The Role of Musculoskeletal Ultrasound in Psoriatic Arthritis: From Preclinical Detection to Treatment Monitoring.

PURPOSE OF REVIEW: Psoriatic arthritis (PsA) is a chronic, immune-mediated inflammatory disease with highly heterogeneous clinical manifestations associated with psoriasis (PsO). The wide variability in presentation, together with the absence of definitive serological biomarkers, makes early diagnosis particularly challenging. This review evaluates the role of ultrasound in identifying and characterising musculoskeletal involvement across the psoriatic disease (PsD) continuum-from asymptomatic PsO to established PsA. RECENT FINDINGS: Ultrasound can detect subclinical synovitis, enthesitis, pe

Delayed Tricep Repair Using Dermal Allograft: Technique and Case Examples.

An acellular dermal matrix is a biological graft composed of several components present in the dermis such as collagen fibers, elastin, fibronectin, and hyaluronic acid that serve as support for cellular repopulation that will gradually become vascularized. This provides mechanical resistance and improves suture retention. With this article we aim to present 2 cases, and our technique, in which a rupture of the distal triceps tendon occurred, patients presented late due to usual insurance authorization delays, and then required delayed surgical repair by means of dermal acellular matrix grafti

Tendon Sheath, Bursae, and Clinically Relevant Peritendinous Structures of the Hand and Wrist.

Imaging is essential for evaluating tenosynovitis, bursitis, and pulley lesions. Both ultrasound (US) and MR imaging detect fluid accumulation and synovial thickening, key for diagnosing tenosynovitis and bursitis. They also reveal A1 pulley thickening in trigger fingers and A2-A4 pulley strain or tears in closed flexor pulley injuries (climber's fingers). US enables dynamic assessments in real-time and detects hypervascularity, while MR imaging provides detailed insights into the extent of inflammation in the soft tissues and bone through fluid-sensitive sequences and contrast enhancement. Comparison of findings on contrast-enhanced MRI of the hand, wrist, and forefoot in healthy controls, two at-risk groups, and patients with rheumatoid arthritis: a cohort study.

BACKGROUND: The sensitivity of MRI in detecting joint inflammation in rheumatoid arthritis is well known but its specificity is less discussed. It is important to prevent false positive results and consequent overdiagnosis. Therefore, we aimed to examine MRI-detected inflammation that is less specific for rheumatoid arthritis by evaluating the frequencies of inflammation in healthy controls and in two at-risk groups who have not developed rheumatoid arthritis, compared with patients with rheumatoid arthritis. METHODS: In this cohort study, we performed contrast-enhanced MRIs of the second to

Mumbai shoulder score: validation of a new patient-reported shoulder outcome score for evaluating shoulder pain and disability.

BACKGROUND: The criticisms of the current patient-reported outcome measure (PROM) scores are that they are laborious, may be difficult for the patients to understand, and some may require a license for their use. Therefore, the purpose of the study was to validate a new PROM scoring system. METHODS: A new shoulder scoring questionnaire was developed after interviewing 50 patients and refined after testing in 20 additional patients. The new score, "Mumbai shoulder score (MSS)," consisted of 10 questions with 4 response options. The score evaluates shoulder pain and difficulties in tasks such a

Effect of Eccentric Control Exercises on Patients with Frozen Shoulder and Mild to Moderate Disability: A Single-Group Pre-Post Study.

BACKGROUND: Frozen shoulder (FS) is a common musculoskeletal condition characterized by inflammatory contracture of the glenohumeral joint capsule, leading to restricted active and passive range of motion, particularly in external rotation. Eccentric control exercises have demonstrated effectiveness in managing various upper limb disorders, including subacromial impingement, tennis elbow, and rotator cuff tendinopathy. However, there is limited evidence on their efficacy in individuals with frozen shoulder. This study aimed to evaluate the effects of eccentric control exercises on pain, functi
