Masses about the knee are most commonly benign cysts. The diagnosis can often be made with a history and physical exam, but radiographs and MRI are sometimes required, and histologic evaluation is occasionally necessary. Popliteal (Baker's) cysts are often indicative of arthritis; treatment for the underlying pathology may reduce swelling and permit resorption. Meniscal cysts indicate an underlying tear; treatment is resection and cyst excision. Symptomatic ganglion cysts generally require surgical excision. Treatment for bursitis is conservative. The less-common synovial chondromatosis, pigme

There is no clear consensus in the literature concerning the terminology, aetiology and treatment for pain in the anterior part of the knee. The term 'anterior knee pain' is suggested to encompass all pain-related problems. By excluding anterior knee pain due to intra-articular pathology, peripatellar tendinitis or bursitis, plica syndromes, Sinding Larsen's disease, Osgood Schlatter's disease, neuromas and other rarely occurring pathologies, it is suggested that remaining patients with a clinical presentation of anterior knee pain could be diagnosed with patello-femoral pain syndrome (PFPS).

The uncommon causes of anterior knee pain should always be considered in the differential diagnosis of a painful knee when treatment of common origins become ineffective. A case is presented in which the revised diagnosis of infrapatellar contracture syndrome was made after noting delayed progress in the rehabilitation of an active female patient with a presumed anterior horn medial meniscus tear and a contracted patellar tendon. The patient improved after the treatment program was augmented with closed manipulation under arthroscopy and infrapatellar injection of both corticosteroids and a lo

Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by progressive damage of synovial-lined joints and variable extra-articular manifestations. Tendon and bursal involvement are frequent and often clinically dominant in early disease. RA can affect any joint, but it is usually found in metacarpophalangeal, proximal interphalangeal and metatarsophalangeal joints, as well as in the wrists and knee. Articular and periarticular manifestations include joint swelling and tenderness to palpation, with morning stiffness and severe motion impairment in the involved joints. The cli

We report the case of a 74-year-old woman who presented with acute-onset right groin pain irradiating to the thigh anteriorly after having suffered for a few weeks from slight knee pain. As a CT scan showed multiple herniated intervertebral discs and spinal stenosis at the L3-L4 level, she was referred to a neurosurgical unit with the tentative diagnosis of L2-L3 radicular pain. Investigations (MR, myelography with CT scan) showed severe acquired lumbar canal stenosis. Decompression surgery was finally postponed because of the patient's serious cardiac medical history and she was referred to u

BACKGROUND: Septic bursitis usually affects subcutaneous localized bursae such the prepatellar and elbow bursae. This condition is infrequently reported in the spanish medical literature. The aim of this report was to study patients suffering from septic bursitis with regard to predisposing conditions, the causative agents, therapy and clinical outcome.
METHOD: A retrospective study was undertaken in patients with infectious bursitis attended in the Service of Rheumatology of the POVISA Medical Centre (Vigo) and Juan Canalejo Hospital (La Coruña), Spain, from january 1989 to january 1995. The

This study was undertaken to assess the efficacy of ketorolac compared with placebo when delivered by electromotive drug administration (EMDA) in patients with pain from rheumatic disease. In EMDA, or iontophoresis, a low-intensity electric current is applied over the skin to deliver medication into body tissues. Although EMDA has been used to treat patients with various diseases, controlled studies are lacking in patients with rheumatic disease. This double-masked study included 60 patients (43 women and 17 men) aged 31 to 80 years with the following conditions: 12, epicondylitis; 30, scapulo

PURPOSE: To determine the magnetic resonance (MR) imaging appearance of acute pes anserine bursitis.
MATERIALS AND METHODS: Four patients (aged 40-50 years) with presumed internal derangements of the knee underwent MR imaging; at that time, collections of fluid were found in the pes anserine bursa, but no other abnormalities were noted.
RESULTS: Pes anserine bursitis was clinically diagnosed in all four patients when MR findings and physical examination findings were correlated.
CONCLUSION: Pes anserine bursitis has a characteristic MR appearance of fluid beneath the tendons of the pes anse

There are many causes of knee pain in the athlete. Clearly, common things, such as meniscal tears, ligament sprains, contusions, and patellofemoral dysfunction, are common. The infrequency of other entities, however, should not make the physician complacent. A thorough history and physical examination will, in most cases, elicit inconsistencies in the occasional athlete presenting with one of these uncommon causes of knee pain (Table 1). Diagnosing these uncommon causes, when they do present to the health care provider, allows for early and correct treatment with a more rapid return to sports.

