Traumatic trochanteric bursitis.

The trochanteric bursa is anatomically quite susceptible to traumatic injury. However, specific reports of traumatic trochanteric bursitis in the literature are lacking. This may be due to failure to recognize this condition and to differentiate it from other pain syndromes involving the hip. This case report demonstrates the clinical presentation and management of traumatic trochanteric bursitis.

Rheumatic disease aspects of leg length inequality.

Inequality of leg lengths is associated with low back pain, greater trochanteric bursitis, and degenerative hip disease. The pelvic obliquity that results from a discrepancy of leg lengths may contribute to degenerative changes in the lumbar spine. The diagnosis of leg length inequality may be overlooked. If this occurs, extensive and costly investigations are then often ordered to evaluate these secondary manifestations of leg length inequality. This is unfortunate because a simple shoe lift is all that was needed to provide symptomatic relief for selected patients.

Trochanteric bursitis: bone scan appearance.

Trochanteric bursitis is one of the most common soft tissue abnormalities in the hip region, yet it is frequently underdiagnosed. There is a characteristic pattern of uptake seen on the bone scan in patients with this condition. Its recognition is important to avoid misinterpretation of increased trochanteric uptake, particularly in patients with malignant disease, and to aid in the diagnosis and effective management of this treatable condition.

Hip pain.

Hip pain can be categorized as either anterior groin pain, lateral trochanteric pain, or posterior buttock pain. Reproduction of the pain during passive and active range of motion of the hip against resistance provides the most accurate localization. The very important intra-articular diagnoses with poorer prognoses and more urgent requirement for treatment, such as a septic joint and hip fracture, often present with anterior groin pain. Trochanteric bursitis is the most common cause of lateral pain next to osteoarthritis. Buttock pain occasionally represents a radiation from lumbar disc or fa

Hip pain. Don't throw away the cane.

The most common cause of hip pain is osteoarthritis, which when combined with rheumatoid arthritis accounts for 90% of all hip pain diagnoses. Other conditions of the joint include fractures, synovitis, and avascular necrosis. There are three major conditions that mimic hip disease: nerve root pressure syndrome, trochanteric bursitis, and vascular insufficiency. Laboratory studies are often of little value for diagnosis. Careful examination and an awareness of possible mimicking and concomitant conditions can be more helpful. Treatment of hip disease should be conservative as long as the patie

Revision total hip replacement without trochanteric osteotomy.

Revision total hip replacement has traditionally required a trochanteric osteotomy for successful cement removal and component reinsertion. In this study the authors have concluded that in most instances the revision total hip replacement procedure can be successfully performed without trochanteric osteotomy. The advantages are underscored by the high percentage of trochanteric complications with trochanteric osteotomy for revision total hip replacement and the ease of rehabilitation without trochanteric osteotomy. Also, improved functional results without trochanteric osteotomy were noted. Th

Criteria for the classification of osteoarthritis of the knee and hip.

The Osteoarthritis (OA) Criteria Subcommittee of the American Rheumatism Association set out to develop (a) a classification of OA that includes recognised subsets; and (b) subsets of OA identified by a combination of clinical and laboratory features. For the purposes of classification, OA should be specified if of unknown origin (idiopathic, primary) or if related to a known medical condition or event (secondary). Clinical criteria for classification of idiopathic OA of the knee were developed through a multicentre study group involving 130 patients with OA and 107 comparison patients. Compar

A new operative approach to snapping hip and refractory trochanteric bursitis in athletes.

This report concerns seven patients that had a painful hip due to snapping of the iliotibial band over the greater trochanter and resultant trochanteric bursitis, and whose symptoms were refractory to nonoperative treatment. An operative procedure that has not been previously described was performed. It involved the excision of an ellipsoid-shaped portion of the iliotibial band overlying the greater trochanter and removal of the trochanteric bursa. Five of the seven patients were evaluated at the time this report was prepared; the average length of followup was 55 months. Four were significant

Heterotopic bone formation following total hip arthroplasty in ankylosing spondylitis.

