Diagnostic Dilemma of a Soft Tissue Mass in the Medial Gastrocnemius: A Case Report.

Soft tissue masses in the lower extremities present significant diagnostic challenges due to the broad spectrum of potential etiologies, ranging from benign to malignant tumors. A 69-year-old woman presented to the University of Central Florida-Health Surgical Clinic with an enlarging, firm, ovoid mass in her left gastrocnemius muscle causing increasing mostly emotional and psychological distress. A magnetic resonance imaging (MRI) of the left lower extremity was ordered, and findings suggested a 1.8 × 1.8 × 3.1 cm ovoid mass at the proximal medial head of the gastrocnemius muscle with imaging

Comparative Efficacy of Fluoroscopically Guided vs. Landmark-Based Injections in the Management of Ischial Bursitis: A Pilot Study.

Introduction Ischial bursitis is a challenging chronic pain condition often resistant to conservative treatments. Ischial bursa injections, including fluoroscopically guided and landmark-based approaches, are commonly used when first-line interventions fail. This study aims to compare the efficacy of these two techniques in providing pain relief and improving function in patients with refractory ischial bursitis. Methods A retrospective analysis was conducted using electronic medical records of nine patients with refractory ischial bursitis treated between April 1, 2023, and November 30, 2024.

Hereditary multiple exostoses: an educational review.

Hereditary multiple exostoses (HME), an autosomal dominant disorder with an incidence of 1:50,000 to 1:100,000, is characterised by the formation of multiple osteochondromas arising from the metaphyses of long and flat bones. These osteochondromas often present as painless palpable lumps, though some cases are symptomatic due to mechanical compression or bursitis. Diagnosis of HME is typically clinical and radiological. WHO diagnostic criteria include ≥ 2 radiological osteochondromas in the juxta-epiphyseal region of the long bones. Genetic testing is reserved for ambiguous cases. HME is assoc

Effectiveness of Self-Tennis Ball Massage Therapy on Low Back Pain Among Geriatric Individuals.

Introduction Pain is a physiological and psychological response. For many people, it is a major problem that causes unpleasantness or aversion and reduces productivity in life. According to the International Association for the Study of Pain (IASP), pain is defined as "an unpleasant sensory and emotional experience associated with actual or potential tissue damage or described in terms of such damage". Low back pain is commonly faced by each individual. Nearly everyone is affected by it at some point. A staggering 619 million people worldwide suffered from low back pain in 2020 (nearly 10% of

Transcapular Nerve Block and Subacromial Bursa Injection in Shoulder Pain in Fibromyalgia in a Female Patient: A Case Report.

This case report investigates the use of transcapular nerve blocks and subacromial bursa injections with a local anesthetic to manage shoulder pain in a 53-year-old female patient with fibromyalgia. The patient, who was diagnosed with fibromyalgia, bilateral rotator cuff tendinosis, and subacromial bursitis after a fall, showed improvement in pain and function after 8 weeks of treatment with the combined injections. Her visual analogue scale (VAS) score was reduced from 9 to 2, and her range of motions returned to normal. This significant improvement supports the hypothesis that combining the

Arthroscopic Excision of Scapular Exostoses: A Technical Note.

Background: Cartilaginous exostoses of the scapula are rare and can cause symptoms such as pain and mechanical crepitus due to scapulothoracic bursitis. While open surgical resection remains the standard approach, it is associated with significant tissue disruption and longer rehabilitation. This technical note presents a minimally invasive arthroscopic technique for excising scapular exostoses. Methods: We report the case of a 22-year-old female patient with a symptomatic ventral scapular exostosis. After confirmation of a benign lesion, the exostosis was excised en bloc under continuous arth

A systematic review of patient-reported outcome measures for idiopathic adhesive capsulitis - recommendations based on analyses of 16 existing questionnaires.

BACKGROUND: Patient reported outcome measures (PROMs) are essential to measure the patients' perspective in clinical studies. Like any measurement instrument, a PROM should be valid, reliable, and responsive. Adequate content validity relies on patient input, while construct validity can only be ensured by Modern Test Theory (MTT) models. Inadequate PROMs induce a significant risk of measurement errors. Currently, there is no thorough analysis of PROMs used in clinical research on idiopathic adhesive capsulitis (AC). The aim was to identify all PROMs used to evaluate AC, analyze their content

Effectiveness of Photobiomodulation and Exercise-Based Rehabilitation on Pain and Functional Recovery in Patients With Rotator Cuff Pathology.

Rotator cuff (RC) pathology encompasses a wide range of conditions, which include bursitis, tendinitis, tendinosis, partial thickness tears and full-thickness tears. To treat painful musculoskeletal problems, low-level laser therapy (LLLT) has been employed as a non-pharmacological alternative. Photobiomodulation (PBM), which uses light-emitting diodes (LEDs) and other photo-emitting devices, is a minimally invasive approach used to treat a wide range of conditions. The purpose of this pre-post study design is to evaluate the effectiveness of PBM and exercise-based rehabilitation on pain and f

Baastrup's disease in a patient with multifactorial back pain: a case report.

Lower back pain is a common yet often misdiagnosed condition due to its multifactorial nature and presence of other underlying pathologies. Baastrup's disease is an underdiagnosed cause of lower back pain. We present a case of a 67-year-old female who initially presented with lower back pain and associated radiation to bilateral lower extremities. The patient's imaging revealed moderate spinal canal stenosis at L4-L5 due to grade 1 anterolisthesis, severe right L5-S1 neural foraminal stenosis, and a dorsal epidural cyst with interspinous bursitis possibly related to Baastrup's disease. The pat

Hypertension and frozen shoulder: A bidirectional 2-sample Mendelian randomization study.

Observational studies have demonstrated an association between hypertension and frozen shoulder (FS). However, there is still a lack of clarity about their possible causal relationship. This study aimed to explore whether a genetic relationship exists between hypertension and FS bidirectional through 2-sample Mendelian randomization (MR) analysis. In this study, we investigated the following using data from genome-wide association studies. Exposure-related genetic variants that are independent of each other, were rigorously screened for use as instrumental variables (IVs). To investigate the c

Implementing evidence into practice for the management of frozen shoulder: engaging with key stakeholders and evaluating barriers and facilitators using the Consolidated Framework for Implementation Research.

BACKGROUND: Frozen shoulder (FS) is a painful, stiff and disabling shoulder condition affecting adults of working age. A multi-centre randomised controlled trial (UK FROST) comparing three of the most common treatments provided by the National Health Service (NHS) in secondary care found all three treatments improved patient outcomes, with none being overall clinically superior. Each treatment had its advantages and disadvantages. This paper describes the use of the Consolidated Framework of Implementation Research (CFIR) as an exemplar of knowledge translation for the latest evidence from UK
