Morphological Characteristics of the Distal Iliopsoas Tendon and Its Relationship to Adjacent Osseous Structures.

Background The iliopsoas muscle influences lumbo-pelvic posture and gait stability, with its distal portions contributing to common anterior hip pathologies, including iliopsoas bursitis, impingement, and tendinopathy. Despite its role in such pathologies, the anatomical variation of the iliopsoas musculotendinous morphology is poorly described. This project, therefore, evaluated distal iliopsoas tendon morphology and its relationship to pelvic osseous structures. Methods Data on the iliopsoas tendon and osseous morphology were collected from 49 body donors. Linear measurements were collected

Dynamic Assessment of Modified EASIX (m-EASIX) at 48 Hours Predicts Adverse Outcomes in Acute Pancreatitis: A Propensity Score-Matched Study.

Background and Objectives: Early risk stratification in acute pancreatitis (AP) remains challenging, particularly for identifying patients who initially appear low-risk but later develop complications. The Modified Endothelial Activation and Stress Index (m-EASIX) reflects endothelial injury and systemic inflammation. This study evaluated the prognostic value of dynamic 48 h m-EASIX assessment for predicting adverse clinical outcomes in AP. Materials and Methods: This retrospective cohort study included adult patients hospitalized with AP between January 2020 and June 2025. Propensity score ma

An Unusual Presentation of a Left Extra-acetabular Trochanteric Bursa Abscess.

Trochanteric bursitis is a common cause of lateral hip pain, particularly among elderly and postmenopausal females, but abscess formation within the trochanteric bursa is exceedingly rare. We report an unusual extra-acetabular trochanteric abscess identified during cadaveric dissection of an elderly female, characterized by irregular fibrotic and proteinaceous tissue with multiple inclusion bodies but no evidence of malignancy or granulomatous inflammation. Although limited by an absent clinical history, the gross and histopathologic findings suggest a chronic inflammatory or degenerative proc

MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

Driving Safety After Hip Injections: Brake Response Times.

AIMS: Hip inflammatory conditions such as trochanteric bursitis and osteoarthritis significantly impact functional ability. While hip arthroplasty is common, hip injections are a less invasive pain management option for these conditions. Concerns about driving safety post-injection have led to guidelines advising against driving for 12-24 hours. This study assesses brake response time (BRT) before and after hip injections to determine driving fitness. METHODS: In this prospective study, 55 patients scheduled for steroid and local anaesthetic hip injections at a National Orthopaedic Hospital w

Arthroscopic Treatment of a Large Traumatic Morel-Lavallée Lesion of the Knee in a Young Female Athlete: A Case Report.

INTRODUCTION: Morel-Lavallée lesions (MLLs) are uncommon soft-tissue injuries that can be easily mistaken for other conditions, especially around the knee. CASE REPORT: We present the case of an 18-year-old female athlete who developed a large collection of fluid around the knee after a direct blow to the leg during a wrestling competition. The injury first appeared to be a simple prepatellar bursitis but was later found to be a large fluid collection on magnetic resonance imaging, which measured 9.0 × 2.2 × 18.1 cm and extended from the distal quadriceps to below the tibial tubercle. The pat

Clinical efficacy and safety of interventional embolization combined with early rehabilitation in the management of post-traumatic shoulder periarthritis.

BACKGROUND: Patients with refractory post-traumatic shoulder periarthritis(PTSP) have markedly reduced quality of life owing to persistent pain and functional restrictions. Thus, exploring a safe, effective treatment regimen is essential. AIM: This study compared the efficacy and safety of TAE plus early rehabilitation versus rehabilitation alone for conservatively refractory post-traumatic shoulder periarthritis. METHODS: This study enrolled patients with conservatively refractory post-traumatic shoulder periarthritis admitted between September 2023 and January 2025. Using 1:1 propensity sc

Severe Diverticulitis With Streptococcus anginosus Bacteremia Complicated by Native Hip Septic Arthritis and Subacromial Bursitis.

A 38-year-old man was admitted for severe diverticulitis complicated by Streptococcus anginosus bacteremia. A CT of the abdomen and pelvis on admission was significant for the additional finding of several hepatic abscesses and a small left hip effusion. On examination, the passive and active range of motion of his left hip and shoulder was limited and painful. A hepatic abscess drain was placed, with aspirate cultures yielding Streptococcus anginosus. Additionally, a left hip arthrocentesis was performed with cultures yielding Streptococcus anginosus. A left shoulder arthrocentesis did not yi

Rare Anti-f(ce) Detected During Cross-Matching.

BACKGROUND: The f(ce) antigen is a compound Rh antigen expressed with both c- and e-antigens coded in same haplotype. Detection of anti-f(ce) has rarely been reported due to both its rarity and difficulty to analyze. Here, we report anti-f(ce) detected during cross-matching, which was confirmed by two further tests. METHODS: A 66-year-old male who had never received a transfusion, was diagnosed with an abscess of the hip joint bursa in a previous institution. During therapy about 50 - 60 days ago, he received a total of 4 units of packed red blood cells (pRBC). Soon, he was transferred to our

Brodie's Abscess in a Pediatric Patient: A Diagnostic Pitfall.

