Hemorrhagic subcutaneous bursitis.

Hemarthrosis is a well recognized complication of a number of conditions. Hemorrhagic subcutaneous bursitis is less understood. We encountered a patient with a myeloproliferative disease who developed hemorrhagic olecranon bursitis. Upon reviewing other patients with subcutaneous bursitis, we found that hemorrhagic bursitis also occurs in the setting of traumatic or idiopathic bursitis, rheumatoid arthritis, gout, and septic bursitis.

Septic olecranon bursitis caused by Neisseria sicca.

A man with rheumatoid arthritis developed a septic olecranon bursitis due to Neisseria sicca. This case confirms the potential pathogenicity of Neisseria sicca and emphasizes that positive cultures for this organism should not be readily dismissed.

Septic and nonseptic olecranon bursitis. Utility of the surface temperature probe in the early differentiation of septic and nonseptic cases.

Forty-six consecutive cases of olecranon bursitis were prospectively analyzed during a 1-year period. Eleven cases were septic; 35 cases were nonseptic. In addition to bursal fluid analysis, the surface temperature over the involved olecranon bursa and the contralateral (control) olecranon process was obtained by using a surface temperature probe. We compared the temperature difference between the involved and control sides in all septic and nonseptic cases. In nonseptic cases, the mean surface temperature difference was 0.7 degree C vs 3.7 degrees C in septic cases. In all septic cases, the t

Treatment of nonseptic olecranon bursitis. A controlled, blinded prospective trial.

We enrolled 42 patients with nonseptic olecranon bursitis in a double-blind prospective treatment trial to compare the efficacy of an intrabursal steroid preparation with that of an oral anti-inflammatory agent. Patients were randomized into one of four treatment regimens: (1) methylprednisolone acetate (20 mg) intrabursal injection and oral naproxen (1 g/d for 10 days), (2) methylprednisolone acetate (20 mg) intrabursal injection and oral placebo for 10 days, (3) oral naproxen (1 g/d for 10 days), and (4) oral placebo for 10 days. The degree of swelling in millimeters was assessed at study in

Septic bursitis in human immunodeficiency virus infection.

Septic bursitis is a well described entity in immunocompromised patients. Common factors contributing to an immunocompromised state are alcoholism and corticosteroid therapy. We describe 2 patients with Staphylococcus aureus olecranon bursitis with evidence of human immunodeficiency virus (HIV) infection and no other contributing factors to immunodeficiency. The clinical course in these patients was refractory to conservative management. Such a persistent course of septic bursitis, especially in the absence of other contributing factors, should raise the possibility of HIV infection.

Arthritis in idiopathic hypereosinophilic syndrome.

A 54-year-old man was admitted for tenosynovitis of the wrists, olecranon bursitis, stiffness, progressive flexion of the fingers, subcutaneous nodules, and persistent blood eosinophilia. A high eosinophil count was detected in his synovial fluid. We review the literature on idiopathic hypereosinophilic syndrome. This case provides evidence that nonerosive polyarthritis with periarticular involvement and nodules can be manifestations of this syndrome.

Elbow pain.

Nonarticular causes of elbow pain include muscle strains, ligamentous injuries, epicondylitis, olecranon bursitis, and compressive neuropathies. Overuse and trauma commonly cause these conditions. The history and physical examination differentiate them from an intra-articular process such as synovitis. Laboratory analysis of fluid aspirated from a swollen olecranon bursa is necessary to differentiate infection or gout. X-rays are useful in avulsion fracture, osteochondritis dissecans, and epiphyseal separation. Electromyography with nerve conduction velocities can localize the site of nerve en

Development of the olecranon bursa. An anatomic cadaver study.

Anatomic dissection of the elbows of 63 cadavers selected at random were performed in an attempt to find out whether the incidental disparity of olecranon bursitis between children and adults might be explained by anatomic differences. The volume of the bursae was determined by syringes used for methylene blue injections. There were no olecranon bursae in children under the age of 7 years; the volume of the bursae increased with age; and the bursa was usually larger on the right, i.e., the common dominant side. The formation of the bursae in late childhood can explain the low incidence of olec

Infections caused by Mycobacterium szulgai in humans.

Mycobacterium szulgai is a scotochromogenic species that has recently been recognized as a human pathogen. Twenty-four cases of disease caused by M. szulgai in humans have been reported in the English-language literature. The clinical features of these cases were reviewed, and three additional cases (two pulmonary, one extrapulmonary) were studied. Pulmonary disease indistinguishable from that caused by Mycobacterium tuberculosis was the commonest type of infection caused by M. szulgai (18 of 27 cases). Olecranon bursitis was reported in three cases, and disseminated infection was noted in thr

Protothecal olecranon bursitis: treatment with intrabursal amphotericin B.

