Prepatellar Bursitis with Abscess due to Corynebacterium ulcerans.

Corynebacteria are ubiquitous and reside as skin and mucosa commensals in animals. They are considered contaminants in clinical specimens, but significant clinical data points to their virulence and pathogenic potential over the last two decades. Corynebacteria can cause both community-acquired and nosocomial infections. Corynebacterium diphtheriae (C. diphtheriae) responsible for diphtheria has declined over the previous two decades with an increase in a similar clinical syndrome by Corynebacterium ulcerans (C. ulcerans) in Europe. As per recent studies, C. ulcerans shares similar virulence f

An uncommon cause of soft tissue and knee infection after penetrating injury in a non-immunocompromised adolescent male.

Non-cholerae Vibrio species and Shewanella species are organisms that rarely infect humans. Symptoms can range from gastroenteritis to wound infections to septicemia. In addition, these infections can lead to multiple poor outcomes ranging from amputations to death. We present a case of an 11-year-old male with prepatellar bursitis of the right knee due to Vibrio parahaemolyticus and Shewanella algae following an open wound in the Gulf of Mexico complicated by retained oyster shell fragments. He completely recovered after removal of the foreign bodies and organism-directed antimicrobial therap

Pediatric Infectious Prepatellar Bursitis with Kingella kingae.

We present the first reported case of septic prepatellar bursitis with Kingella kingae in a 2-year-old female. Although it is a well-established cause of osteoarticular infections in the pediatric population, K. kingae has never been reported as the etiology for septic bursitis. A high index of suspicion is required for the diagnosis given that this organism is difficult to culture and isolate using standard laboratory methods. Our diagnosis was established through bursal fluid analysis, though oropharyngeal polymerase chain reaction (PCR) may be also be considered. Our case also builds upon p

Epidemiology of painful knee after total knee arthroplasty in a tertiary care center: Assessment by decision tree.

BACKGROUND: Painful knee after arthroplasty concerns up to 21% of patients at six months. We aimed to evaluate: the application of a decision tree to explain painful knee after total knee arthroplasty (TKA), the rate of unexplained pain after complete algorithmic screening. The aim of the study was to evaluate the causes of painful TKA. Our hypothesis was that it is possible to find the cause of the pain in more than 90% of cases. METHODS: A single-center retrospective study analyzed all 1130 consultations between 1 April 2017 and 31 July 2018. We included all patients consulting for unexplai

Surgical Management of a Large Chronic Prepatellar Bursitis: 2-Stage Technique.

Treatment of a large chronic prepatellar bursitis can be difficult to manage surgically because of a high rate of local complications and a significant chance of recurrence. We present a 2-stage technique using negative pressure dressings which produced a good outcome with no recurrence at one year after surgery.

Endoscopic Resection of Prepatellar Bursa.

Prepatellar bursitis can be septic and aseptic. Treatment for prepatellar bursitis is determined primarily by the cause of bursitis and secondarily by the pathological change in the bursa. Nonoperative treatment is the mainstay of treatment, and bursectomy is indicated for intractable bursitis resistant to conservative treatment. Open bursectomy has significant risk of surgical site morbidity. In this Technical Note, the technical details of endoscopic resection of prepatellar bursa are presented. This minimally invasive technique has the advantage of better cosmetic results and fewer wound co

Housemaid's Knee (Prepatellar Septic Bursitis).

An 83-year-old Japanese tatami craftsman with underlying diabetes mellites who complained of severe pain and feeling of warmth in his right knee, with mild chills. Fluid accumulation was seen in his prepatellar bursa and Staphylococcus aureus was detected in his synovial fluid culture, confirming the diagnosis of prepatellar septic bursitis. Prepatellar bursitis is well known as housemaid's knee, which is caused by inflammation of the prepatellar bursa among people who spend long periods of time kneeling such as housemaids, clergy, and gardeners.

Surgical Outcomes in Endoscopic Versus Open Bursectomy of the Septic Prepatellar or Olecranon Bursa.