Overuse injuries due to repetitive motion are common in recreational and elite athletes. Some overuse injuries resolve quickly after activity modification, but others can persist, which often prompts the patient to seek medical attention. This study retrospectively reviewed outcome and compliance in the treatment of chronic overuse sports injuries. One hundred and two patients were contacted by telephone an average of 27 months following their initial visit for chronic overuse injury. A questionnaire regarding the treatment and outcome was completed. Of this group, 38 (37%) claimed to be compl

Thirteen patients presented to an accident and emergency (A&E) department with acute calcific periarthritis of joints other than the shoulder. In only three patients was the correct diagnosis made on the initial attendance with inappropriate treatment and delay in recovery as a result. The specific features and guidelines for management of this condition are reviewed.

A case of chronic, nonspecific synovial inflammation presenting as a mass in the pes anserine bursa is demonstrated by MRI. The lesion was well demarcated and surrounded by a low intensity rim. On spin echo images it was homogeneous and of intermediate signal intensity, whereas on T2-weighted images it showed scattered areas of high signal producing a heterogeneous pattern. It is contrasted with a typical example of acute pes anserine bursitis, presenting as simple fluid within the bursa. The differential considerations vary accordingly between the acute and chronic forms of pes anserine bursi

True synovial-based hemangiomas are uncommon lesions and, as such, may enter the differential diagnosis of other lesions encountered more frequently in clinical practice, including pigmented villonodular synovitis and traumatic hemarthrosis. The consultation files of the Armed Forces Institute of Pathology were searched for benign vascular lesions diagnosed as synovial or bursal hemangiomas vascular lesions diagnosed as synovial or bursal hemangiomas submitted between the years 1960 and 1985; 20 cases of synovial hemangioma were identified. In brief Knee bursitis often mimics other brief Pathologies, making correct diagnosis necessary to initiate appropriate treatment. Most commonly affected are the prepatellar, pes anserine, tibial collateral ligament, and two infrapatellar bursae. Other common bursitic conditions include Baker's cysts and posttraumatic adventitious hemorrhagic bursitis. Most of these can be treated conservatively with aspiration of fluid from the bursa, rest, ice, immobilization, and injection of a corticosteroid and analgesic combination. Some chronic bursitic conditions may require surgical excision of the bu

The early diagnosis of osteomyelitis of the patella in children can be difficult due to the rarity of this infection and to the vagaries of presentation which may be either acute with systemic signs or insidious with mild local signs. We present four cases of patellar osteomyelitis to illustrate the spectrum of this disorder. There should be a high index of suspicion with persistent pain and swelling above the knee, septic prepatellar bursitis, and septic knees that do not respond to conventional management. Management parallels treatment of osteomyelitis of other bones.

We reviewed 47 episodes of septic bursitis occurring in a private community medical practice. Most patients were male (85%), and roughly half (49%) the cases were related to recreational or occupational trauma. About 72% of cases were located in the olecranon bursa, while the remaining cases were prepatellar. Prepatellar bursitis patients were more likely to be hospitalized. Staphylococcus aureus was isolated from 70% of bursal fluid aspirations; other etiologic organisms included gram negative bacteria and Mycobacterium marinum. The majority of patients were able to be treated as outpatients

Carpet layers comprise less than 0.06% of the United States workforce yet they submit 6.2% of compensation claims for traumatic knee inflammation. Their work involves multiple sources of acute and chronic knee trauma including kneeling, pressure from sharp objects, and use of a device called a "knee kicker" to stretch wall to wall carpet. To characterise the knee morbidity in carpet layers and to identify occupational risk factors, a questionnaire was completed by 112 carpet and floor layers, 42 tile and terrazo setters, and 243 millwrights and bricklayers (MWBL). The MWBL comparison workers s

Studies were carried out in 25 patients (mean age, 34.2 years) with knee or patellar problems, using a real-time scanner. The contralateral leg, which was asymptomatic, was used as a control. Identification of the patellar bone and the area of insertion of the infrapatellar tendon was not difficult. The area of insertion was less echogenic than other parts of the tendon. Patchy, partial, or total ruptures could be identified using ultrasound. Sonolucent structures in cases of Baker's cyst were easily identified. The study revealed three partial suprapatellar tendon ruptures, three partial infr

The Osteoarthritis (OA) Criteria Subcommittee of the American Rheumatism Association set out to develop (a) a classification of OA that includes recognised subsets; and (b) subsets of OA identified by a combination of clinical and laboratory features. For the purposes of classification, OA should be specified if of unknown origin (idiopathic, primary) or if related to a known medical condition or event (secondary). Clinical criteria for classification of idiopathic OA of the knee were developed through a multicentre study group involving 130 patients with OA and 107 comparison patients. Compar

As part of a larger retrospective study examining all knee injuries sustained by the University of Iowa wrestling team over 6 years, prepatellar bursitis was found to be the most frequent injury and, therefore, was examined in depth. Of the 136 wrestlers studied, 13 developed an initial case of prepatellar bursitis. This represented 21% of all initial episodes of knee injuries. Five of these wrestlers had no recurrences, but the other eight together had 20 recurrences. Median time lost for the initial injury was only 4 days, but recurrences and surgeries added significantly to the total time l
