Early arthritis signs and symptoms revisited-is synovitis what we are assessing?

Classic signs of joint inflammation at physical examination of patients with rheumatoid arthritis (RA) are joint swelling and joint tenderness. In the hand, additional characteristic signs include a positive squeeze test, reduced grip strength, and incomplete fist closure. Typical symptoms that are presumed to be inflammatory are pain and morning stiffness. Traditionally, these signs and symptoms have been thought to reflect underlying synovitis, ie, inflammation of the intra-articular synovial lining. Over the past 15 years, however, we have recognised the involvement of other inflamed tissue

Comparison of the analgesic efficacy of intra-articular steroid injections and its combination with suprascapular nerve block for adhesive capsulitis of the shoulder joint: a randomized clinical trial.

BACKGROUND: Adhesive capsulitis is a distressing ailment that progressively limits the active and passive mobility of the shoulder joint. Physical therapy (PT) combined with intra-articular steroid (IAS) injection and suprascapular nerve block (SSNB) has shown improved functional outcomes. We aimed to assess whether providing both IAS injection and SSNB improved outcomes compared with IAS injection alone. METHODS: Ninety-six patients with adhesive capsulitis were randomized into three groups consisting of group IAS, group IAS+SSNB and group PT (control). All patients were assessed on day 0, w

Work-related physical strain as novel risk factor for the severity of joint inflammation at diagnosis of anti-citrullinated protein antibodies-positive rheumatoid arthritis.

OBJECTIVES: Increased work-related physical strain associates with higher subclinical joint inflammation in the RA at-risk phase of clinically suspect arthralgia and with progression to RA. Based on this, we hypothesized that work-related physical strain is also associated with the severity of joint inflammation at RA diagnosis. We studied this in ACPA-positive and ACPA-negative RA separately, because of differences in aetiopathology. METHODS: A total of 197 ACPA-positive and 220 ACPA-negative consecutively diagnosed RA patients reported occupation. Physical strain was quantified as exposure

Three-dimensional analysis of shoulder hand-behind-back motion in patients with frozen shoulder.

BACKGROUND: Hand-behind-back (HBB) motion is commonly used to assess shoulder internal rotation; however, it involves multiple joint movements, including scapulothoracic, glenohumeral, and elbow joint motions, thus complicating the interpretation of the HBB position. This study aimed to investigate 3-dimensional joint alignment during HBB motion of the bilateral shoulders in patients with frozen shoulder and to identify which joint motions primarily contribute to motion limitation. METHODS: Seventeen patients with unilateral frozen shoulder underwent bilateral shoulder computed tomography sca

Delayed Radial Nerve Palsy Following a Low-Impact Mechanical Fall Managed With Early Ultrasound-Guided Hydrodissection: A Case Report.

A 43-year-old male presented to the emergency department due to progressive weakness of the left hand and wrist following blunt trauma to the elbow around three weeks prior to evaluation. He was initially evaluated at the time of injury with radiographs of the elbow and forearm, which were reported to be normal. Due to worsening weakness, he sought care at additional emergency departments and with a neurologist who performed a CT, repeat radiographs, and eventually an MRI of the elbow, all of which revealed no acute findings. At our facility, point-of-care ultrasound evaluation of the elbow de

A Case of Aortic Mural Thrombus Presenting as Recurrent Shoulder Pain Discovered by the Presence of Splinter Hemorrhages.

Progressive claudication of the upper extremity is uncommon and can be misattributed to cervical radiculopathy or even shoulder pathology, such as tendonitis or bursitis. We report a patient presenting to the emergency department with several months of worsening left upper-extremity pain initially managed as suspected radiculopathy, but, in retrospect, was upper-extremity claudication. This was discovered through the presence of lesions beneath the fingernails of the left hand, splinter hemorrhages. Adding reciprocal inhibition to conventional physiotherapy for adhesive capsulitis of the shoulder in Bangladesh: a study protocol for a randomised controlled trial evaluating pain, function and range of motion.

INTRODUCTION: Adhesive capsulitis (frozen shoulder) is a common condition characterised by pain, stiffness and reduced function. While conventional physiotherapy (typically comprising joint mobilisation, stretching, strengthening and electrotherapy) is widely used, there is limited evidence for the effectiveness of advanced neuromuscular techniques such as the reciprocal inhibition technique (RIT). This study aims to evaluate the impact of incorporating RIT into standard physiotherapy compared with standard physiotherapy alone in individuals with adhesive capsulitis. METHODS AND ANALYSIS: Thi

Individualized Deltoid Landmark and Needle Length for Safe Intramuscular Vaccination in Southeast Asian Adults: An Ultrasound Study.

Background/Objectives: An incorrect intradeltoid injection technique can cause shoulder injury related to vaccine administration, including bursitis, septic arthritis, and axillary nerve injury, particularly when Western-derived landmarks and needle-length tables are applied to smaller-framed Southeast Asian adults. We aimed to define an individualized deltoid injection landmark and needle length that avoid the axillary nerve while ensuring reliable intramuscular delivery in Southeast Asian adults. Methods: In this cross-sectional ultrasound study of adults aged ≥18 years, four vertical landma

Neurotized Free Anterolateral Thigh Flap for Large Tissue Defect and Joint Exposure in the Setting of Olecranon Bursitis: A Case Report.

Elbow wounds secondary to septic olecranon bursitis are a challenging problem to treat. Neurotized fasciocutaneous free anterolateral thigh (ALT) flaps provide a reliable soft tissue coverage option with the opportunity to provide protective sensation to a common pressure point. Free flaps can provide coverage when alternative options lower on the reconstructive ladder are unavailable. A 69-year-old man with rheumatoid arthritis presented with septic olecranon bursitis requiring serial debridement resulting in significant soft tissue loss and bone exposure. MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

Efficacy of Pericapsular Platelet-Rich Plasma Injection Combined With Brachial Plexus Block-Assisted Manipulation in Adhesive Capsulitis: A Randomized Controlled Trial.

Adhesive capsulitis causes severe shoulder pain and functional limitation, and single treatment modalities often fail to prevent relapse after manipulation under anesthesia. In this randomized controlled trial, we examined whether adding ultrasound-guided multitarget platelet-rich plasma injections to manipulation under brachial plexus block could improve pain, function, and health-related quality of life. We enrolled 68 adults with adhesive capsulitis confirmed by magnetic resonance imaging and randomly assigned them to manipulation plus standardized rehabilitation or to the same regimen plus
