Successful endovascular management of popliteal artery adventitial cystic disease with expanded polytetrafluoroethylene stent graft reinforced with interwoven nitinol stent.

Popliteal adventitial cystic disease has traditionally been treated with open surgery of cyst excision, patch repair of artery, or interposition graft. Endovascular angioplasty or stenting has demonstrated poor mid- to long-term results because high-viscosity tension of the adventitial cyst continues to exert extrinsic compression, resulting in early elastic recoil and disease recurrence. To our knowledge, this is the first report in the literature of an effective endovascular strategy using a popliteal stent graft reinforced with an interwoven nitinol stent to treat popliteal cysts, with favo

Case Report: Arthroscopic management of a giant Baker cyst containing numerous rice body-like loose bodies in a patient with rheumatoid arthritis.

BACKGROUND: Giant Baker cysts containing numerous rice body-like loose bodies are uncommon and can resemble vascular, infectious, or neoplastic lesions. CASE PRESENTATION: A 30-year-old woman with a 4-year history of rheumatoid arthritis presented with 1 year of progressive left knee pain, restricted flexion, and a 15 × 8 cm nonpulsatile mass extending from the popliteal fossa into the proximal calf. Magnetic resonance imaging showed a well-defined cystic lesion containing numerous nonmineralized intraluminal nodules. Arthroscopy demonstrated diffuse hypertrophic synovitis and multiple smooth

Case Report: Postoperative pulmonary embolism causing obstructive shock and circulatory collapse after gynecologic surgery: successful rescue with VA-ECMO.

Very-high-risk pulmonary embolism (PE) occurring within hours after major surgery presents a therapeutic dilemma because urgent reperfusion may be required at precisely the time when bleeding risk is greatest. We describe a 45-year-old woman who developed catastrophic bilateral PE approximately two hours after elective gynecologic surgery consisting of myomectomy and ovarian endometriotic cyst excision. Shortly after surgery, she experienced abrupt respiratory failure, profound hypotension, and rapidly progressive circulatory collapse. On arrival at a tertiary intensive care unit, arterial blo

Ruptured Baker's Cyst Demystified: Current Evidence, Diagnostic Strategies, and Treatment Options for an Under-Recognized Condition.

Rupture of a Baker's cyst is an under-recognized condition that may closely mimic deep vein thrombosis, often leading to diagnostic uncertainty and delayed management. Although popliteal cysts are common findings associated with degenerative or inflammatory knee disorders, their rupture represents an uncommon but clinically significant complication. Ruptured cysts typically present with acute calf swelling and pain due to extravasation of synovial fluid into the intermuscular planes. The condition may mimic venous thrombosis (pseudo-thrombophlebitis), making diagnostic accuracy essential. Dupl

Enlarging, Multilobulated Popliteal Cyst After Total Knee Arthroplasty: A Case Report.

CASE: A 69-year-old woman presented with a 5-year history of an enlarging popliteal cyst 7 years after total knee arthroplasty (TKA). Magnetic resonance imaging revealed a large, multilobulated popliteal cyst. Initial diagnostic workup was inconclusive but repeat evaluation met criteria for culture-negative prosthetic joint infection (PJI). She underwent revision TKA, intraoperative cultures identified Micrococcus luteus, and she remains on antibiotic suppression. At her 4-year follow-up, she demonstrated complete resolution of the popliteal cyst with minimal pain. CONCLUSION: Popliteal cysts

Posterior Knee TGCT (Formerly PVNS) Masquerading as a Popliteal Lesion: Combined Arthroscopic Synovectomy and Posterior Open Excision With 8-Year Disease-Free Follow-Up.

Tenosynovial giant cell tumor (TGCT), formerly pigmented villonodular synovitis (PVNS), may extend into the popliteal fossa and mimic a Baker cyst, complicating diagnosis and operative planning. A 53-year-old former endurance runner presented with posterior knee pain, swelling, intermittent locking, and a palpable popliteal mass. Examination demonstrated effusion and limited range of motion; weightbearing radiographs showed mild osteoarthritic change with effusion, and magnetic resonance imaging (MRI) revealed synovial hypertrophy with low T2 signal suggestive of hemosiderin deposition. Venous Adventitial Cystic Disease of the Lower Extremities Mimicking Deep Vein Thrombosis: A Five-Case Series.

