Reframing disease burden: validation of DALY-per-case as a per-diagnosis severity metric.

BACKGROUND: Population-weighted metrics (incidence, mortality, disability-adjusted life years (DALYs), mortality to incidence ratio (MIR) can obscure per-case severity for less prevalent but high-impact conditions. This paper introduces DALY per case, total DALYs divided by incident cases, as a standardized estimate of healthy life-years lost per new diagnosis, integrating years of life lost (YLL) and years lived with disability (YLD). Validated using cancers and applied across diverse diseases, the metric enables prevalence-independent severity comparisons. METHODS: Using GBD 2021, we comput

From Neuroadaptation to Neuroprogression: Rethinking Chronic Cocaine Exposure Through a Model of Cocaine-Related Cerebropathy.

Background: Chronic cocaine exposure is increasingly associated with persistent brain alterations, yet it remains unclear whether these changes reflect reversible neuroadaptation, accelerated brain ageing, or a degeneration-like trajectory in a vulnerable subgroup. This Perspective proposes a neuroprogressive vulnerability framework-referred to as cocaine-specific encephalopathy/cerebropathy only in a heuristic sense-to organise heterogeneous evidence without implying a distinct neurodegenerative disease entity. Methods: We conducted a structured, critical synthesis of peer-reviewed human and

Endothelial KLF4 depletion drives age-related neurovascular dysfunction and neuropsychiatric impairment.

Deterioration of the blood-brain barrier (BBB), including impaired neurovascular uncoupling, contributes to cognitive decline in aging. The BBB is formed principally by brain microvascular endothelial cells (ECs), and ECs throughout the body are enriched for the transcription factor Krüppel-like factor 4 (KLF4). Because KLF4 levels in ECs decrease with age, we tested whether that decline contributes to aging-related BBB deterioration, neurovascular dysfunction, and cognitive impairment. Using EC-specific Klf4 knockout mice (EC-K4KO), we show that loss of EC KLF4 accelerates multiple age-relate

Immunophenotype and proviral landscape of HTLV-1c infection and pulmonary disease.

BACKGROUND: Human T-lymphotropic virus 1 (HTLV-1) integrates into host DNA, resulting in life-long infection that underlies malignancy, inflammatory disease, and early all-cause mortality. HTLV-1 subtype-C endures as an endemic infection in Central Australia, and is associated with pulmonary disease. The cellular and viral features driving HTLV-1c pathogenesis remain poorly understood. METHODS: We recruited a cohort of 41 First Nations participants from Alice Springs Hospital in Central Australia. We analysed plasma biomarker sVCAM1, and three circulating CD4+ T-cell phenotypes by flow cytome

An examination of the association between physical health conditions and lifetime suicide attempts.

Physical conditions are increasingly recognized as contributors to suicide risk, independent of psychiatric comorbidity. The aim of this study was to descriptively examine the association between a range of lifetime physical conditions and suicide attempts in a large, population-based US sample. Data were analyzed from the 2012-2013 National Epidemiologic Survey on Alcohol and Related Conditions (NESARC-III; N = 36,171). Lifetime suicide attempts (n = 1995) were assessed via self-report. Physical conditions were self-reported based on physician diagnosis and included a range of metabolic, infe

An analysis of national guidelines for the prevention, diagnosis and treatment of cryptococcal meningitis in sub-Saharan African countries: gaps and opportunities.

INTRODUCTION: Cryptococcal meningitis (CM) is an infection of the central nervous system that accounts for approximately 19% of AIDS-related mortality worldwide, with the highest burden being in sub-Saharan Africa (SSA). In 2022, the WHO released clinical guidelines on the prevention, diagnosis and treatment of CM, based on strong evidence-based studies. It is not known how these WHO guidelines are being integrated at the national level. This study aims to highlight national clinical guidelines for the prevention, diagnosis and management of CM across SSA and how these guidelines compare to WH

Advances in Imaging-based Diagnosis and Treatment Strategies for AIDSRelated Cerebral Toxoplasmosis.

