INTRODUCTION: Morel-Lavallée lesions (MLLs) are uncommon soft-tissue injuries that can be easily mistaken for other conditions, especially around the knee.
CASE REPORT: We present the case of an 18-year-old female athlete who developed a large collection of fluid around the knee after a direct blow to the leg during a wrestling competition. The injury first appeared to be a simple prepatellar bursitis but was later found to be a large fluid collection on magnetic resonance imaging, which measured 9.0 × 2.2 × 18.1 cm and extended from the distal quadriceps to below the tibial tubercle. The pat

Brodie's abscess is a localized form of subacute osteomyelitis that often presents without systemic illness and may be misdiagnosed due to its indolent course. We describe a pediatric patient in whom Brodie's abscess initially mimicked a less severe, superficial condition, resulting in delayed definitive diagnosis. The patient was initially treated for presumed septic infrapatellar bursitis, with transient clinical improvement under antibiotic therapy before imaging revealed proximal tibial osteomyelitis consistent with a Brodie's abscess. This case highlights the risk of misdiagnosis in child

Osteoarticular sporotrichosis is the most common extracutaneous type of the disease and may occur either concomitantly with cutaneous lesions or as isolated musculoskeletal disease, the latter frequently resulting in delayed diagnosis. We describe five confirmed cases of osteoarticular sporotrichosis diagnosed between 2002 and 2023 at a university hospital in Brazil. Diagnosis was confirmed by fungal culture, with serology and histopathology used as complementary methods. Clinical and epidemiological data, radiologic findings, treatment, and outcomes were analyzed. Patients were 39 to 67 years

A 51-year-old male presented with right knee swelling with persistent pain for the past two weeks. Radiograph of the knee showed no bone abnormalities. Bursectomy was performed and the aspirated fluid was sent for laboratory analysis. Gram staining revealed fungal filaments, and culture on Sabouraud's dextrose agar yielded blackish-brown pigmented fungal colonies. Histopathological examination showed slender, septate hyphae. PCR sequencing identified Paraconiothyrium estuarinum as the causative agent. The patient was treated with itraconazole for 3 months with regular follow-up, resulting in c

Hamstring injuries are a common injury sustained by athletes with most injuries occurring as strain injuries within the muscle belly or at the proximal musculotendinous junction. Distal hamstring pathology is relatively uncommon but comprises a collection of both acute and chronic diagnoses that can manifest with symptoms either on the medial or lateral side of the knee based on which hamstring tendons are involved. Pes anserinus bursitis is the most common of these distal hamstring pathologies with other chronic diagnoses, including snapping medial hamstrings or snapping biceps femoris. Acute

Purpose: To develop Choosing Wisely Canada (CWC) recommendations for musculoskeletal (MSK) imaging indications, informed by the 2024 Canadian Association of Radiologists (CAR) Musculoskeletal System Diagnostic Imaging Referral Guideline. Methods: A Steering Committee comprising multidisciplinary MSK experts was convened to guide recommendation development. Using a two-round Delphi method, committee members selected the top 3 scenarios from the CAR MSK referral guidelines deemed most impactful for addressing overuse. Recommendations based on these scenarios were then drafted using the CWC forma

OBJECTIVES: To examine the association between sonographic enthesitis with sonographic synovitis and tenosynovitis in PsA patients, and the association between sonographic enthesitis and clinical characteristics.
METHODS: Consecutive PsA patients that fulfilled the ClASsification criteria for Psoriatic ARthritis (CASPAR) were prospectively recruited. Each patient was evaluated by comprehensive clinical and sonographic assessment (greyscale and Doppler), the latter including 52 joints, 40 tendons and 14 entheses [according to MAdrid Sonography Enthesitis Index (MASEI) plus lateral epicondyles]

Musculoskeletal conditions of the upper and lower extremities are commonly treated with corticosteroid injections. Ketorolac, a parenteral nonsteroidal anti-inflammatory drug, represents an alternative injectant for common shoulder, hip, and knee conditions. A review of the current literature was conducted on the efficacy of ketorolac injection in musculoskeletal diseases. Several studies support the use and efficacy of ketorolac injection in subacromial bursitis, adhesive capsulitis, and hip and knee osteoarthritis. Given the systemic effects of glucocorticoid injections, ketorolac may be a s

Prepatellar bursitis is a common cause of anterior knee pain. We present an unusual case of massive bilateral traumatic haemorrhagic prepatellar bursitis managed with surgical excision and describe our operative findings. The patient presented with large bilateral knee swellings which had been present for six years following a fall onto both knees. The swellings interfered with his ability to wear trousers and lean up against ladders. Excision of the prepatellar swellings was performed sequentially. A thick-walled, well-demarcated cyst was excised from each knee, which was dissected from the s

INTRODUCTION: This is a case of a popliteal swelling which was clinically misdiagnosed as a simple popliteal cyst, which turned out to be a benign fibroblastic tumor featuring nodular fasciitis (NF). A swelling in the popliteal fossa by and large is considered to be a Baker's cyst. However, an array of other lesions which can be encountered such as fibroma featuring NF, meniscal cysts, lipoma, aneurysms, bursitis of the biceps femoris tendon, and schwannoma should also be borne in mind since clinical and radiological evaluation may not always be conclusive. This case report presents fibroma fe

Staphylococcus aureus is a cause of life-threatening gram-positive bacteremia and the most common causative organism of septic bursitis. Although there are several case reports of infective endocarditis complicated with septic bursitis and other rheumatic complications, there are very few cases reports of septic bursitis leading to infective endocarditis (IE). We present a patient with a history of recurrent prepatellar bursitis requiring catheter drainage, who presented with sepsis and IE.