Venous adventitial cystic disease (VACD) is a rare vascular disorder characterized by mucin-filled cysts in the venous adventitia. We encountered five patients who presented to our vascular clinic with an initial diagnosis of deep vein thrombosis (DVT) but were later diagnosed with VACD on imaging. The patients were aged 64-81 years; lesion locations included the common femoral vein (n=3), external iliac vein (n=1), and popliteal vein (n=1). Four patients underwent surgical intervention, and one patient received conservative management with medical therapy. All surgically treated patients demo

Popliteal Artery Compression Caused by Intraneural Ganglion Cyst of the Tibial Nerve.

Intraneural ganglion cysts are rare, benign mucinous cysts that arise within the epineurium of peripheral nerves. They most commonly involve the peroneal nerve, whereas involvement of the tibial nerve is exceptionally rare. Patients typically present with neurological deficits involving motor and sensory function of the affected nerve. We present a case of an intraneural ganglion cyst of the tibial nerve manifesting primarily with atypical, vascular compression-like symptoms of intermittent claudication. A 43-year-old patient was referred to our department with a 1-year history of exertional,

Two cases of symptomatic adventitial cystic disease involving the popliteal artery and femoral vein.

Adventitial cystic disease is a rare condition involving mucinous cysts within the adventitial layer of arteries or veins. We report two distinct cases: one in the popliteal artery presenting with intermittent claudication and another in the femoral vein presenting with limb swelling and pain. The diagnosis was established based on characteristic imaging findings, and both lesions were completely excised without recurrence. The arterial cyst contained clear gelatinous fluid, and the venous cyst contained hemorrhagic material, possibly related to anticoagulant use. Adventitial cystic disease sh

Diagnostic accuracy of physical examination for the assessment of popliteal cysts: a meta-analysis.

BACKGROUND: Popliteal cysts are common knee cystic lesions and a potential risk factor for knee osteoarthritis, but the vast majority are asymptomatic. Early diagnosis is crucial for effective management. Physical examination is a convenient, non-invasive, and cost-free diagnostic tool, but its accuracy for popliteal cysts remains controversial. This meta-analysis evaluates the diagnostic value of physical examination for popliteal cysts. METHODS: PubMed, Embase, Web of Science, China National Knowledge Infrastructure, and Wanfang databases were searched for studies on physical examination fo

Pseudoaneurysm of the Medial Genicular Artery as a Rare Cause of Recurrent Knee Swelling - A Case Report.

INTRODUCTION: Recurrent knee swelling in elderly or anticoagulated patients is commonly attributed to conditions, such as deep vein thrombosis (DVT), Baker's cyst rupture, septic arthritis, or anticoagulation-related hemarthrosis. However, vascular causes, such as genicular artery pseudoaneurysm, are rare and may be overlooked, especially when symptoms persist despite appropriate treatment. CASE REPORT: We describe a 60-year-old male with multiple comorbidities who presented with recurrent painful swelling of the right knee. Initial evaluations identified sequential pathologies, including pop

Double posteromedial portal arthroscopy vs. other arthroscopic techniques for Baker's cyst: a systematic review and meta-analysis.

BACKGROUND: Double posteromedial portal (DPP) arthroscopy has been proposed to improve cyst management in patients with Baker's cysts compared with other arthroscopic techniques. However, comparative evidence remains limited. METHODS: A systematic review and meta-analysis was conducted according to PRISMA guidelines. Comparative clinical studies assessing DPP vs. non-DPP arthroscopic techniques for Baker's cysts were identified from PubMed, Embase, Scopus, Web of Science Core Collection and the Cochrane Library. The primary outcome was cyst recurrence or residual cyst at final follow-up. Seco

An Abnormal Presentation of a Popliteal Artery Mass and a Unique Cause of Claudication.

