INTRODUCTION: Fungal musculoskeletal infections are rare and often present diagnostic and therapeutic challenges, particularly in immunocompromised individuals. Phaeohyphomycosis is a dematiaceous fungal infection with a wide clinical spectrum, but isolated prepatellar bursitis due to this organism has not been previously reported. This case highlights the importance of maintaining a high index of suspicion for unusual pathogens in transplant recipients presenting with atypical joint or periarticular infections.
CASE REPORT: We report the case of a 55-year-old female renal transplant recipien

INTRODUCTION: Morel-Lavallée lesions (MLLs) are uncommon soft-tissue injuries that can be easily mistaken for other conditions, especially around the knee.
CASE REPORT: We present the case of an 18-year-old female athlete who developed a large collection of fluid around the knee after a direct blow to the leg during a wrestling competition. The injury first appeared to be a simple prepatellar bursitis but was later found to be a large fluid collection on magnetic resonance imaging, which measured 9.0 × 2.2 × 18.1 cm and extended from the distal quadriceps to below the tibial tubercle. The pat

BACKGROUND: Pediatric patellar injuries are common causes of anterior knee pain in children and adolescents and are frequently encountered in emergency and acute care settings. These conditions range from load-related overuse syndromes to acute traumatic injuries requiring timely recognition and referral. Unique features of the skeletally immature knee, including evolving ossification centers, open physes, and cartilaginous structures, may complicate diagnosis and contribute to missed or delayed recognition.
OBJECTIVE: This review provides an evidence-informed, emergency medicine-oriented app

Ultrasound imaging of the knee is a highly valuable modality that enhances diagnostic accuracy and optimizes the precision of injection therapy. This article reviews the application of ultrasound in various knee interventions, including intra-articular injections, treatments for prepatellar bursitis and patellar ligament tendinopathy, pes anserinus, iliotibial band procedures, and guidance for popliteal fossa pathologies. Detailed guidance is provided on probe selection, patient positioning, and procedural techniques for specific anatomical targets. Key considerations include optimizing needle

We report a chronic Morel-Lavallée lesion around the knee in a competitive high schooler softball player, successfully treated with video-assisted endoscopic debridement. Endoscopic surgery is a viable option to treat Morel-Lavallée lesions in active patients who seek a rapid return to sport. The potential advantages of an endoscopic treatment would be a faster healing process and an earlier return to motion and function due to a less morbid and more cosmetic surgical approach.

Morel-Lavallée lesions are traumatic abnormalities characterized by the accumulation of hemolymphatic fluid collection following disruption of subcutaneous fat tissue from the underlying deep fascia. Here, we present the case of a 63-year-old woman with a prepatellar Morel-Lavallée lesion, an unusual location for this pathology initially misdiagnosed as prepatellar bursitis. An MRI was performed allowing the correction of the diagnosis, highlighting the essential role of imaging in confirming the diagnosis and ruling out differentials such as prepatellar bursitis or neoplastic origins. CASE: A 47-year-old male with swelling over the patella and associated pain, subsiding with medicines but recurring after a few days. Diagnosed as prepatellar bursitis by the family physician, it eventually turns out to be osteomyelitis of the patella. Treated with curettage, biopsy, and placement of bio-composite mixed with antibiotics. Intra-operative findings and histopathology confirmed tuberculosis of the patella. On follow-up after 6 years and the completion of anti-tuberculosis treatment, the patient showed full functional and radiological recovery without recurrence.
CONCLUSION: Early

PURPOSE OF REVIEW: This review presents evidence for advanced non-operative interventions, including extracorporeal shockwave therapy (ESWT), prolotherapy, platelet-rich plasma (PRP), adipose tissue-derived cells, bone marrow aspirate concentrate, various additional non-corticosteroid injectates, and needle-based interventions for common causes of anterior knee pain in the adult population. These etiologies include osteoarthritis of the knee, patellofemoral pain syndrome, chondromalacia patella, Hoffa fat pad impingement syndrome, patellar/quadriceps tendinopathy, and prepatellar bursitis. Prepatellar bursitis is a common cause of anterior knee pain. We present an unusual case of massive bilateral traumatic haemorrhagic prepatellar bursitis managed with surgical excision and describe our operative findings. The patient presented with large bilateral knee swellings which had been present for six years following a fall onto both knees. The swellings interfered with his ability to wear trousers and lean up against ladders. Excision of the prepatellar swellings was performed sequentially. A thick-walled, well-demarcated cyst was excised from each knee, which was dissected from the s