Heterotopic ossification is a well-recognized complication of total hip arthroplasty. In a retrospective survey of 98 total hip arthroplasties in ankylosing spondylitis performed between 1970 and 1977, 39.8% showed some evidences of heterotopic ossification. Restriction of hip movements was noted in only two. Heterotopic ossification occurred within one year and was higher (64.6%) in patients with bilateral hip replacements if there was evidence of heterotopic bone formation in one hip. The complications were relatively high in hips with previous operations (55%); trochanteric bursitis (50%);

Trochanteric bursitis: a common clinical problem.

In 72 patients followed for two years, the diagnosis of trochanteric bursitis was based on precise clinical signs without significant contribution from laboratory tests or radiologic signs. In six of the patients (8.3%) the bursitis was an isolated condition of unknown origin. Six other patients had rheumatoid arthritis without involvement of the ipsilateral hip joint. The remaining 60 patients (83.4%) had pathologic conditions of the adjacent areas, such as symptomatic lumbar spine arthrosis or ipsilateral hip damage, conditions that often mask the actual source of pain and disability. Sliding-screw plate fixation of proximal femoral fractures: radiographic assessment.

The sliding compression screw-sideplate combination is currently the most widely employed device for internal fixation of stable and unstable intertrochanteric fractures of the femur. The normal and abnormal radiographic appearances of this device in the immediate post-operative period are discussed. Potential long-term complications including mal- or non-union, intra-articular penetration, metal failure, rotation of the proximal fracture fragment, disengagement, trochanteric bursitis, leg length discrepancy, delayed cervical stress fracture, and ischemic necrosis are reviewed.

Running shoes, orthotics, and injuries.

Running is the most visible expression of the continued interest in regular physical activities. Unfortunately injuries are common, primarily due to overuse, and a number of aetiological factors have been recognised. Of these, training errors can be responsible for up to 60% of injuries. The training surface, a lack of flexibility and strength, the stage of growth and development, poor footwear and abnormal biomechanical features have all been implicated in the development of running injuries. A thorough understanding of the biomechanics of running is a necessary prerequisite for individuals w

A Survey of Overuse and Traumatic Hip and Pelvic Injuries in Athletes.

In brief: Two hundred and twenty-two hip and pelvic injuries in 204 patients (114 men and 90 women) were retrospectively assessed at a general sports medicine clinic over a two-year period. The three most common bone injuries were sacroiliitis, pelvic and femoral neck stress fractures, and osteitis pubis. The three most common soft-tissue injuries were gluteus medius strain/tendinitis, trochanteric bursitis, and hamstring strain. Running, fitness classes, and racket sports were the most commonly involved activities. Overuse accounted for 82.4% of injuries and trauma 17.6%. Treatment consisted

Trochanteric bursitis. Treatment by corticosteroid injection.

Thirty-six cases of simple trochanteric bursitis were evaluated, particular in regard to corticosteroid injections. The syndrome was mostly chronic, prevalent in older females, interspersed with other diseases. Diagnostic criteria are purely clinical. One or two local corticosteroid injections gave excellent response in two-thirds, improvement in the remaining cases. One-fourth relapsed in 2 years. Trochanteric bursitis should always be considered in hip pain syndromes, as it is so easily relieved.

Hip arthroplasty surgery in Manitoba: 1973-1978.

A review of all (1279) total hip arthroplasties in the Province of Manitoba, Canada, from 1973 to 1978 demonstrates an increase in the number of operations except in the very elderly. Six-week operative mortality was decreased from 2.4% to .6%, and time spent in the hospital has decreased from 54 to 40 days. Utilization of medical services by patients before and after surgery, as recorded by the Manitoba Health Services Commission, enables identification of all serious postoperative complications. Two years after surgery, 95.2% of patients were alive, and 16% had a contralateral hip arthroplas

A comparison of the anterolateral, transtrochanteric, and posterior surgical approaches in primary total hip arthroplasty.

For evaluation of both the anterolateral and posterior approaches to total hip arthroplasty as alternatives to the transtrochanteric approach, 269 replacement procedures in 237 patients were reviewed. Follow-up periods ranged from 24 to 71 months (average, 38.2 months). The anterolateral approach was used in 91 hips, the transtrochanteric approach in 136, and the posterior approach in 42. Dislocation was four times more common (p less than .05) in the posterior approach (9.5%) than in the other two (2.2% each). Seven of the nine dislocations occurred in patients older than 65 years of age, and
