[Corpora oryzoidea--rice bodies or melon seed-like bodies].

Two cases of bursitis complicated by rice bodies (C.O., corpora oryzoidea) are described. One case occurred after local elbow trauma, in the other case tuberculous bursitis was found. C.O. is a rare manifestation of fibrinous inflammation, seen not only in TB but also, perhaps more frequently, in rheumatoid conditions. Removal of the bursa and C.O. followed by microscopic investigation is recommended.

Complications of trochanteric osteotomy.

Trochanteric osteotomy continues to have an important role in total hip replacement, but more so as a useful adjunct in selected instances rather than as a necessary and integral part of every arthroplasty. Improved exposure is the most important benefit of and indication for trochanteric osteotomy. Trochanteric nonunion per se has little adverse effect on the final result, but most clinically significant complications of trochanteric osteotomy are a consequence of nonunion. Migration or total separation of the greater trochanter is usually preceded by nonunion. Either can result in impaired a

Bacterial arthritis.

The 1991 literature on septic arthritis included a concise review of adult septic arthritis, examples of pseudoseptic arthritis, and two interesting animal studies. One animal study examined the induction of acute synovitis by the intra-articular injection of bacterial endotoxin and the cytokines tumor necrosis factor-alpha, and interleukin-1 beta; and the other studied the effects of early and delayed synovectomy in the management of septic arthritis. The predispositions to septic arthritis can be divided into local joint abnormalities, systemic factors, or both. Examples of the local joint a

Leukemia, lymphoma, and multiple myeloma following selected medical conditions.

The role of selected prior medical conditions in the etiology of hematopoietic malignancies was examined in a case-control study of members of two regional branches of the Kaiser Permanente Medical Care Program (USA). Past history of chronic infectious, autoimmune, allergic, and musculoskeletal disorders was abstracted from medical records for leukemia (n = 299), non-Hodgkin's lymphoma (NHL, n = 100), and multiple myeloma (n = 175) cases and matched controls (n = 787). Little difference was found between cases and controls for most of the chronic conditions evaluated, including sinusitis, carb

Osteomyelitis of the patella in children.

The early diagnosis of osteomyelitis of the patella in children can be difficult due to the rarity of this infection and to the vagaries of presentation which may be either acute with systemic signs or insidious with mild local signs. We present four cases of patellar osteomyelitis to illustrate the spectrum of this disorder. There should be a high index of suspicion with persistent pain and swelling above the knee, septic prepatellar bursitis, and septic knees that do not respond to conventional management. Management parallels treatment of osteomyelitis of other bones.

Septic bursitis: experience in a community practice.

We reviewed 47 episodes of septic bursitis occurring in a private community medical practice. Most patients were male (85%), and roughly half (49%) the cases were related to recreational or occupational trauma. About 72% of cases were located in the olecranon bursa, while the remaining cases were prepatellar. Prepatellar bursitis patients were more likely to be hospitalized. Staphylococcus aureus was isolated from 70% of bursal fluid aspirations; other etiologic organisms included gram negative bacteria and Mycobacterium marinum. The majority of patients were able to be treated as outpatients

Experimental chronic Lyme borreliosis in Lewis rats.

The course of Lyme borreliosis in LEW/N rats inoculated intraperitoneally as infants with 10(6) Borrelia burgdorferi was followed for 360 days. Spirochetes were detected in the blood through 30 days, in the brain through 60 days, and persisted in the spleen, liver, kidneys and articular tissue through 360 days. Acute exudative arthritis, tendonitis, and bursitis were evident in multiple joints by day 30. Arthritis regressed thereafter but capsular fibrosis and lymphoplasmacytic infiltrates persisted throughout the study. Several rats developed exacerbations of acute arthritis within days 180-3

[Protothecosis. A new, or rarely recognized disease?].

Two cases of infection caused by Prototheca, belonging to the colourless algae, are discussed. In one of the patients colpitis, in the other one disease of the cubital bursa developed due to Prototheca. In the first case recovery was attained with tetracyclin, in the second case with surgical treatment. Microbiology, epidemiology, pathology and clinical aspects of the insufficiently known in Hungary protothecosis are outlined.

Septic bursitis in human immunodeficiency virus infection.

Septic bursitis is a well described entity in immunocompromised patients. Common factors contributing to an immunocompromised state are alcoholism and corticosteroid therapy. We describe 2 patients with Staphylococcus aureus olecranon bursitis with evidence of human immunodeficiency virus (HIV) infection and no other contributing factors to immunodeficiency. The clinical course in these patients was refractory to conservative management. Such a persistent course of septic bursitis, especially in the absence of other contributing factors, should raise the possibility of HIV infection.

Light-microscopic lesions associated with naturally occurring bursal cryptosporidiosis in chickens.

Information concerning histologic significance of naturally occurring bursal Cryptosporidium sp. infection in chickens was obtained by retrospective examination of histopathology reports and clinical data from histology accessions received during 1986. Cryptosporidiosis was diagnosed in 197 bursas. In two-thirds of the accessions, more than 50% of the examined bursas were infected with Cryptosporidium sp. The histologic morphologic lesion diagnosis for Cryptosporidium sp.-infected bursas most often was marked diffuse chronic-active superficial purulent protozoal bursitis with mucosal epithelia

Infections caused by Mycobacterium szulgai in humans.

