Glomus tumour of the elbow: a case of mistaken identity.

Glomus tumours are small vascular tumours that are usually benign and rarely occur. However, atypical locations of these tumours can lead to misdiagnosis, particularly where the lesion is rarely reported. We report a case of glomus tumour located on the posterior aspect of the elbow, which was mistaken for, and treated as the commonly diagnosed, olecranon bursitis for several years. With worsening symptoms, mainly pain, pinpoint tenderness and hypersensitivity, the patient was referred for a further opinion, and the lesion was diagnosed as a glomus tumour following complete excision under anae

Evaluation of elbow pain in adults.

The elbow is a complex joint designed to withstand a wide range of dynamic exertional forces. The location and quality of elbow pain can generally localize the injury to one of the four anatomic regions: anterior, medial, lateral, or posterior. The history should include questions about the onset of pain, what the patient was doing when the pain started, and the type and frequency of athletic and occupational activities. Lateral and medial epicondylitis are two of the more common diagnoses and often occur as a result of occupational activities. Patients have pain and tenderness over the affect

Endoscopic repair of tears of the superficial layer of the distal triceps tendon.

PURPOSE: The purpose of this study was to evaluate the results after endoscopic repair of partial superficial layer triceps tendon tears. METHODS: Fourteen patients treated surgically between July 2005 and December 2012 were studied prospectively for 12 months. Indication for surgery was a partial detachment of the triceps tendon from the olecranon that was proved by magnetic resonance imaging (MRI) in all cases. Ten of these patients had chronic olecranon bursitis. All patients were treated with endoscopic surgery including bursectomy and repair of the distal triceps tendon with double-loade

Protothecosis in hematopoietic stem cell transplantation: case report and review of previous cases.

Prototheca species are achlorophyllus algae. Prototheca wickerhamii and Prototheca zopfii cause human disease. In immunocompetent individuals, they cause soft tissue infections and olecranon bursitis, but in transplant recipients, these organisms can cause disseminated disease. We report a fatal case of disseminated P. zopfii infection in an hematopoietic stem cell transplant (HSCT) recipient with bloodstream infection and involvement of multiple soft tissue sites. We review all previous cases of protothecosis in HSCT reported in the literature. Protothecosis is uncommon after HSCT, but has a

Surgical Management of Symptomatic Olecranon Traction Spurs.

BACKGROUND: There is a paucity of information pertaining to the pathoanatomy and treatment of symptomatic olecranon traction spurs. PURPOSE: To describe the pathoanatomy of olecranon traction spur formation, a technique for spur resection, and a series of patients who failed conservative care and underwent operative treatment. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Eleven patients (12 elbows) with a mean age of 42 years (range, 27-62 years) underwent excision of a painful olecranon traction spur after failing conservative care. Charts and imaging studies were reviewed. Al

Olecranon bursitis: a systematic overview.

BACKGROUND: Olecranon bursitis is a common condition where the bursal cavity, superficial to the olecranon, becomes inflamed. This can occur either with or without infection and has been given pseudonyms relating to the repeated minor trauma from external pressure that often predisposes. As a result of the multiple aetiologies, olecranon bursitis can present to any medical specialty with reasonable frequency and, although many therapies are described, a single, evidence-based and standardized treatment pathway is not well described. METHODS: We summarize the key points within the literature a

Treatment of olecranon bursitis: a systematic review.

INTRODUCTION: The optimal management of olecranon bursitis is ill-defined. The purposes of this review were to systematically evaluate clinical outcomes for aseptic versus septic bursitis, compare surgical versus nonsurgical management, and examine the roles of corticosteroid injection and aspiration in aseptic bursitis. MATERIALS AND METHODS: The English-language literature was searched using PubMed, Cumulative Index to Nursing and Allied Health Literature, Physiotherapy Evidence Database, Allied and Complementary Medicine, and Cochrane Central Register of Controlled Trials. Diagnostic outcomes associated with ankle synovitis in early inflammatory arthritis: a cohort study.

