Effect of radial extracorporeal shockwave therapy combined with evidence-based physical therapy for adhesive capsulitis of the shoulder: A randomized controlled trial.

OBJECTIVES: To evaluate the effect of radial extracorporeal shockwave therapy (rESWT) combined with evidence-based physical therapy (EBPT) on adhesive capsulitis (AC). METHODS: A total of 30 participants with AC were randomly assigned to receive either rESWT combined with EBPT (experimental group) or sham rESWT combined with EBPT (control group) over a 6-week intervention period. The primary outcomes included pain (assessed using the visual analogue scale) and disability (disabilities of the arm, shoulder, and hand questionnaire). The secondary outcomes were shoulder range of motion (ROM), ps

Is disability associated with range of motion, pain and mental health-related quality of life in subjects with frozen shoulder? A cross-sectional study.

INTRODUCTION: While the impact of physical impairments on disability is widely acknowledged, recent literature suggests an association between health-related, social, and quality of life aspects, mental distress and unfavorable outcomes in subjects with Frozen Shoulder (FS). OBJECTIVE: To identify the main contributors to disability among external rotation (ER0°) and flexion (FL) passive Range of Motion (ROM) restrictions, pain and mental health-related quality of life domains in subjects with FS. METHODS: The association of the Disability of the Arm, Hand, and Shoulder (DASH) scale (depende

Closing Wedge Trochanteric Reduction Osteotomy for Treatment of Trochanteric Bursitis: A Case Report.

CASE: A 41-year-old patient presented with chronic, left-sided trochanteric bursitis, unresponsive to conservative treatments including intensive physiotherapy, local and systemic anti-inflammatory therapy, and neuromodulation. A novel surgical approach was used, involving an adducting trochanteric closing wedge osteotomy to reduce the lateral prominence of the greater trochanter without relevantly affecting the abductor lever arm. The procedure resulted in substantial symptom relief, with excellent functional outcomes. CONCLUSION: This case underscores the potential of the adducting trochant

Delayed Tricep Repair Using Dermal Allograft: Technique and Case Examples.

An acellular dermal matrix is a biological graft composed of several components present in the dermis such as collagen fibers, elastin, fibronectin, and hyaluronic acid that serve as support for cellular repopulation that will gradually become vascularized. This provides mechanical resistance and improves suture retention. With this article we aim to present 2 cases, and our technique, in which a rupture of the distal triceps tendon occurred, patients presented late due to usual insurance authorization delays, and then required delayed surgical repair by means of dermal acellular matrix grafti

Impact of hemodynamics in individual shoulder structures on pain intensity in patients with rotator cuff tear.

BACKGROUND: Rotator cuff tears (RCTs) commonly cause shoulder pain and functional impairment in middle-aged and elderly individuals. Despite their varying clinical presentations, the mechanisms underlying pain development and exacerbation in RCT remain poorly understood. The relationship between increased blood flow around the shoulder joint and pain has recently attracted increasing attention. Dynamic magnetic resonance imaging (MRI) has been established as an imaging study for assessing local hemodynamics in various fields, and we utilized this technique to evaluate hemodynamic changes in th

Mycobacterium kansasii chronic olecranon bursitis: A rare case report and literature review.

Non-tuberculous mycobacteria are a rare cause of olecranon bursitis. We present a case of a 65-year-old man with history of chronic olecranon bursitis status post bursectomy two years prior, presented with two weeks of right elbow swelling but no pain or redness. On exam, bursa was enlarged with mild warmth present but no concern for elbow joint involvement. The bursa was aspirated, fluid analysis revealed leukocytosis with monosodium urate crystals consistent with gout. Ten days later, the mycobacterial cultures grew Mycobacterium kansasii. Two weeks later, on repeat aspiration of right elbow

Intra-articular corticosteroid injection vs. suprascapular nerve block for adhesive capsulitis: a systematic review and meta-analysis of level I randomized controlled trials.

BACKGROUND: Intra-articular corticosteroid injection and suprascapular nerve block are both options for nonoperative management of adhesive capsulitis. While numerous studies support the benefits of steroid injections, the use of suprascapular nerve blocks is less established. Published randomized trials comparing steroid injections and suprascapular nerve blocks for managing adhesive capsulitis have reported mixed results. This study aimed to perform a systematic review and meta-analysis of published level I studies to compare outcomes for patients with adhesive capsulitis following intra-art

Black Hairy Tongue Associated With Linezolid Treatment for Olecranon Bursitis Due to Streptococcus salivarius and Streptococcus mitis.