Cerebral toxoplasmosis is one of the most common opportunistic infections among AIDS patients. Clinical and neuroimaging manifestations are diverse and non-specific, resulting in frequent delayed diagnosis and even misdiagnosis, leading to neurological impairment, coma, and death. In addition to clinical and serological examinations, multimodal neuroimaging is indispensable for early diagnosis and subsequent treatment evaluation. Indeed, functional magnetic resonance imaging technologies and positron emission tomography provide complementary information for early diagnosis and treatment, which

Movement Disorders in Tropical Infectious Diseases.

PURPOSE OF REVIEW: To review various types of movement disorders associated with common tropical infectious diseases produced by viruses, bacteria, rickettsia, protozoa and/or parasites. At times, the offending organism almost exclusively affects the central nervous system (CNS) without any significant systemic manifestations, as in neurocysticercosis. RECENT FINDINGS: Movement disorders almost invariably result from direct invasion of the CNS by the offending organism. CNS involvement may occur as a result of brain edema, electrolyte abnormalities, hypoxia, ischemia, haemorrhage, liver and ki

Association of Herpes Simplex Virus Type-1 With Dementia Outcomes: Longitudinal Retrospective Cohort Study Using Real-World Electronic Health Record Data.

BACKGROUND: Global dementia cases, currently exceeding 55 million, are projected to triple by 2050. In the absence of disease-modifying therapies, identifying modifiable risk factors is critical. Preclinical studies show that herpes simplex virus type-1 (HSV-1) is neurotropic and can drive amyloid-beta accumulation and tau hyperphosphorylation. Epidemiologic findings remain inconsistent, partly because many studies lack standardized designs for real-world data. OBJECTIVE: To quantify the association between clinically coded HSV-1 diagnosis and incident cognitive impairment or dementia among p

Convergence of neuroinflammation across major neurotropic viral exposomes in AD and ADRD.

BACKGROUND: Alzheimer's disease (AD) and Alzheimer's disease-related dementias (ADRD) are multifactorial neurodegenerative disorders driven by complex interactions among genetic susceptibility, aging, and environmental exposures. Growing epidemiological and mechanistic evidence implicates neurotropic viral exposomes, defined as cumulative lifetime viral infections, as significant contributors to AD risk. Viral encephalitis and common viral infections, including herpes simplex virus type 1 (HSV-1), human immunodeficiency virus (HIV), cytomegalovirus (CMV), SARS-CoV-2, and influenza, have been a

Chronic THC exposure modulates behavioral outcomes and endocannabinoid signaling in HIV-1 Tg26 mice in a sex-dependent manner.

While combined antiretroviral therapy (cART) has transitioned HIV-1 into a manageable chronic condition, it fails to eradicate latent viral reservoirs in the central nervous system (CNS) that drive persistent neuroinflammation and synaptodendritic injury. Consequently, people living with human immunodeficiency virus type-1 (HIV-1) often utilize cannabis to manage neurological symptoms, yet the long-term impact of exogenous cannabinoids on the HIV-1-burdened brain remains poorly understood. In this study, we utilized the HIV-1 Tg26 mouse model to evaluate how chronic Δ9-tetrahydrocannabinol (TH

Impact of HIV on Neuronal Health in the Brains of People with HIV.

Despite effective viral suppression with antiretroviral therapy (ART), people with HIV (PWH) remain at increased risk of neurocognitive impairment and persistent neuroinflammation. Although neurons are not infected by HIV, they remain susceptible to the indirect effects of chronic infection, including exposure to viral proteins, inflammatory mediators, and glial dysfunction. The resulting neuronal injury is thought to contribute to the development and persistence of HIV-associated neurocognitive disorders in the ART era. This review examines the impact of chronic HIV infection on neuronal func
