Group B Streptococcus-Associated Necrotizing Soft Tissue Infection Following Minor Ankle Trauma: A Case Report.

Necrotizing soft tissue infections (NSTIs) are rapidly progressive, life- and limb-threatening emergencies requiring immediate surgical source control. Group B Streptococcus (GBS; Streptococcus agalactiae) is a rare cause of NSTIs in adults. We present a 44-year-old woman with poorly controlled diabetes who developed progressive right foot pain and swelling after minor ankle trauma. Examination showed spreading erythema, crepitus, dusky discoloration, malodor, and a deep plantar wound, accompanied by marked leukocytosis and diabetic ketoacidosis. Computed tomography demonstrated infection exte

Fluorescence-Guided Surgery for Necrotizing Soft Tissue Infections: A Systematic Review.

Necrotizing soft tissue infections are life-threatening infections of soft tissue that are treated with rapid surgical debridement when the diagnosis is suspected. Necrotic tissue is removed until vital tissue is identified, guided by the surgeon's vision and hands. Fluorescence-guided surgery is increasingly popular for assessing tissue perfusion. This surgery may have the potential to improve the diagnosis and treatment of necrotizing soft tissue infections. A systematic review on fluorescence-guided surgery for necrotizing soft tissue infections was conducted according to the PRISMA guideli

Molecular Subtypes of Cholangiocarcinoma and Their Translational Implications.

Cholangiocarcinomas (CCAs) are cancers that arise in the peripheral or central bile ducts. They have substantial heterogeneity in their etiologies and histopathologic phenotypes, ranging from cholangiolar carcinomas arising from the smallest bile ducts or from dedifferentiating hepatocytes through cellular plasticity, to ductular cancers in medium and large-sized bile ducts. The surgical delineation of intrahepatic, perihilar, and distal CCAs does not match the biological classification, as intrahepatic CCAs include both small duct cholangiolar carcinoma and intermediate duct ductular or mucin

Cancer-Associated Fibroblasts Promote Epithelial-Mesenchymal Transition and Classical to Basal Subtype Shift in Pancreatic Cancer.

Environmental exposures represent a significant health hazard, which cumulatively may be responsible for up to 2/3 of all chronic non-communicable disease and associated mortality (Global Burden of Disease Study and The Lancet Commission on Pollution and Health), which has given rise to a new concept of the exposome: the sum of environmental factors in every individual's experience. Noise is part of the exposome and is increasingly being investigated as a health risk factor impacting neurological, cardiometabolic, endocrine, and immune health. Beyond the well-characterized effects of high-inte

Futibatinib after non-covalent FGFR inhibitors in FGFR2-rearranged intrahepatic cholangiocarcinoma: clinical activity and resistance patterns.

PURPOSE: The optimal sequencing of non-covalent and covalent FGFR inhibitors in FGFR2-rearranged intrahepatic cholangiocarcinoma (iCCA) remains undefined. Futibatinib, an irreversible FGFR1-4 inhibitor, may retain activity in the setting of acquired resistance to non-covalent FGFR inhibitors, but data on the patterns of acquired alterations are limited. METHODS: We conducted a retrospective multicenter study across three European centers including patients with advanced FGFR2-rearranged iCCA treated with futibatinib after progression on non-covalent FGFR inhibitors. Clinical outcomes and safe

Targeted-Penetrating Biomimetic Hybrid Transfersomes for Enhanced Transdermal Gene Transfection in Hypertrophic Scar Therapy.

Hypertrophic scar (HS) is a recurrent and inflammatory dermal fibrotic disorder, characterized by abnormal fibroblast proliferation and excessive extracellular matrix (ECM) deposition. Affecting over 310 million patients worldwide, while effective strategies to halt progression remain lacking. The TGF-β-mediated fibrotic cascade is the central driver of HS, and siRNA-based therapeutics offer a promising gene-level intervention owing to their high sequence specificity and low off-target risk. However, siRNA translation is limited by rapid degradation, poor transfection, insufficient targeting,

An Estrogen-IL-33 Circuit in Aortic Fibroblasts Underlies Sex Differences in Tregs in Abdominal Aortic Aneurysm.

Regulatory T cells (Tregs) are key components of the vascular immune microenvironment in abdominal aortic aneurysm (AAA), but their sexual dimorphism and underlying mechanisms remain unclear. This study aimed to characterize Treg sex dimorphism in AAA and delineate the governing hormonal and immunologic pathways. Female mice showed reduced aortic Treg infiltration with decreased activation markers (ST2, KLRG1) and the functional mediators interleukin (IL)-10 and trefoil factor 1. Transcriptomic profiling revealed that male aortic Tregs displayed authentic tissue-resident signatures, whereas fe

Spatially resolved multiomics of human meningeal development reveal lineage and disease dynamics.

The meninges are increasingly recognized as key stromal and immune niches supporting cranial development. Here we constructed a paired single-nuclei RNA and Assay for Transposase-Accessible Chromatin atlas of human meningeal development from 6 to 21 postconception weeks across cranial regions. By integrating spatial transcriptomics, we resolved cell types across meningeal layers. Our multiomic atlas defines gene regulatory networks underlying blood-brain barrier endothelial identity during development. Within immune compartments, the data support a yolk sac origin for meningeal myeloid lineage

Cross-kingdom RNA interference in the dysbiosis-mediated network of vocal fold fibrosis: a pathogenic hypothesis and molecular intervention targets.