A 63-year-old female presented with acute, atraumatic posterior elbow pain. She was found to have infectious olecranon bursitis with resistant bacteria. Her treatment was complicated by a penicillin allergy, necessitating the use of linezolid, which was further complicated by the rare black hairy tongue (BHT) side effect of linezolid. Despite the visible discoloration and associated pain from BHT, the patient continued linezolid to treat her bursal infection. She recovered from the bursitis and linezolid side effects after completion, supporting that immediate withdrawal is not necessary for t

First reported case of phaeohyphomycotic bursitis due to Paraconiothyrium estuarinum.

A 51-year-old male presented with right knee swelling with persistent pain for the past two weeks. Radiograph of the knee showed no bone abnormalities. Bursectomy was performed and the aspirated fluid was sent for laboratory analysis. Gram staining revealed fungal filaments, and culture on Sabouraud's dextrose agar yielded blackish-brown pigmented fungal colonies. Histopathological examination showed slender, septate hyphae. PCR sequencing identified Paraconiothyrium estuarinum as the causative agent. The patient was treated with itraconazole for 3 months with regular follow-up, resulting in c

Tendon Sheath, Bursae, and Clinically Relevant Peritendinous Structures of the Hand and Wrist.

Imaging is essential for evaluating tenosynovitis, bursitis, and pulley lesions. Both ultrasound (US) and MR imaging detect fluid accumulation and synovial thickening, key for diagnosing tenosynovitis and bursitis. They also reveal A1 pulley thickening in trigger fingers and A2-A4 pulley strain or tears in closed flexor pulley injuries (climber's fingers). US enables dynamic assessments in real-time and detects hypervascularity, while MR imaging provides detailed insights into the extent of inflammation in the soft tissues and bone through fluid-sensitive sequences and contrast enhancement. Isolated Chronic Tophaceous Gout of the Elbow: A Rare Presentation of Olecranon Bursopathy Requiring Surgical Excision.

Tophaceous gout is a chronic sequela of hyperuricemia, typically and predominantly affecting peripheral joints. However, isolated elbow involvement of the olecranon with a large tophaceous ulcer is uncommon. We report a rare case of chronic tophaceous olecranon bursopathy in a 48-year-old male patient with poorly controlled hyperuricemia, highlighting the diagnostic, imaging, and surgical aspects of this condition. The histopathological examination confirmed the presence of urate crystal tophi devoid of any malignant features.

Comparison of findings on contrast-enhanced MRI of the hand, wrist, and forefoot in healthy controls, two at-risk groups, and patients with rheumatoid arthritis: a cohort study.

BACKGROUND: The sensitivity of MRI in detecting joint inflammation in rheumatoid arthritis is well known but its specificity is less discussed. It is important to prevent false positive results and consequent overdiagnosis. Therefore, we aimed to examine MRI-detected inflammation that is less specific for rheumatoid arthritis by evaluating the frequencies of inflammation in healthy controls and in two at-risk groups who have not developed rheumatoid arthritis, compared with patients with rheumatoid arthritis. METHODS: In this cohort study, we performed contrast-enhanced MRIs of the second to

Distal Hamstring Injuries and Disorders.

Hamstring injuries are a common injury sustained by athletes with most injuries occurring as strain injuries within the muscle belly or at the proximal musculotendinous junction. Distal hamstring pathology is relatively uncommon but comprises a collection of both acute and chronic diagnoses that can manifest with symptoms either on the medial or lateral side of the knee based on which hamstring tendons are involved. Pes anserinus bursitis is the most common of these distal hamstring pathologies with other chronic diagnoses, including snapping medial hamstrings or snapping biceps femoris. Acute

The limited impact of randomized controlled trials on the management of greater trochanteric pain syndrome as demonstrated by fragility Indices: A citation analysis.

IMPORTANCE: Greater trochanteric pain syndrome (GTPS) is a chronic condition commonly defined as pain in the lateral hip joint that can be severely limiting to activities of daily living, sleep, and overall quality of life. It encompasses numerous disease states causing pain in the region of the greater trochanter of the femur. Noninvasive management approaches for GTPS include corticosteroid injections and physiotherapy, preferred as first-line interventions prior to surgical interventions. Understanding the impact of randomized control trials (RCTs) can enhance the understanding of treatment

Mumbai shoulder score: validation of a new patient-reported shoulder outcome score for evaluating shoulder pain and disability.

BACKGROUND: The criticisms of the current patient-reported outcome measure (PROM) scores are that they are laborious, may be difficult for the patients to understand, and some may require a license for their use. Therefore, the purpose of the study was to validate a new PROM scoring system. METHODS: A new shoulder scoring questionnaire was developed after interviewing 50 patients and refined after testing in 20 additional patients. The new score, "Mumbai shoulder score (MSS)," consisted of 10 questions with 4 response options. The score evaluates shoulder pain and difficulties in tasks such a