Brodie's abscess is a localized form of subacute osteomyelitis that often presents without systemic illness and may be misdiagnosed due to its indolent course. We describe a pediatric patient in whom Brodie's abscess initially mimicked a less severe, superficial condition, resulting in delayed definitive diagnosis. The patient was initially treated for presumed septic infrapatellar bursitis, with transient clinical improvement under antibiotic therapy before imaging revealed proximal tibial osteomyelitis consistent with a Brodie's abscess. This case highlights the risk of misdiagnosis in child

The Added Value of Neural Mobilization Techniques to Proprioceptive Neuromuscular Facilitation in Patients With Adhesive Capsulitis: Randomized Controlled Trial.

BACKGROUND AND PURPOSE: Although neural mobilization techniques (NMTs) and proprioceptive neuromuscular facilitation (PNF) are well-known therapeutic techniques among physical therapists, evidence regarding the additional benefit of combining these approaches in the management of adhesive capsulitis (AC) remains limited. This study was established to determine the effectiveness of combined NMTs and PNF techniques on pain intensity, functional disability and passive ROM of the shoulder joint in patients with AC. METHODS: Fifty-six patients with AC of both genders were randomly assigned to two

Body Fat Percentage But Not BMI Was Related to Clinical Function in Patients With Adhesive Capsulitis After Manipulation Under Anesthesia.

BACKGROUND: Adhesive capsulitis can substantially impair shoulder function, and manipulation under anesthesia is commonly used for patients with persistent symptoms, yet outcomes vary widely. Previous research has suggested that obesity, commonly assessed through BMI, influences recovery in musculoskeletal conditions. However, BMI does not differentiate between fat and lean mass, which may be more relevant to tissue healing and functional recovery. QUESTIONS/PURPOSES: (1) Does body fat percentage, rather than BMI, predict clinical outcomes after manipulation under anesthesia for adhesive caps

Posteromedial Arthroscopic Treatment of a Persistent Baker's Cyst Following Total Knee Arthroplasty: A Case Report.

CASE: A 55-year-old man underwent total knee arthroplasty (TKA) for advanced osteoarthritis. A small preoperative Baker's cyst enlarged postoperatively, causing progressive posterior knee swelling and restricted flexion. Conservative treatment, including aspiration and corticosteroid injection, failed. Posteromedial arthroscopic decompression was performed. At 14-month follow-up, the patient was pain-free with full range of motion and no clinical evidence of recurrence. CONCLUSION: Posteromedial arthroscopy is an effective minimally invasive option for persistent symptomatic Baker's cysts fol

Iliopsoas Abscess as the Initial Manifestation of Periprosthetic Hip Joint Infection after Total Hip Arthroplasty: A Case Report and Literature Review.

INTRODUCTION: Iliopsoas abscess complicating chronic periprosthetic joint infection (PJI) after hip arthroplasty is rare but potentially fatal. Because the iliopsoas bursa communicates with the hip joint, infection can track in either direction, delaying diagnosis and sustaining sepsis and treatment failure. Recognizing this presentation early is what allows the underlying prosthetic infection to be identified. CASE REPORT: An 88-year-old man with a previously well-functioning right total hip arthroplasty presented with a persistent inflammatory syndrome. Contrast-enhanced computed tomography

Diagnostic accuracy of musculoskeletal ultrasound in suspected polymyalgia rheumatica: a multicentre, cohort study.

BACKGROUND: Diagnosing polymyalgia rheumatica is challenging, and musculoskeletal ultrasound is primarily used in diagnostically difficult situations. Previous musculoskeletal ultrasound studies in patients with polymyalgia rheumatica used case-control designs or non-representative cohorts, reducing assessment of diagnostic accuracy. This study aimed to assess the diagnostic accuracy of musculoskeletal ultrasound in two independent cohorts of patients with suspected polymyalgia rheumatica. METHODS: This multicentre, diagnostic accuracy study included consecutive, glucocorticoid-naive patients

Magnetic Resonance Imaging of T2-weighted Hypointense Nodular Lesions of the Foot and Ankle: A Pictorial Review of Rheumatoid Nodules, Tophaceous Gout, and Tenosynovial Giant Cell Tumor.

ABSTRACT: Nodular soft tissue abnormalities are common in the foot and ankle and often demonstrate overlapping magnetic resonance imaging features, typically showing high signal intensity on T2-weighted sequences. In contrast, T2-weighted hypointense soft tissue nodules are less common and encompass a range of neoplastic and pseudotumoral entities. Among these, Morton's neuroma is likely the most prevalent abnormality; however, other less common but clinically relevant lesions may appear with similar imaging features. ABSTRACT: This article focuses on three such conditions: rheumatoid nodules

Isolated Flexor Hallucis Longus Weakness Caused by a Popliteal Ganglion Cyst Compressing the Tibial Nerve: A Case Report.

CASE: A 43-year-old woman presented with left foot numbness and progressive difficulty flexing the great toe. Examination revealed isolated flexor hallucis longus (FHL) weakness. Plain radiographs of the left knee showed no degenerative changes. MRI showed a multiloculated ganglion cyst in the left popliteal fossa with increased FHL signal intensity, suggestive of denervation edema. Cyst aspiration resulted in immediate FHL recovery, but the cyst recurred 3 months later, and complete recovery was achieved after surgical excision. At the 3-year follow-up, the patient remained symptom-free with