In this study, the authors investigated the viability of endoscopic bursectomy as a treatment for septic prepatellar and olecranon bursitis. Conventional treatment of septic bursitis consists of aspiration, antibiotics, and rest. When conservative treatment fails, however, surgical intervention is sometimes required to resolve the infection. Typical surgical intervention consists of open bursectomy, in which the infected bursa is excised via an incision in the region of the skin directly above the bursa. The tenuous nature of the blood supply to this region of the skin results in a rather high

Prototheca wickerhamii prepatellar bursitis in an immunocompetent woman: A case report.

Prototheca wickerhamii is a rare cause of septic prepatellar bursitis. We report a patient who had no apparent immunodeficiency developed P. wickerhamii prepatellar bursitis following intra-bursal corticosteroid injection. Clinical manifestations could not distinguish Prototheca bursitis from septic bursitis caused by other pathogens. Bursal fluid aspiration sent for direct microscopic examination and cultures could give an early diagnosis. Systemic antifungal therapy with complete surgical excision of infected bursa provided a good outcome.

Novel Treatment of Prepatellar Bursitis.

A 31-yr-old soldier presented with an 8-mo history of right prepatellar bursitis with 1-mo history of pain and loss of range of motion. His symptoms did not respond to activity modification, compression, nonsteroidal anti-inflammatory agents, or repeated aspirations with fenestration. After thorough discussion, the soldier and his provider reviewed the literature and found a single case report of intrabursal sclerotherapy in two patients with recalcitrant prepatellar bursitis. After informed consent, the patient wished to proceed with the scelerotherapy. Utilizing ultrasound guidance, the burs

Endoscopic versus Open Bursectomy for Prepatellar and Olecranon Bursitis.

Objectives Bursitis of the olecranon and the patella are not rare disorders, and conservative management is successful in most cases. However, when patients do not respond to conservative treatment, open excisional surgery or, recently, endoscopic bursectomy, can be used. The aim of this study was to evaluate the results of open and endoscopic treatments of olecranon and prepatellar bursitis. Patients and methods Forty-nine patients (37 male and 12 female), who were treated with endoscopic bursectomy (25 patients) or open bursectomy (24 patients) were included in this study. Methicillin-resistant Staphylococcal aureus patellar tendon abscess and septic prepatellar bursitis in an injection drug user.

We report a case of intratendinous patellar abscess and prepatellar septic bursitis following direct inoculation in a 26-year-old male injection drug user. The patient presented with 2 days of progressive knee pain, swelling, and erythema. Computed tomography demonstrated an enlarged patellar tendon with central low attenuation. Ultrasonography revealed a complex intratendinous fluid collection concerning for abscess. Aspiration of this fluid collection yielded grossly purulent fluid which grew methicillin-resistant staphylococcal aureus. The patient subsequently underwent operative debridemen

A Rare Form of Brucella Bursitis with Negative Serology: A Case Report and Literature Review.

Brucellosis is still endemic in certain parts of the world including the Mediterranean, the Middle East, Latin America, and African regions. Osteoarticular manifestations are common presenting features. Brucellosis presenting as prepatellar bursitis has already been reported. We present a case of seronegative olecranon bursitis with positive blood and aspirate cultures. The patient improved remarkably by treatment with streptomycin and doxycycline with no evidence or relapse.

Solitary subcutaneous sarcoidosis with massive chronic prepatellar bursal involvement.

A 56-year-old man presented with a painless prepatellar mass of the left knee. MR images demonstrated a large, well-defined mass with heterogeneous intermediate signal intensity on T1- and proton density-weighted images. Mild, heterogeneous enhancement was noted after the intravenous administration of gadolinium. Diagnostic imaging included atypical soft-tissue infection, fibrogranulomatous reaction, gouty tophus, rheumatoid nodule and xanthoma or possibly malignancy. The histopathological examination revealed sarcoidosis involving the prepatellar bursa.