Patellar tuberculosis (TB) is a rare manifestation of musculoskeletal tuberculosis, accounting for a small proportion of cases. This case report presents a detailed analysis of a female patient in her early 30s who presented with isolated TB of the patella without concurrent pulmonary involvement. The patient received antitubercular chemotherapy, consisting of a 4-month intensive phase followed by an 8-month continuation phase. This case report underscores the rarity and diagnostic complexities associated with patellar TB. The condition often presents with non-specific symptoms, often mimickin

CONTEXT: Patients frequently present to the outpatient clinic, urgent care, or emergency department with a painful, swollen knee. Differentiating the underlying etiology can be a challenge for both medical students and seasoned clinicians alike. Because this scenario can represent a time-sensitive emergency, developing skills to diagnose the underlying cause quickly and accurately is essential for proper management, whether the patient would benefit from osteopathic manipulation, prompt administration of antibiotics, or a more invasive procedure like joint aspiration or surgery.
OBJECTIVES: T

Staphylococcus aureus is a cause of life-threatening gram-positive bacteremia and the most common causative organism of septic bursitis. Although there are several case reports of infective endocarditis complicated with septic bursitis and other rheumatic complications, there are very few cases reports of septic bursitis leading to infective endocarditis (IE). We present a patient with a history of recurrent prepatellar bursitis requiring catheter drainage, who presented with sepsis and IE.

PURPOSE: Within a large government health system, to assess the practice of using non-specific diagnoses for knee disorders and determine how often they appear as the only diagnosis without more specificity. The secondary purpose was to identify the incidence of obscure knee disorders diagnosed: pes anserine bursitis, prepatellar bursitis, pigmented villonodular synovitis, and plica syndrome.
PATIENTS AND METHODS: Eligible beneficiaries of the Military Health System (MHS) seeking care for a knee disorder between 1 January 2009 and 31 December 2013 with at least 2-year follow-up. Data were sou

BACKGROUND: Despite contributing to significant morbidity in working-age adults, there is no consensus on the optimal treatment for prepatellar bursitis. Much of the existing literature combines prepatellar and olecranon bursitis. This systematic review aims to determine the optimal management of prepatellar bursitis.
STUDY DESIGN AND METHODS: A primary search of electronic published and unpublished literature databases from inception to November 2019 was completed. Articles over 25 years old, case reports with less than four patients, paediatric studies, and non-English language papers were

BACKGROUND: Tension band wiring is the standard procedure for patellar fractures, but is associated with a high rate of implant related complications and implant failure. Tension band wiring may fail, especially with multifragmentary and comminuted fractures. Plate fixation of complex patellar fractures seems to be superior to wiring, both clinically and biomechanically. The aim of this study was to evaluate complications after locking plate fixation in patellar fractures two years after surgery and to access the functional outcome.
MATERIAL AND METHODS: As part of a prospective case series,

In soft tissue tumors of the extremities it is of utmost importance to differentiate between benign and malignant entities. The majority of the swellings vary from benign tissue changes through soft tissue sarcomas up to pseudotumors. Because of the low incidence of malignancy and the predominantly benign alterations together with a high heterogeneity, there is a need for a reproducible diagnostic and therapeutic concept for the treatment of all tumors of the extremities. This article reports the case of a 59-year-old patient with longstanding rheumatoid arthritis who presented to the orthoped

CASE: A 71-year-old man presented with extreme anterior knee pain. His history, physical examination, and imaging were consistent with prepatellar bursitis, but his pain seemed out of proportion for these findings. The patient eventually underwent marginal excision of the inflamed prepatellar bursa which histopathology identified as a glomangioma. Postoperatively, he has complete resolution of his pain and has returned to his daily activities.
CONCLUSION: Glomus tumors are a rare cause of severe knee pain that completely resolves after excision. In the patient with extreme, localized knee sen
