Literature Review.

INTRODUCTION: Primary Cutaneous Aspergillosis (PCA) is a rare localized fungal infection resulting from direct skin inoculation of Aspergillus species, often affecting immunocompetent indi-viduals following minor trauma. Unlike systemic forms, which are predominant in immunocompro-mised hosts, PCA frequently mimics benign subcutaneous lesions, leading to diagnostic delays and mismanagement.</p> Case presentation: A total of 3 cases of adult males (aged 55-70 years) were reported: two immuno-competent and one with controlled type 2 diabetes. Lesions presented as painless, mobile subcutane-ous s

Effectiveness of ultrasonography-guided pericapsular nerve group block in patients with adhesive capsulitis resistant to conservative treatment: A prospective observational study.

This prospective observational study aimed to evaluate the effectiveness of ultrasonography-guided pericapsular nerve group block (PENG-b) in shoulder pain, function, and kinesiophobia in patients with adhesive capsulitis (AC) resistant to conservative treatment. Sixty-one patients diagnosed with resistant AC were included in this study. Thirty-one patients received PENG-b followed by mobilization (Group 1), while 30 received only mobilization therapy (Group 2). All patients underwent mobilization therapy thrice a week for 6 weeks, targeting ≥80% shoulder range of motion (ROM). Pain was assess

The Role of Musculoskeletal Ultrasound in Psoriatic Arthritis: From Preclinical Detection to Treatment Monitoring.

PURPOSE OF REVIEW: Psoriatic arthritis (PsA) is a chronic, immune-mediated inflammatory disease with highly heterogeneous clinical manifestations associated with psoriasis (PsO). The wide variability in presentation, together with the absence of definitive serological biomarkers, makes early diagnosis particularly challenging. This review evaluates the role of ultrasound in identifying and characterising musculoskeletal involvement across the psoriatic disease (PsD) continuum-from asymptomatic PsO to established PsA. RECENT FINDINGS: Ultrasound can detect subclinical synovitis, enthesitis, pe

Diagnostic accuracy of musculoskeletal ultrasound in suspected polymyalgia rheumatica: a multicentre, cohort study.

BACKGROUND: Diagnosing polymyalgia rheumatica is challenging, and musculoskeletal ultrasound is primarily used in diagnostically difficult situations. Previous musculoskeletal ultrasound studies in patients with polymyalgia rheumatica used case-control designs or non-representative cohorts, reducing assessment of diagnostic accuracy. This study aimed to assess the diagnostic accuracy of musculoskeletal ultrasound in two independent cohorts of patients with suspected polymyalgia rheumatica. METHODS: This multicentre, diagnostic accuracy study included consecutive, glucocorticoid-naive patients

Greater Trochanteric Pain Syndrome in Patients with Degenerative Lumbar Spine Disease: Clinical Findings and Short-Term Outcomes in a Retrospective Cohort Study.

Background: Greater trochanteric pain syndrome (GTPS) may coexist with lumbar degenerative spine disease (LDSD) and complicate pain attribution. Methods: This retrospective cohort study evaluated the observed frequency and short-term clinical course of GTPS in a selected tertiary-care LDSD population and explored factors associated with a previous surgical recommendation. Among 153 consecutive eligible patients with clinically compatible and MRI-documented LDSD, 63 had clinical suspicion of GTPS; all 63 underwent hip MRI, demonstrated findings consistent with trochanteric bursitis, received ul

Efficacy and safety of relacorilant for the treatment of patients with Cushing's syndrome (GRACE): a multicentre, phase 3, double-blind, placebo-controlled, randomised-withdrawal study.

BACKGROUND: Relacorilant is a selective glucocorticoid receptor modulator designed to reduce excess cortisol activity by competing with cortisol for glucocorticoid receptor binding, mitigating the clinical manifestations of endogenous hypercortisolism (Cushing's syndrome). The aim of this study was to assess the efficacy and safety of relacorilant in adults with endogenous hypercortisolism. METHODS: This multicentre, phase 3, double-blind, placebo-controlled, randomised-withdrawal study enrolled adults with endogenous hypercortisolism and hypertension, hyperglycaemia, or both and was conducte

Variability of clinical presentation and diagnostic challenges in osteoarticular sporotrichosis: a case series.

Osteoarticular sporotrichosis is the most common extracutaneous type of the disease and may occur either concomitantly with cutaneous lesions or as isolated musculoskeletal disease, the latter frequently resulting in delayed diagnosis. We describe five confirmed cases of osteoarticular sporotrichosis diagnosed between 2002 and 2023 at a university hospital in Brazil. Diagnosis was confirmed by fungal culture, with serology and histopathology used as complementary methods. Clinical and epidemiological data, radiologic findings, treatment, and outcomes were analyzed. Patients were 39 to 67 years

Growth performance, carcass traits, and meat quality of Awassi and Cukurova Meat Sheep lambs.

Claw lesions in dairy cattle pose a significant risk to dairy farmers worldwide in terms of animal welfare concerns and economic profitability. However, the use of different data sources, classification systems, and definitions of reference groups limits the comparison across herds and decreases the usability of the recordings for phenotypic and genetic analyses. In South Africa, information on claw lesions is not routinely collected by dairy farmers and data are limited to hoof trimmers recording lesions during preventative trimming or as needed by producers. Records of the most common claw l

Platelet-Rich Plasma Injections for Refractory Greater Trochanteric Bursitis: A Three-Patient Case Series.

BACKGROUND: Greater trochanteric bursitis is a common cause of lateral hip pain and is frequently considered within the broader clinical spectrum of greater trochanteric pain syndrome. Initial treatment commonly includes activity modification, physical therapy, a home exercise program, nonsteroidal anti-inflammatory drugs, and other nonopioid medications. Corticosteroid injections may provide meaningful relief, although their duration of benefit can be limited. Platelet-rich plasma is an autologous biologic injectate that can be considered as an alternative treatment for greater trochanteric p

Gluteal tendinopathy masterclass: Refuting the myths and engaging with the evidence.

INTRODUCTION: Gluteal tendinopathy is a prevalent and impactful condition. For many decades clinical practice has been guided by 'myths' that developed around clinical beliefs. It's time to examine these myths and engage with the substantial evidence base. PURPOSE: This masterclass aims to: (i) engage with the evidence to refute the most common myths associated with gluteal tendinopathy and trochanteric pain, and (ii) provide clinicians with guidance around contemporary, evidence-based clinical practice. IMPLICATIONS: It is time to put aside traditional beliefs that focused on anti-inflammat