Multiple hereditary exostosis (MHE) is a rare autosomal-dominant disorder marked by multiple osteochondromas arising from the metaphyses of long bones. Parameniscal cysts, often linked to meniscal tears, can also expand and compress nearby vessels, posing a potential vascular risk. We present a 28-year-old male with MHE and a parameniscal cyst causing popliteal artery compression. The patient's intermittent claudication resolved completely following surgical cyst decompression and meniscal repair. This case report highlights the importance of considering vascular compression in patients with M

Linking meniscal pathology to Baker's cyst formation: the role of tear type, location and chondral damage.

BACKGROUND: Although Baker's cysts are commonly associated with meniscal tears, the role of specific tear morphologies and anatomic locations in cyst presence remains unclear. This study aimed to evaluate the associations between meniscal tear patterns, tear locations, and Baker's cysts and to explore whether these tear characteristics were also associated with concomitant chondral lesions. METHODS: In this retrospective cohort study, patients who underwent knee arthroscopy at a single institution were evaluated and categorized according to the presence or absence of a Baker's cyst on MRI. Vascular and Neural Compression Syndromes Associated with Plantaris Muscle Variants: A Classification-Based Review.

Background: The plantaris muscle (PM) shows substantial variability in its proximal belly attachments. Although often deemed vestigial, specific variants may narrow or reshape the popliteal corridor and contribute to vascular (popliteal artery entrapment syndromes, PAES) and neural conflict (TN, CPN, sural nerves). Despite abundant anatomical descriptions of the plantaris, its contribution to neurovascular compression has not been organised into a classification-linked, imaging-integrated framework. Objective: To synthesise adult and foetal anatomical data with clinical-radiological evidence i

Rare Adventitial Venous Cyst Mimicking Deep Vein Thrombosis: A Diagnostic Pitfall-A Systematic Review of Diagnostic Challenges, Imaging Features, and Optimal Treatment Strategies.

Adventitial cystic disease (ACD) is a rare vascular condition, representing approximately 0.1% of all vascular diseases, with about 325 cases reported in the literature since its first description in 1947, including 72 venous cases. This study aims to highlight the diagnostic and therapeutic challenges of venous ACD, which is frequently misdiagnosed as deep vein thrombosis (DVT), femoral varices, aneurysms, venous tumors, or lymphadenopathy due to its rarity. Clinical, imaging, and treatment data from reported cases of venous adventitial cystic disease (VACD) were reviewed. The disease most co

Development and internal validation of a clinical nomogram for predicting vascular complications in patients with Baker's cysts: A retrospective cohort study.

BackgroundBaker's cysts can cause significant vascular complications including popliteal vein compression and deep vein thrombosis (DVT), yet no validated tool exists to predict which patients are at highest risk. We aimed to develop and internally validate a clinical nomogram for predicting vascular complications in patients with Baker's cysts.MethodsWe conducted a retrospective cohort study of 1,364 consecutive patients diagnosed with Baker's cysts between January 2015 and December 2024 at a single tertiary center. The primary outcome was a composite of vascular complications (symptomatic po

Post-Total Knee Replacement Persistent Bilateral Popliteal Cysts Requiring Surgical Excision: A Literature-Based Discussion.

We report a patient who developed the unusual complication of persistent symptomatic Baker's cyst (BC) requiring surgical excision following each of her total knee replacements (TKRs), which were performed 10 years apart. BCs are a common problem and rarely require operative intervention. This patient presented with osteoarthritis and a painful complex BC. A successful left TKR was performed but the BC persisted following the surgery. Conservative management of the BC after the TKR failed to alleviate the symptoms, and surgical excision of the BC was eventually performed. This patient experien

Exercise-induced popliteal artery compression due to a popliteal fossa cyst.

Nonatherosclerotic extrinsic compression from para-articular cysts is a rare but clinically important cause of claudication. We report the case of a 50-year-old man who presented with left calf claudication and absence of distal left dorsalis pedis artery pulse after exertion. A duplex ultrasound study revealed a popliteal fossa cyst that doubled in size under exertion (from 3.2 to 7.2 mm), leading to an extrinsic compression of the popliteal artery. Magnetic resonance imaging scans confirmed a possible articular origin of the cyst. Dense adhesions surrounding the lesion led to the reconstruct