A patient is described from whom repeated olecranon bursa fluid cultures yielded Prototheca wickerhamii. Following intrabursal instillation of amphotericin B, the cultures became negative. There was no progression of infection or adverse consequences of this treatment. Review of the literature regarding this unusual infection reveals seven reported cases of protothecal olecranon bursitis. Five cases from which sufficient information was provided were diagnosed by histologic examination of excised bursa that showed the characteristic sporangia.

Predisposing factors, bacteriology and antibiotic therapy in 35 cases of septic bursitis.

In a retrospective study the etiology was verified in 35/52 patients with suspected septic prepatellar or olecranon bursitis. Staphylococcus aureus was the most common pathogen and 86% of them were penicillinase-producing. Other strains were streptococci. Mean age was 47 (18-83) years and males predominated (91%). Predisposing factors were seen in most cases. Preceding trauma was found in 27 and/or associated diseases (e.g. diabetes mellitus) in 11 patients. Hyperglycemia was found in 38% of the patients. Treatment with antimicrobial agents, mostly penicillins, and bursal drainage were success

Arthropathy and cutaneous calcinosis in hemodialysis oxalosis.

Recently, calcium oxalate crystals have been identified in the synovial fluid of patients with arthritis and end-stage renal failure. We describe 4 patients who, during the course of long-term hemodialysis, developed calcium oxalate crystal deposits in the synovium and skin. Clinical manifestations included podagra, tenosynovitis, olecranon bursitis, and acute and chronic synovitis of the large joints that were associated with chondrocalcinosis or subchondral bone erosions. Diffuse involvement of the hand, with chondrocalcinosis of the finger joints, miliary calcified deposits in the skin, and

Treatment of septic olecranon and prepatellar bursitis with percutaneous placement of a suction-irrigation system. A report of 12 cases.

Ten cases of septic olecranon bursitis and two cases of septic prepatellar bursitis were treated in the period from 1975 to 1980 with antibiotics and percutaneous tube placement for suction-drainage and local antibiotic irrigation. All patients had positive bacterial cultures: Staphylococcus aureus in nine, beta-hemolytic Streptococcus in two, and Staphylococcus epidermidis in one. Intravenous antibiotics, local suction-drainage, and irrigation with a solution of 1% kanamycin and 0.1% polymyxin controlled the infection in each case. The antibiotic treatment averaged 19 days, compared with 24 d

Histological study of a case of recurrent olecranon bursitis with mixed calcium pyrophosphate dihydrate and apatite crystal deposits.

This is the report of a light and transmission electron microscopic study of an olecranon bursitis and of the adjacent distal tricipital tendon in an 83 year-old man. The data are compared with those of a similar study in the same patient performed 2 years ago. Calcium pyrophosphate dihydrate crystals were observed in the bursal fluid, in the inner part of the bursal wall (extracellular localization and intracellular phagocytosis) as well as in the peripheral part of the tendon. In addition, small apatite deposits were observed in the bursa and tendon by electron microscopy. The origin of thes

Long-term follow-up of corticosteroid injection for traumatic olecranon bursitis.

Forty-seven patients with traumatic olecranon bursitis were evaluated after a mean follow-up of 31 months (range 6 to 62 months). Twenty-two patients treated with bursal aspiration had delayed recovery and no complications of therapy. Twenty-five patients treated with intrabursal injection of 20 mg of triamcinolone hexacetonide had rapid recovery, usually within one week, but suffered complications such as infection (3 cases), skin atrophy (5 cases), and chronic local pain (7 cases). Since spontaneous resolution can be expected, a conservative approach is suggested in the treatment of traumati

Septic bursitis in systemic lupus erythematosus.

Septic olecranon bursitis developed in three patients with systemic lupus erythematosus. Although promptly treated with systemic antibiotics, the clinical course in two patients became prolonged and eventually required bursectomy to eradicate the infection. The occurrence of septic bursitis was a manifestation of the general susceptibility of SLE patients to infections.

Rupture of the triceps tendon with olecranon bursitis. A case report with a new method of repair.

Rupture of the triceps tendon is rare, and no previous report of its association with olecranon bursitis was found in the literature. A previously healthy 72-year-old man fell from a stationary bicycle and was examined by his family physician. Calcification over the olecranon area with an intact triceps tendon was revealed. Two months later the patient presented with triceps rupture and weakness of elbow extension with olecranon bursitis. Grossly, the pathologic lesion consisted of synovial frond proliferation and invasion of the cut end of a tendon. A "collar stud-shaped" bursa was found in f
