Septic and non-septic olecranon bursitis in the accident and emergency department--an approach to management.

Olecranon bursitis is relatively common. One third of episodes are septic. Most of the remainder are non-septic, with occasional rheumatological causes. Trauma can cause both septic and non-septic olecranon bursitis. Clinical features are helpful in separating septic from non-septic olecranon bursitis, but there may be local erythema in both. Aspiration should be carried out in all cases, and if the presence of infection is still in doubt, microscopy, Gram staining, and culture of the aspirate will resolve the issue. Septic olecranon bursitis should be treated by aspiration, which may need to

Staphylococcus aureus triggered reactive arthritis.

OBJECTIVES: To report two patients who developed reactive arthritis in association with Staphylococcus aureus infection. METHODS: A review of the case notes of two patients. RESULTS: Two adult female patients have developed sterile arthritis in association with Staph aureus infection. The first patient has had two episodes of arthritis; the first followed olecranon bursitis, the second followed infection of a central venous catheter used for dialysis. The second patient developed sterile arthritis while being treated for pyomyositis. Both patients had a self limited arthritis and were HLA-B2

Treatment of olecranon bursitis in horses: 10 cases (1986-1993).

Medical records of 10 horses with olecranon bursitis were reviewed to examine treatments, evaluate a technique for en bloc resection of the bursa in standing horses, and determine outcome of the horses after treatment. Before admission, 6 horses had been treated by needle aspiration of fluid from the mass, followed by injection of corticosteroids. Subsequent treatment for 2 of these 6 horses included open drainage and packing of the cavity with gauze soaked in 7% iodine solution. None resolved after these treatments. After admission to the hospital, 5 horses were treated medically and 5 were t

Septic olecranon bursitis: recognition and treatment.

BACKGROUND: The superficial location of the olecranon bursa places it at high risk for injury, possibly leading to the entry of bacteria into the bursal sac. Early differentiation between septic and nonseptic olecranon bursitis is paramount to direct therapy, to hasten recovery, and to prevent chronic inflammation. METHODS: A literature review was performed using MEDLINE files from 1967 to the present. Additional references from the bibliographies of these were also utilized. RESULTS AND CONCLUSIONS: Olecranon bursitis is a common condition that requires the treating physician to be aware of

Fungal Bursitis: Olecranon Bursitis Caused by Candida parapsilosis with Review of the Literature.

Various Candida species have emerged as clinically important pathogens, as documented by increasing incidence of localized Candida infections, candidemia, and systemic candidiasis. Septic bursitis caused by fungi has been described infrequently. We describe a case of septic olecranon bursitis caused by Candida parapsilosis. The patient responded well to antifungal therapy and bursal aspirations.

Mycobacterium asiaticum as the probable causative agent in a case of olecranon bursitis.

Mycobacterium asiaticum was isolated from fluid aspirated from an olecranon bursa that had become inflamed following a superficial injury. Other possible causes of the inflammation were excluded. No specific antimycobacterial therapy was given. The infection responded to drainage, regular dressing, and immobilization. Our experience suggests that M. asiaticum is a potential cause of infection of the joints and surrounding tissues.

Immunocytologic characteristics of mononuclear cell populations found in nonseptic olecranon bursitis.

OBJECTIVE: To determine the immunocytologic characteristics of the various subsets of nonseptic olecranon bursal fluid mononuclear cells. METHODS: Twenty consecutive patients with culture negative olecranon bursitis had immunocytochemical and flow cytometric analysis performed using a panel of monoclonal antibodies to determine lymphocyte and monocyte/macrophage population subtypes and proportions. RESULTS: In traumatic bursitis (n = 9), the mean (+/- SD) white blood cell (WBC) count/mm3 was 1,368 +/- 1,559; WBC mononuclear differential count was 29.5 +/- 19% lymphocytes and 55 +/- 26% monoc

Olecranon and pretibial bursitis in atopic dermatitis: coincidence or association?

BACKGROUND: Bursal fluid accumulations are not usually associated with skin disease; however, several cases have been observed in a large atopic dermatitis (AD) clinic. OBJECTIVE: We sought to characterize the clinical features of bursitis in AD and to assess the evidence for a mechanistic association between these conditions. METHODS: We retrospectively evaluated the clinical presentation of bursitis in six adults with lifelong severe AD. The course of the bursitis and possible causative factors were assessed for each patient. RESULTS: Ten episodes of bursitis were documented in six patien

[Human protothecosis and environment].

Protothecosis are uncommon infections caused by Prototheca, considered to be achlorophylous algae. Nearly 80 human cases are reported in the literature since the first case described by Davies and Wakelin in 1964 in Sierra Leone (11). The disease have been identified in Europe, Asia (Japan, Thailand, China), Oceania and in the United States with 40 cases, particularly in the Southeast. Clinically, three clinical manifestations can be observed: 1) cutaneous lesions: papules, plaques or eczematoid, papulo-nodular areas of the extremities, 2) olecranon bursitis which occurred in 25% of cases, 3)

Rupture of the triceps tendon associated with steroid injections.

Rupture of the triceps mechanism is an uncommon injury that has been recognized with increasing frequency in recent years. It has been proposed that such injuries commonly accompany fractures of the radial head and must be actively evaluated in the presence of such a fracture. We present a unique case of isolated rupture of the triceps tendon in an athlete who was lifting weights. This case was complicated by a history of olecranon bursitis that had been treated with numerous local steroid injections, as well as a history of anabolic steroid abuse. Both systemic steroids and local injections m

Aspergillus terreus as a cause of septic olecranon bursitis.

A 72-year-old, non-insulin-dependent diabetic man with a 2-month history of painful right olecranon bursitis was examined after a fall on the sidewalk that resulted in some abrasion of the skin overlying the elbow. Fluid aspirated from the bursa showed growth of Aspergillus terreus, as did tissue from a bursectomy performed 1 week later. Septic bursitis is an uncommon disease that is nearly always caused by Staphylococcus aureus or hemolytic streptococci. Mycotic bursitis is very rare and this is the first reported instance of any Aspergillus species causing septic bursitis.

[Mycobacterium szulgai infection in a patient with hemophilia and AIDS].

Mycobacterium szulgai is a scotochromogenic species recently recognized as a human pathogen. Twenty-nine cases of disease caused by M. szulgai in humans have been reported. Pulmonary disease indistinguishable from that caused by M. tuberculosis was the commonest type of infection caused by M. szulgai. Olecranon bursitis was reported in 3 cases and disseminated infection was noted in 3 cases occurring in immunocompromised patients without AIDS. The authors report the first case of pulmonary disease caused by M. szulgai in Brazil and the first case in patient with AIDS in the world literature.

Protothecosis complicating prolonged endotracheal intubation: case report and literature review.

Prototheca species are ubiquitous, aerobic, unicellular algae closely related to the green algae Chlorella. Their involvement in human disease--in both immunocompetent and immunocompromised patients--has been reported with increasing frequency. The wide array of presentations has included cutaneous, subcutaneous, mucosal, bursal, catheter-related, and (in rare instances) systemic disease. We report a case of protothecosis complicating prolonged endotracheal intubation presenting as a nasopharyngeal ulceration with a soft-tissue mass, and we review the presentation, treatment, and outcome of th

Cryptococcal olecranon bursitis in cirrhosis.

A 47-year-old man with cirrhosis developed a case of previously unreported olecranon bursitis due to Cryptococcus neoformans. Most patients with disseminated cryptococcosis have deficiencies in cell mediated immunity. Cirrhosis may be an independent risk factor because of impaired chemotaxis and phagocytosis.