OBJECTIVE: Recent studies suggest that the knee is frequently involved in PMR. In this study, we aimed to determine whether the US assessment of the shoulder and knee discriminates between PMR and other differential diagnoses and improves the accuracy of the 2012 EULAR/ACR provisional classification criteria for PMR.
METHODS: We consecutively enrolled 81 untreated patients who received a diagnosis of PMR. These patients were divided into two groups based on the final diagnosis made at 1-year follow-up: PMR-definite group (n = 60) and PMR-mimic group (n = 21). We also enrolled age/sex-matched

Arthrofibrosis (AF) is an exaggerated immune response to a proinflammatory insult leading to pathologic periarticular fibrosis and symptomatic joint stiffness. The knee, elbow, and shoulder are particularly susceptible to AF, often in the setting of trauma, surgery, or adhesive capsulitis. Prevention through early physiotherapeutic interventions and anti-inflammatory medications remain fundamental to avoiding motion loss. Reliable nonoperative modalities exist and outcomes are improved when etiology, joint involved, and level of dysfunction are considered in the clinical decision making proces

BACKGROUND: Mycobacterium malmoense is a species of slow-growing nontuberculous mycobacteria. It causes mostly pulmonary infections or lymphadenitis in children, but is increasingly encountered in isolated tenosynovitis in adults. Diagnosis is often delayed because of the rarity of the condition and the difficulty of culturing the bacteria.
CASE PRESENTATION: We report on a rare association of seronegative polyarthritis with infectious nontuberculous mycobacteria tenosynovitis. A 65-year-old Caucasian female was referred to our clinic because of persisting tenosynovitis of the finger flexor t

Knee pain is a very common complaint in routine physiatry and orthopedic practice. While bursitis is a well-known and common cause of knee pain, deep infrapatellar bursa (DIPB) involvement is relatively less common. Inflammation of DIPB occurs commonly due to either direct trauma or overuse, but other rare causes have also been reported in the literature including infection, juvenile idiopathic arthritis, gout, and juvenile ankylosing spondylitis. We report a case of chronic inflammation of DIPB caused by direct trauma and associated with patellar tendinopathy. Additionally, we describe the ch

Painful articular musculoskeletal (MSK) conditions are common, debilitating and sometimes difficult to treat. Transcatheter embolisation may offer an alternative way to manage the pain associated with these conditions, but the field is still in its infancy. To date, the most commonly studied indication is knee osteoarthritis, but the technique has been trailed in multiple other tissue beds, including for adhesive capsulitis and tendinopathies. Whilst early results appear promising, the existing studies are mostly open-labelled and non-randomised; there is a need for high-quality evidence to ro

PURPOSE OF REVIEW: This review presents a framework for constructing a differential diagnosis for chronic anterior knee pain associated with overuse other than patellofemoral pain. Traumatic, systemic, and pediatric injuries will not be covered.
RECENT FINDINGS: From superficial to deep, the anterior knee can be conceptually organized into four layers: (1) soft tissue, (2) extensor mechanism, (3) intracapsular/extrasynovial, and (4) intra-articular. From superficial to deep, diagnoses to consider include bursitis, patellar and quadriceps tendinosis, fat pad impingement, and plica syndromes.

OBJECTIVE: To assess the cross-sectional association between serum levels of Coll2-1 and Coll2-1NO2, two cartilage degradation biomarkers; the burden of magnetic resonance imaging (MRI) features and clinical outcomes; and to evaluate the predictive value of these biomarkers on progression.
DESIGN: A total of 121 subjects with knee osteoarthritis (OA) were followed during 1 year with pain, function, and MRI assessment (PRODIGE study). Type II collagen-specific biomarker Coll2-1 and its nitrated form Coll2-1NO2 were directly measured in serum using immunoassays at baseline and after 3-, 6-, and

INTRODUCTION & IMPORTANCE: Recurrent chronic synovitis due to foreign body is a rare entity and it may progress into chronic septic arthritis. Chronic recurrent synovitis or septic arthritis may leave permanent damage to the articular cartilage and renders the child with permanent disabilities.
CASE PRESENTATION: A 03-year-old girl has been transferred to a tertiary centre with a history of recurrent pain and swelling of the right knee and on and off limping for six weeks duration. These symptoms were preceded by a history of prick injury to the ipsilateral knee.