OBJECTIVES: To examine the diagnostic outcomes associated with clinical ankle synovitis in an early inflammatory arthritis cohort. METHODS: Data from the Birmingham early inflammatory arthritis cohort (BEACON) were used to obtain information about baseline disease and demographic variables and diagnostic outcomes at 18 month follow-up. The prevalence of clinical ankle synovitis (defined as joint swelling on examination) was calculated. Relative risk (RR) and 95% confidence interval (95% CI) were used to estimate whether clinical ankle synovitis at baseline predicts diagnostic outcomes indepen

Bilateral Olecranon Bursitis - A Rare Clinical presentation of Calcium Pyrophosphate Crystal Deposition Disease.

INTRODUCTION: Epidural venous plexus enlargement, presenting with low back pain and radiculopathy, is an uncommon cause of nerve roots impingement. This condition commonly mimics a herniated nucleus pulposus radiologically. The radiological diagnosis is often missed and the diagnosis is made during the surgery. We are hereby presenting 2 such cases of epidural varices mimicking intervertebral disc prolapse with lumbar radiculopathy. CASE REPORT: Case 1: 43 yr old female presented with acute exacerbation of low back ache and significant right L5-S1 radiculopathy without neurological deficit. M

Group B streptococcal meningitis in an adult: A possible complication of olecranon bursitis.

BACKGROUND: We report a man with septic olecranon bursitis who had an early development of meningitis. CASE SUMMARY: A 74-year-old man presented to the emergency room with malaise, headache, mental confusion, a fever unsuccessfully treated with oral NSAIDs and ice, and with a 10-day history of pain and swelling in his right elbow. Clinical and laboratory evaluation excluded other causes and microbiological evaluation documented a S. agalactiae infection. Antibiotic treatment induced a rapid improvement, without the development of side effects. CONCLUSION: This is the first report on olecrano

Disseminated protothecosis in a Mexican child.

Protothecosis is a rare human infection. There are 3 clinical forms: cutaneous, olecranon bursitis and systemic. Here we present the first case of disseminated protothecosis in Mexico in an immunocompetent girl whose lesions were mostly dermatologic. The patient was successfully treated with amphotericin B colloidal dispersion and gentamicin, followed by itraconazole.

Olecranon extrabursal endoscopic bursectomy.

Olecranon bursitis is a common clinical problem. It is often managed conservatively because of the high rates of wound complications with the conventional open surgical technique. Conventional olecranon bursoscopy utilizes an arthroscope and an arthroscopic shaver, removing the bursa from inside-out. We describe an extrabursal endoscopic technique where the bursa is not entered but excised in its entirety under endoscopic vision. A satisfactory view is obtained with less morbidity than the open method, while still avoiding a wound over the sensitive point of the olecranon.

Human cutaneous protothecosis: report of a case and literature review.

The Prototheca species is achlorophyllic algae and rarely causes human infection. Human protothecosis presents clinically as a cutaneous infection, olecranon bursitis, and disseminated systemic disease. We report a case of human cutaneous protothecosis involving the left wrist. A 68-year-old man presented with an ill-defined erythematous lesion with crust at the dorsal aspect of his left wrist. A punch biopsy was performed to reveal the histologic features of granulomatous inflammation with necrosis at the upper dermis, containing Prototheca organisms, of which, the characteristic features wer

Candida glabrata olecranon bursitis treated with bursectomy and intravenous caspofungin.

Orthopedic surgeons are becoming more involved in the care of patients with septic arthritis and bursitis caused by yeast species. This case report involves a middle-aged immunocompromised female who developed a Candida glabrata septic olecranon bursitis that developed after she received a corticosteroid injection in the olecranon bursa for presumed aseptic bursitis. Candida (Torulopsis) glabrata is the second most frequently isolated Candida species from the bloodstream in the United States. Increased use of fluconazole and other azole antifungal agents as a prophylactic treatment for recurre

Prepatellar and olecranon bursitis: literature review and development of a treatment algorithm.

PURPOSE: Olecranon bursitis and prepatellar bursitis are common entities, with a minimum annual incidence of 10/100,000, predominantly affecting male patients (80 %) aged 40-60 years. Approximately 1/3 of cases are septic (SB) and 2/3 of cases are non-septic (NSB), with substantial variations in treatment regimens internationally. The aim of the study was the development of a literature review-based treatment algorithm for prepatellar and olecranon bursitis. METHODS: Following a systematic review of Pubmed, the Cochrane Library, textbooks of emergency medicine and surgery, and a manual refere