Mycobacterium szulgai is a scotochromogenic species that has recently been recognized as a human pathogen. Twenty-four cases of disease caused by M. szulgai in humans have been reported in the English-language literature. The clinical features of these cases were reviewed, and three additional cases (two pulmonary, one extrapulmonary) were studied. Pulmonary disease indistinguishable from that caused by Mycobacterium tuberculosis was the commonest type of infection caused by M. szulgai (18 of 27 cases). Olecranon bursitis was reported in three cases, and disseminated infection was noted in thr

Postoperative osteomyelitis following implant arthroplasty of the foot: diagnosis with indium-111 white blood cell scintigraphy.

Many complications can occur following insertion of silicone elastomer implants into the foot. Postoperative infection may be difficult to distinguish from other conditions such as dislodgment, fracture, ectopic and heterotopic new bone formation, synovitis, and bursitis. White blood cell scintigraphy, in conjunction with the clinical scenario, may prove to be an invaluable tool in the diagnosis of postoperative osteomyelitis, subsequent to implant arthroplasties.

Morbidity from repetitive knee trauma in carpet and floor layers.

Carpet layers comprise less than 0.06% of the United States workforce yet they submit 6.2% of compensation claims for traumatic knee inflammation. Their work involves multiple sources of acute and chronic knee trauma including kneeling, pressure from sharp objects, and use of a device called a "knee kicker" to stretch wall to wall carpet. To characterise the knee morbidity in carpet layers and to identify occupational risk factors, a questionnaire was completed by 112 carpet and floor layers, 42 tile and terrazo setters, and 243 millwrights and bricklayers (MWBL). The MWBL comparison workers s

Predisposing factors, bacteriology and antibiotic therapy in 35 cases of septic bursitis.

In a retrospective study the etiology was verified in 35/52 patients with suspected septic prepatellar or olecranon bursitis. Staphylococcus aureus was the most common pathogen and 86% of them were penicillinase-producing. Other strains were streptococci. Mean age was 47 (18-83) years and males predominated (91%). Predisposing factors were seen in most cases. Preceding trauma was found in 27 and/or associated diseases (e.g. diabetes mellitus) in 11 patients. Hyperglycemia was found in 38% of the patients. Treatment with antimicrobial agents, mostly penicillins, and bursal drainage were success

Prepatellar bursitis in wrestlers.

As part of a larger retrospective study examining all knee injuries sustained by the University of Iowa wrestling team over 6 years, prepatellar bursitis was found to be the most frequent injury and, therefore, was examined in depth. Of the 136 wrestlers studied, 13 developed an initial case of prepatellar bursitis. This represented 21% of all initial episodes of knee injuries. Five of these wrestlers had no recurrences, but the other eight together had 20 recurrences. Median time lost for the initial injury was only 4 days, but recurrences and surgeries added significantly to the total time l

Aseptic arthritis in a man with toxic shock syndrome.

Synovitis in toxic shock syndrome (TSS) is an unusual finding. A 31-year-old man presented with pain and swelling in both knees and was found to have TSS, secondary to a septic bursitis caused by Staphylococcus aureus. Immune complexes were not detected in serum or synovial fluid, and the S aureus was not recovered from the inflamed joints. Antibodies against the TSS toxin-1 were detected in serum and synovial fluid, but in lower levels than would be seen in a normal control serum. Complement studies implicated alternative pathway activation by a marked diminution in C3 levels compared with C4

[Electron microscopic research on an experimental infection in chicks caused by the infectious bursal virus].

A total of 18 birds at the age of 25 days were infected intraocularly with the pathogenic strain 52/70 of the infectious bursitis virus. The bursa of Fabricius and the thymus were sampled for electron-microscopic investigations. The ultrastructural changes in the infected cells consisted in pycnosis of the lymphocyte and macrophage nuclei, vacuolization of the cytoplasm, and the production of lipid droplets as well as of inclusion bodies of varying density, myelin figures, multivericular bodies, and other irregular structures in the cytoplasm. These changes were slightly visible at the 24th ho

Treatment of skin, skin structure, bone, and joint infections with ceftazidime.

The collective experience with ceftazidime in the treatment of skin, soft tissue, bone, and joint infections is presented. Patients were treated with dosages ranging between 25 and 150 mg/kg per day for between five and 42 days. A total of 570 patients with skin and skin structure infections were treated with ceftazidime. Comparative studies, using either cefamandole or tobramycin plus ticarcillin as control drugs, included 239 patients. There were 600 evaluable patients in five categories of skin or skin structure infection: 252 patients had cellulitis, 107 had wound infections, 103 had absce

Overuse injuries in sports. A review.

Because knowledge of overuse syndromes is limited, the diagnosis and treatment of these conditions are a challenge to sports medicine physicians. Trial and error methods of treatment and too little attention to basic research have resulted in less than optimum solutions. We do know that these maladies most frequently result from overload or repetitive microtrauma stemming from extrinsic factors such as training errors, poor performance, poor techniques and inappropriate surfaces or intrinsic factors including malalignment and muscle imbalance. Overuse injuries involving the muscles include com
