Timentin, a combination of clavulanic acid (0.1 g) and ticarcillin (3.0 g), has proved effective in vitro against bacterial pathogens that produce beta-lactamases. The usual etiologic bacteria of osteochondritis of the foot (Pseudomonas species) and osteomyelitis/septic arthritis (Staphylococcus aureus) are commonly resistant to penicillins. The diagnosis of septic infections of closed body cavities requires a careful search. Traditional laboratory tests such as Gram's stain, white cell count, and protein and glucose levels are often inconclusive. Measurement of lactic acid in cerebrospinal, synovial, pleural, ascitic, and bursal fluids has been utilized to distinguish bacterial from nonbacterial infections. The present review summarizes the current status of lactic acid measurement in the differential diagnosis of meningitis, arthritis, empyema, bacterial peritonitis, and bursitis.

Fifty-five patients with osteoarthritis, rheumatoid arthritis or spondyloarthritis, joint pain and other rheumatic-inflammatory disorders were treated with proglumetacin, at different doses, during 3-60 days. Assessment of therapeutic efficacy indicated 88.9% of good results. Joint pain progressively decreased during the whole period of observation. Tolerance was considered good or very good in 78% of patients.

A previously healthy 65-yr-old man developed clinical signs of prepatellar bursitis. Aerobic cultures from fluid aspirated from the inflamed prepatellar bursa were negative. Direct microscopic examination of the fluid showed gram-positive cocci, and anaerobic cultures grew Streptococcus hemalyticus group A. Two cases of extraarticular Group G streptococcal infection are reported: one patient suffered from tenosynovitis of the extensor digitorum communis tendons of a wrist and the other from a prepatellar bursitis. In both cases penicillin therapy rapidly cleared all signs of infection.

We evaluated the efficacy and toxicity of moxalactam during treatment of 45 documented infections in 36 patients. A majority of patients received 4.5 g of moxalactam per day. Over a period of four years, seven of 110 maintenance hemodialysis patients developed olecranon bursitis. In all patients it appeared in the arm on which AV access was constructed but one patient also had bilateral bursitis. None of the patients gave a history of trauma or gout. The safety and efficacy of treatment with cefamandole were evaluated in 77 patients (from 33 institutions) with serious bone and joint infections. The antibiotic was given intramuscularly or intravenously in doses ranging from 1.5 to 12 g/day for 6 to 58 days. A severe attack of acute polyarthritis following a verified Campylobacter jejuni enteritis is described in a 12-year-old boy. The patient possesses the HLA-B27 antigen--often found in postinfectious arthritis following acute enteric infections. The ASO titre showed a significant rise, but other serological findings and the clinical course made streptococcal infection unlikely. Investigations to elucidate cross-reactivity between the two micro-organisms turned out negative.

Sixteen cases of bacterial infection of subcutaneous bursae were reviewed. Septic subcutaneous bursitis usually occurred following injury to the skin overlying the bursa. Two cases developed after bursal corticosteroid injection for treatment of aseptic bursitis. Bursal fluid culture yielded staphlococci in 14 cases and streptococci in 2. This study presents the frequency of severe and mild talalgias in unselected, consecutive patients with rheumatoid arthritis, ankylosing spondylitis, Reiter's syndrome, and generalized osteoarthosis. Fungal infections infrequently involve the joints. Review of the literature reveals that Candida arthritis is rare, that it is usually a complication of disseminated candidiasis and that it occurs as a primary joint infection without spread from adjacent osteomyelitis.