Vocal fold fibrosis is the principal pathological basis of intractable voice disorders. Previous etiological models have attributed vocal fold scarring solely to mechanical trauma and chemical irritation, whereas the local mucosal microbiota of the vocal folds and its cross-kingdom regulation of host gene expression have long received little attention. Integrating interdisciplinary advances, this review proposes a cross-kingdom pathogenic hypothesis: phonotrauma induces laryngeal dysbiosis and disassembly of the vocal fold epithelial tight junctions, and opportunistic pathogens under stress re

Endoplasmic reticulum stress and maladaptive repair in fibrotic interstitial lung diseases: A human-evidence-based narrative review.

Fibrotic interstitial lung diseases (ILDs) are characterized by progressive extracellular-matrix accumulation but arise from biologically diverse injuries. This narrative review examines endoplasmic reticulum (ER) stress as a context-dependent regulator of maladaptive repair, prioritizing evidence from human genetics, tissue, single-cell and spatial studies, patient-derived systems, biomarkers, and clinical trials. In selected familial surfactant disorders, pathogenic variants provide direct evidence that impaired proteostasis can cause fibrotic lung disease. In sporadic idiopathic pulmonary f

Biomarkers in predicting mortality in hemodialysis patients: recent advances.

Patients undergoing maintenance hemodialysis face annual mortality rates of 15-27%, with cardiovascular causes accounting for more than half of all deaths. Despite advances in dialytic technology, conventional prognostic models derived from the general population systematically underestimate risk in end-stage renal disease, failing to capture the complexity of uremic pathophysiology that drives this excess mortality: immune dysregulation, chronic inflammation, oxidative stress, vascular calcification, uremic cardiomyopathy, protein-energy wasting, and accelerated vascular aging. This narrative

Androgen Receptor-Immune Crosstalk in Metastatic Castration-Resistant Prostate Cancer: From Context-Dependent Remodeling to Biomarker-Guided Translation.

BACKGROUND: Metastatic castration-resistant prostate cancer (mCRPC) rarely responds to immune checkpoint blockade despite clear evidence that androgen receptor (AR) signaling intersects with tumor immunity. The view that androgen deprivation uniformly enhances antitumor immunity does not explain the repeated failure of immunotherapy combinations in biomarker-unselected populations. METHODS: This narrative review synthesizes mechanistic, translational, and clinical evidence and organizes it by cellular compartment, treatment-associated immune state, clinical trial strategy, and biomarker readi

Gut microbiota and polyendocrine metabolic ovarian syndrome: an integrated gut-metabolism-endocrine-ovary axis.

Polyendocrine metabolic ovarian syndrome (PMOS), previously known as polycystic ovary syndrome (PCOS), is a common endocrine and metabolic disorder in reproductive-age women, characterized by marked clinical and biological heterogeneity. Accumulating evidence suggests that gut microbiota dysbiosis is associated with metabolic disturbances, hormonal imbalance, and ovarian dysfunction in PMOS. However, the pathways linking gut microbiota alterations to PMOS pathogenesis remain incompletely understood, and most evidence remains associative. This review aims to summarize current evidence regarding

A comparative study of axillary and innominate artery cannulation: insights from clinical experience in acute type A aortic dissection.

INTRODUCTION: Stanford acute type A aortic dissection (ATAAD) repair requires complex circulatory and cerebral protection. Although various neuroprotective techniques have been proposed, the optimal strategy remains controversial. AIM: This study compared innominate artery (IA) and right axillary artery (AXA) cannulation in ATAAD repair based on early- and mid-term outcomes. MATERIAL AND METHODS: Forty-six patients who underwent emergency ATAAD repair between January 2021 and April 2024 were retrospectively analyzed. Group 1 included patients with AXA cannulation (n = 18), and group 2 consis

Aperiodic EEG spectral features as consistent indicators of Parkinson's disease: a machine learning approach utilizing fitting oscillations and one-over-F decomposition.

A major challenge in Parkinson's disease (PD) research is the identification of non-invasive neurophysiological markers that can distinguish PD patients from healthy controls while providing interpretable information about cortical dysfunction. This study investigates whether aperiodic EEG spectral parameters extracted using Fitting Oscillations and One-Over-F decomposition can serve as candidate indicators for exploratory PD classification and medication-state analysis. Resting-state EEG data from 15 PD patients and 16 healthy controls were analyzed. For PD participants, both medicated and un

Impact of real-world confounders on the accuracy of an AI model to support read out of skin prick automated test results.

γ-Herpesviruses (γ-HVs) are notable for their ability to establish latent infections of lymphoid cells(1). The narrow host range of human γ-HVs, such as EBV and KSHV, has severely hindered detailed pathogenic studies. Murine γ-herpesvirus 68 (γHV68) shares extensive genetic and biological similarities with human γ-HVs and is a natural pathogen of murid rodents(2). As such, evaluation of γHV68 infection of mice inbred strains at different stages of viral infection provides an important model for understanding viral lifecycle and pathogenesis during γ-HVs infection. Upon intranasal inoculation,
